Utility of psychological screening for the diagnosis of pediatric episodic vertigo.

BACKGROUND/OBJECTIVES: Childhood episodic vertigo has been reported to be associated with migraine or childhood periodic syndromes such as benign paroxysmal vertigo of childhood. There is discrete evidence that unexpected recurrent vertigo is associated with a high level of depression and anxiety in adults. However, only a few studies describe the frequency and characteristics of psychiatric comorbidity in vertiginous children. The aim of this study is to evaluate the incidence and characteristics of emotional and behavioral problems using outpatient-based psychological screening tools in chil

Can Type of Dizziness Influence the Vestibular Caloric Test Result?

BACKGROUND: The broad range of vestibular tests used to diagnose labyrinth diseases allows for a functional assessment of the vestibular system. Among the many tests performed, the caloric test is considered the gold standard by providing an objective measurement of the vestibular function for each labyrinth. OBJECTIVE: to correlate the different types of dizziness with the caloric test result. METHODS: a descriptive study was performed based on the previous records of vestibular tests performed on patients with body balance disorders evaluated at Audiology Service between 2000 and 2020. The

The Relationship between the Subjective Visual Horizontal and Ocular Vestibular Evoked Myogenic Potentials in Acute Vestibular Neuritis.

OBJECT: Vestibular evoked myogenic potentials (VEMPs) and the subjective visual horizontal (SVH) (or vertical [SVV]) have both been considered tests of otolith function: ocular-VEMPs (oVEMPs) utricular function, cervical VEMPs (cVEMPs) saccular function. Some studies have reported association between decreased oVEMPs and SVH, whereas others have not. DESIGN: A retrospective study of test results. SETTING: A tertiary, neuro-otology clinic, Royal Prince Alfred Hospital, Sydney, Australia. METHOD: We analyzed results in 130 patients with acute vestibular neuritis tested within 5 days of onset.

Acute vestibulopathy with simultaneous bilateral involvement of the vestibulo-ocular reflex limited to the low-frequency range.

We report a novel type of idiopathic bilateral vestibulopathy with acute simultaneous involvement of the vestibulo-ocular reflex limited to the low-frequency range. A 64-year-old female presented with dizziness, oscillopsia, and difficulty walking. She did not experience rotatory vertigo and did not show any nystagmus. Vestibular function tests showed absent caloric responses in both ears, while vestibulo-ocular reflex (VOR) gains in the video head impulse test (vHIT) were preserved in all six semicircular canals. Cervical and ocular vestibular evoked myogenic potentials in response to air-con…

Vestibular function analysis in patients with unilateral Ménière's disease.

OBJECTIVE: Ménière's disease (MD) is characterized by episodic vertigo, fluctuating hearing loss, tinnitus, and aural fullness. As vertigo is a cardinal symptom, evaluating vestibular function is essential for disease assessment. However, the relationship between vestibular function and the degree or location of endolymphatic hydrops (EH), as well as the stages of Ménière's disease (MD), remains unclear. METHODS: Patients diagnosed with definite Unilateral Ménière's disease (UMD) underwent a comprehensive vestibular function evaluation, including cervical and ocular vestibular evoked myogenic

[Medical informatics systems exemplified by the diagnosis of equilibrium disorders].

An interdisciplinary field, namely the differential diagnosis of balance disorders and vertigo, is used to describe how a medical expert system can be developed using modern computer analysis, medical expertise, and human pattern recognition techniques. The advantages, results, and unresolved issues of close cooperation between biomedical engineers and physicians are described. The aim of this cooperation was to ensure that complicated data were presented in simple graphic form and that large amounts of diagnostic data were optimally linked together for the generation of a recommended diagnosi

Polytomography in Ménière's disease: an update.

In a previous article the use of polytomography of the vestibular aqueduct in Meniere's disease was described. The value of this procedure in the prognosis of surgery was evaluated by contrast of results with a non-radiographically examined population. Since then five more years of experience has been gained through routine radiographic examination of all patients with Meniere's disease. Within the above epoch, several efforts have been made to effectively separate patients into categories based on therapeutic expectations and prognostic stratifications. In addition X-ray studies, glycerol tes

The Evidence for Selective Loss of Otolithic Function.

Recent advances in vestibular testing now permit functional testing of all peripheral vestibular sense organs (all three semicircular canals, utricle, and saccule). This makes it possible to identify patients with isolated dysfunction of the utricle or saccule, even though parallel pathways for vestibular information are ultimately integrated centrally. Selective, isolated unilateral loss of utricular function as measured by ocular vestibular-evoked myogenic potentials (VEMPs) has been observed in patients with normal semicircular canal function as measured by the video head impulse test of al

Unreliable association between self-reported sense of direction and peripheral vestibular function.

