Microsurgical Treatment of Vertebral Artery Dissection: Surgical Strategies and Treatment Outcomes.

BACKGROUND: Vertebral artery dissection (VAD) is a rare cerebrovascular disease that can lead to permanent morbidity or mortality. Open surgery for VAD is challenging; therefore, most cases are managed via endovascular techniques. There are several surgical methods for VAD treatment, including trapping or proximal occlusion with or without bypass; however, the standard treatment protocol is not well established. The aims of this study were to demonstrate surgical strategies, surgical outcomes, and complications of each method and to propose an algorithm to select the appropriate procedure. ME…

Spontaneous vertebral artery dissection in an elder patient: a case report.

Vertebrobasilar artery dissection (VBD) is a rare cause of posterior ischemic strokes. It is more likely to occur in young patients with a history of traumatism or hereditary connective tissue disorders. Spontaneous VBD is rare, especially in elder patients. This paper aims to report the case of an old patient that presented with a posterior ischemic stroke due to a spontaneous vertebral artery dissection (SVAD), linked to atherosclerosis. The diagnosis of SVAD was made by Magnetic resonance imaging (MRI) which is the gold standard technique to diagnose this pathology in a short time so the pa…

Intracranial vessel wall MR imaging of an intradural vertebral artery dissection.

Diagnosis of intracranial arterial dissections can be challenging due to the wide spectrum of imaging presentations. High-resolution vessel wall MR imaging can be a useful adjunct to conventional lumen-based imaging techniques for diagnosing arterial dissections. We present a case of a 37-year-old male with a history of a Wolff-Parkinson-White syndrome presenting with acute onset of nausea, vertigo, and left body hemisensory loss of pain and temperature. A conventional brain MRI identified an acute infarct in the right lateral medulla, concordant with clinical symptoms of Wallenberg syndrome.

Postpartum vertebral artery dissection: case report and review of the literature.

BACKGROUND: Hypertensive disorders of pregnancy are associated with vascular complications, including ischemic stroke and cervical artery dissection. Vertebral artery dissection (VAD), however, is rare. We describe a 31-year-old female who presented with vertigo, nausea, and vomiting and was found to have a VAD. In addition, we discuss the presentation, differential diagnosis, and pathogenesis of this uncommon but clinically significant vascular event and summarize other cases of vertebral artery dissection described in the medical literature. CASE PRESENTATION: A 31-year-old Hispanic woman p…

Impact of initial symptom for accurate diagnosis of vertebral artery dissection.

BACKGROUND: It has been recognized that spontaneous vertebral artery dissection without neurological symptoms is not rare and easily misdiagnosed. Clinical clue for diagnosis of vertebral artery dissection includes initial symptoms such as headache, neck pain, or dizziness. AIM: To assess the role of initial symptoms for diagnosis of spontaneous vertebral artery dissection. METHODS: Between September 2007 and January 2014, we retrospectively reviewed clinical records of 83 patients with unilateral vertebral artery dissection without consciousness disturbance at admission. Based on the diagno…

Vertebral artery dissection in patients with autosomal dominant polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited renal cystic disease, and it is associated with various extrarenal manifestations, including vascular complications, such as intracranial aneurysms, and aortic root dilatation and aneurysms. However, intracranial arterial dissection has rarely been reported. We herein report the cases of 2 patients with ADPKD who developed a vertebral artery (VA) dissection. Dissection was also observed on the other side of the VA and in the internal carotid artery in the first and second patient, respectively. Both patients also

Vertebral artery dissection associated with generalized convulsive seizures: a case report.

A 46-year-old male with juvenile myoclonic epilepsy was admitted to the neurological department for convulsive seizures just after lamotrigine was discontinued. On admission he was awake but had a right-sided hemiparesis with Babinski sign and ataxic finger-nose test on the left side. An MR scan showed a left-sided pontine infarction, an infarct in the left cerebellar hemisphere and a right vertebral artery dissection (VAD). The patient was treated with heparin and an oral anticoagulant for 6 months. Recovery of neurologic function was excellent. In patients with symptoms of disturbances of po…

[Vertebral artery dissection: peculiarities of clinical and magnetic resonance imaging manifestations].

The aim of this study was to analyze clinical and neuroimaging manifestations of vertebral artery (VA) dissection. Twenty seven patients (19 men, 8 women, mean age 34.1±6.1) with the VA dissection were enrolled. All the patients underwent MRA, neck MRI (T1 f-s, T2 f-s) and head MRI. Twenty six of 27 patients had follow-up MRA/MRT studies: during the acute period and after 2 months or later. Clinical manifestations of VA dissection included ischemic stroke (IS, 52%), transient ischemic attack (TIA, 4%) and isolated neck pain/headache (44%). Ninety two percent of patients in the group of patient

Intravascular ultrasonography-guided stent angioplasty of an extracranial vertebral artery dissection.

The authors report on a case of intravascular ultrasonography (IVUS)-guided stent angioplasty for iatrogenic extracranial vertebral artery (VA) dissection in a 49-year-old man after coil embolization for an unruptured aneurysm of the right posterior inferior cerebellar artery. Insignificant dissections occurred during the procedure. Postoperatively, the patient experienced gradually worsening posterior neck pain and headache, and follow-up angiography 8 months after the coil embolization revealed expansion of the dissection. The patient underwent stent angioplasty with IVUS guidance and his sy

CT angiography in the diagnosis of spontaneous extracerebral vertebral artery dissection.

Spontaneous vertebral artery dissection is an underrecognised cause of cerebral ischaemia, most often seen in young adult patients. For this diagnosis cerebral angiography is the most sensitive and specific method. We describe a 30-year-old, male, with cerebellar stroke, in whom spontaneous vertebral artery dissection was diagnosed. Our diagnosis was confirmed by cerebral angiography and CT angiography. In our case anticoagulation (in acute phase) was introduced subcutaneously and later continued orally. The outcome was very good with complete remission. CT angiography seems to be a promising …

Vertebral artery dissection due to indirect neck trauma : an underrecognised entity.

Vertebral artery dissection is an important cause of brain stem stroke, especially in the young. Dissections of carotid and vertebral arteries in neck account for about 20% of strokes in young compared with 2.5% in the elderly. Three patients of vertebral artery dissection related to indirect neck trauma are described. The first patient developed the symptoms while dancing, the second after a trivial fall and the third while he was on a dental chair. None of them had a direct severe neck trauma or concomitant risk factor like hypertension, connective tissue disease or migraine. Clinical sympto…
