Risk factors and poor prognostic factors of preeclampsia in Ibn Rochd University Hospital of Casablanca: about 401 preeclamptic cases.

Preeclampsia is a gestational pathology that complicates 2 to 8% of pregnancies and is one of the major causes of maternal and fetal morbidity and mortality worldwide. The aim of this work was to study the epidemiological profile of preeclampsia in Casablanca and to identify risk factors as well as factors of poor maternal and fetal prognosis. 401 preeclamptic cases were collected in the gynecology-obstetrics "C" Service of Lalla Meryem Maternity of Ibn Rochd University Hospital of Casablanca (2010-2011) were included in this study and a statistical analysis with the SPSS software version (16.

Factors associated with postpartum hemorrhage in selected Southern Oromia hospitals, Ethiopia, 2021: an unmatched case-control study.

BACKGROUND: Around one-fourth of maternal deaths worldwide are attributed to hemorrhage. One of the prevalent types of maternal hemorrhage is postpartum hemorrhage. In spite of this, there is very little data on postpartum hemorrhage. Thus, the intention of this study was to determine factors associated with postpartum hemorrhage among mothers who gave birth in the selected Southern Oromia hospitals in Ethiopia. METHODS: An institutional-based, unmatched case-control study was employed on a sample of 333 (83 cases and 250 controls) mothers who gave birth from March 1-30, 2021. Standardized an

Side effects from epidural analgesia in laboring women and risk of cesarean delivery.

BACKGROUND: Epidural analgesia may cause maternal hypotension and changes in the fetal heart rate. The implications of such side effects on the course of labor and delivery are incompletely understood. OBJECTIVE: This study aimed to assess whether the occurrence of maternal or fetal side effects associated with labor epidural analgesia increased the risk for cesarean delivery. STUDY DESIGN: This was a cohort study of all women who underwent epidural analgesia during labor for the period October 1, 2020 to December 31, 2020. Excluded were cases of multiples, fetal death, noncephalic presentat

A survey of pregnant women's attitude towards breech delivery and external cephalic version.

A structured interview survey was carried out in 150 women who came for their first antenatal visit in a university hospital in Hong Kong between June and July 1998. Their opinions and perceptions of fetal and maternal safeties on different modes of delivery for both cephalic and breech presentation, and external cephalic version (ECV) were surveyed. Their decisions on the management of term breech-presenting pregnancy were examined. Most women (92%) preferred vaginal delivery to Caesarean delivery (CS) in case of cephalic presentation, mainly because it was a natural way of parturition. They

Intra-operative fetal monitoring during non-obstetric surgery for complications after bariatric surgery: a standard operating procedure.

INTRODUCTION: Pregnancy after bariatric surgery is a high-risk pregnancy. Post-bariatric complications may require emergency surgical intervention. During laparoscopic surgery, anesthetic drugs and formation of pneumoperitoneum influence maternal hemodynamics and may impair the perfusion of the utero-placental unit. Subsequent changes of the fetal heart rate (FHR) might be recognized with intra-operative fetal monitoring and guide maternal positioning and hemodynamic management to maintain fetal wellbeing. However, current fetal monitoring guidelines do not include FHR interpretation during ge

[The clininal management of breech presentation].

During the period 1969--1973, a total of 7933 deliveries was carried out at the First University Clinic of Obstetrics and Gynaecology in Vienna. Of these, 328 deliveries were breech presentations and 260 were single-born. The frequency of Caesarean sections in the case of breech presentations was 16,6%, i.e. about four times higher than the average frequency of Caesarean sections in relation to the overall number of obstetric patients of our clinic. The high frequency of premature deliveries in breech presentation (18,46%) is reflected in the perinatal mortality rate; 5,67% of all children bor

Human Placental Growth Hormone Variant in Pathological Pregnancies.

Growth hormone (GH), an endocrine hormone, primarily secreted from the anterior pituitary, stimulates growth, cell reproduction, and regeneration and is a major regulator of postnatal growth. Humans have two GH genes that encode two versions of GH proteins: a pituitary version (GH-N/GH1) and a placental GH-variant (GH-V/GH2), which are expressed in the syncytiotrophoblast and extravillous trophoblast cells of the placenta. During pregnancy, GH-V replaces GH-N in the maternal circulation at mid-late gestation as the major circulating form of GH. This remarkable change in spatial and temporal GH

The effect of magnesium sulfate on the biophysical profile of normal term fetuses.