BACKGROUND: Uni- or bilateral peripheralvestibular impairment causes objective spatial orientation deficits, which can be measured using pen-and-paper-tests or sensorimotor tasks (navigation or pointing). For patients' subjective orientation abilities, questionnaires are commonly used (e.g., Santa Barbara sense of direction scale [SBSODS]). However, the relationship between subjective assessment of spatial skills and objective vestibular function has only been scarcely investigated. METHODS: A total of 177 patients (mean age 57.86 ± 17.53 years, 90 females) who presented in our tertiary Cente

Prevalence of vestibular dysfunction in patients with vestibular schwannoma using video head-impulses and vestibular-evoked potentials.

We sought to investigate the utility of new non-invasive tests of semicircular-canal and otolith function that are usable in the neuro-otology office practice in patients with vestibular schwannoma. Fifty patients with vestibular schwannoma were assessed using a 5-item battery consisting of air-conducted cervical- and bone conducted ocular-vestibular-evoked myogenic potentials (AC cVEMPs and BC oVEMPs) and video head impulse testing (vHIT) in all three canal planes. VEMP asymmetry ratios, latencies, and vHIT gains were used to determine the test sensitivity, relationship with tumour size and t

Vestibular implications of noise-induced hearing loss.

An extensive vestibular examination was carried out in a group of 29 noise-exposed technicians. A spontaneous nystagmus was found in 18 persons, and 24 had a positional nystagmus exceeding a velocity of the slow phase of 5 degrees/s in three or more positions. In 17 subjects a cervical nystagmus could be provoked, while a nystagmus preponderance of more than 20% in the rotation test was found in seven persons. A difference in excitability between the labyrinths of more than 20% was shown by seven subjects. None of the subjects showed pathology in the tests for central vestibular disorders. The

Hearing and Vestibular Function After Preoperative Intratympanic Gentamicin Therapy for Vestibular Schwanomma as Part of Vestibular Prehab.

OBJECTIVE: To evaluate auditory and vestibular function after presurgical treatment with gentamicin in schwannoma patients. BACKGROUND: The vestibular PREHAB protocol aims at diminishing the remaining vestibular function before vestibular schwannoma surgery, to ensure less acute symptoms from surgery, and initiate a more efficient vestibular rehabilitation already before surgery. However, the potential cochleotoxicity of gentamicin is a concern, since modern schwannoma surgery strives to preserve hearing. STUDY DESIGN: Retrospective study. SETTING: University hospital. PATIENTS: Seventeen

Vestibular Function in Judo Athletes: Evidence of Subclinical Vestibular Dysfunction.

BACKGROUND: Judo is a dynamic combat sport demanding high levels of balance, agility, postural control, and kinesthetic awareness. The vestibular system plays a vital role in maintaining balance and spatial orientation, integrating sensory inputs essential for judo performance. However, few studies have directly examined vestibular function in judo athletes, despite their frequent exposure to head impacts and rapid rotational movements that may affect vestibular integrity. PURPOSE: This study aimed to compare the vestibular functions of active judo athletes with those of healthy controls. RE

Concurrent abnormalities in galvanic cervical and ocular vestibular-evoked myogenic potentials as a predictive marker for prolonged dizziness in vestibular neuritis.

BACKGROUND: Despite the growing use of cervical (cVEMP) and ocular (oVEMP) VEMP tests, their effectiveness in predicting chronic dizziness in vestibular neuritis (VN) patients remains unclear. Our research examines the link between long-lasting dizziness and inner ear assessments, encompassing VEMPs induced by air-conducted sound (ACS), bone-conducted vibration (BCV), and galvanic vestibular stimulation (GVS). OBJECTIVES: This study explores prognostic markers by examining the relationship between the persistence of dizziness symptoms and various inner ear test findings in VN patients. MATER

[Childhood peripheral vestibular disorders: a report of three cases].

Vertigo is uncommon in childhood. Its etiology is different from that of adults. Both central and peripheral disorders may give rise to vertigo symptoms in pediatric patients. The evaluation of vertigo in children requires a detailed history taking, clinical examination, audiometric assessment, and vestibular function tests. We present three pediatric patients with vertigo symptoms, together with treatment methods and results of treatment. Their ages ranged from 10 to 14 years. The diagnoses were vestibular neuritis in one patient, and benign paroxismal positional vertigo in two patients. Trea…