Forty term pregnant women with singleton breech gestations admitted for external cephalic version underwent biophysical profile testing before any fluid infusion or medication. After magnesium sulfate had been infused for contraction prophylaxis, the maternal serum magnesium level was measured and a second biophysical profile was performed. The mean (+/- SD) serum magnesium was 5.1 +/- 1.0 mg/dL. The biophysical profile score decreased significantly, reflecting a decrease in fetal breathing activity. In patients with therapeutic maternal serum magnesium levels, loss of any component of the bio

Nocturnal incontinence in children with fetal alcohol spectrum disorders (FASD) in a South African cohort.

INTRODUCTION: Fetal alcohol spectrum disorders (FASD) are one of the leading preventable causes of intellectual disabilities (ID). Not much is known about the topic of pediatric incontinence related to FASD, for example nocturnal enuresis (NE), daytime urinary incontinence (DUI), and fecal incontinence (FI). So far, incontinence problems have been examined among children with other specific syndromes. OBJECTIVE: The aim of the present study is to investigate the possible presence of incontinence among children with FASD in a South African cohort. STUDY DESIGN: The South African version of th

A randomized controlled trial of prophylactic maneuvers to reduce head-to-body delivery time in patients at risk for shoulder dystocia.

OBJECTIVE: To assess whether prophylactic use of the McRoberts maneuver and suprapubic pressure decreased the head-to-body time, as a proxy for shoulder dystocia, in at-risk patients. METHODS: Patients with estimated fetal weights over 3800 g were randomized to undergo the McRoberts maneuver and suprapubic pressure before delivery of the fetal head (prophylactic maneuvers) or to undergo maneuvers only after delivery of the head, if necessary (controls). A total of 185 patients were enrolled in the study. After exclusions (eg, abdominal delivery), there were 128 evaluable vaginal deliveries. T

Prophylactic manual rotation of occiput posterior and transverse positions to decrease operative delivery: the PROPOP randomized clinical trial.

BACKGROUND: Persistent occiput posterior and occiput transverse positions are the most common malpositions of the fetal head during labor and are associated with prolonged second stage of labor, cesarean deliveries, instrumental deliveries, severe perineal tears, postpartum hemorrhage, and chorioamnionitis. Manual rotation is one of several strategies described to deal with these malpositions. OBJECTIVE: This study aimed to determine if the trial of prophylactic manual rotation at the early second stage of labor is associated with a decrease in operative deliveries (instrumental and/or cesare

The Association between Oligohydramnios and the Risk of Malpresentation: A Meta-Analysis.

BACKGROUND: There is no consensus on the role of oligohydramnios on the increased risk of fetal malpresentation. Therefore, we decided, for the first time, to collect all published studies on this topic to synthesize all available data through a systematic review and meta-analysis to evaluate the effect of oligohydramnios on the increased risk of fetal malpresentation. METHODS: A search was done in the following international electronic bibliographic databases: PubMed, Web of Science, and Scopus to April 2021. Heterogeneity among studies was determined by the Q-test and I2 statistic. Publicat

Treatment for patients with recurrent fetal losses positive for anti-cardiolipin beta2 glycoprotein I antibody using Sairei-to (Chai-ling-tang) and low-dose aspirin.

AIM: Recently, it is widely recognized that positivity for anti-phospholipid antibodies is a causative factor for a range of reproductive failures. Anti-cardiolipin beta2 glycoprotein I antibody (anti-CL-beta2-GPI) is a representative anti-phospholipid antibody, which strongly correlates with the development of thrombotic events and diversity of adverse pregnancies. In this series, we aimed to elucidate effective treatment for patients with recurrent fetal losses positive for anti-CL-beta2-GPI using Japanese-modified Chinese herbal medicine. METHODS: Twenty-one patients with recurrent fetal l

From womb to words: the sex-specific interplay of fetal sex hormones and maternal mood on infant language development.

INTRODUCTION: Language development is influenced by biological and environmental factors, including infant hormonal status and maternal mental health. Previous research on the role of infant sex hormones in language development focused on estradiol and testosterone, yet first evidence indicates that dehydroepiandrosterone (DHEA), the dominant fetal steroid hormone, may be a more sensitive biomarker for language development by shaping the organization of the developing brain. Concerning infants' language-learning environment, maternal well-being is a key factor, with maternal depressed mood pos
