Reversible dementia, psychotic symptoms and epilepsy in a patient with vitamin B12 deficiency.

Vitamin B12 deficiency is a common condition, typically associated with megaloblastic anaemia, glossitis and neuropsychiatric symptoms. We report the case of a patient presenting with progressive cognitive and functional deterioration, psychosis and seizures, later found to be secondary to pernicious anaemia. Importantly, the diagnosis of pernicious anaemia was only established 5 years after symptom onset and was overlooked even when the patient was under medical care, in part due to the lack of classic neurological and haematological signs associated with the condition. The patient had a rema

Carnitine deficiency in pregnancy.

BACKGROUND: Carnitine deficiency is a potential cause of metabolic crisis during periods of high energy demand or stress. Affected individuals have very low carnitine levels in blood, decreased carnitine transport in fibroblasts, and commonly have mutations in the OCTN2 gene. CASE: We report management through pregnancy and delivery of a patient with carnitine deficiency who had reduced carnitine transport in fibroblasts, but no mutations in the OCTN2 gene. CONCLUSION: Carnitine deficiency can be treated with exogenous carnitine in select patients during pregnancy. This is especially helpful

Induction and prevention of cleft lip, alveolus and palate and neural tube defects with special consideration of B vitamins and the methylation cycle.

Previous studies on the development of cleft lip, alveolus, palate, and velum and neural tube defects have revealed several shared multifactorial causes. Both anomalies emerge at different times during embryonic development and are not associated with each other unless there is a genetic component to the etiology. Vitamin deficiency disorders are one of several factors contributing to the etiology of these anomalies.Vitamins B6, folic acid and B12 play an essential role in the methylation cycle. A lack of or deficiency in these vitamins thus has severe consequences for the organism, especially

The discovery and characterization of riboflavin.

The first observation of a pigment in milk with yellow-green fluorescence can be traced to the English chemist Alexander Wynter Blyth in 1872, but it was not until the early 1930s that the substance was characterized as riboflavin. Interest in accessory food factors began in the latter half of the 19th century with the discovery of the first vitamin, thiamin. Thiamin was water soluble and given the name vitamin B(1). However, researchers realized that there were one or more additional water-soluble factors and these were called the vitamin B-2 complex. The search to identify these accessory fo

[Livedoid vasculopathy secondary to coeliac disease].

BACKGROUND: Livedoid vasculopathy (LV) is a painful ulcerative condition involving white atrophy and livedo; a histopathologic feature seen is occlusive dermal vasculopathy. This may be associated with coagulation disorders such as hyperhomocysteinaemia (HHC). PATIENTS AND METHODS: We report the case of a 52-year-old woman presenting LV in which an abnormal scan image led us to diagnose coeliac disease. This enteropathy had caused vitamin B12 and folic acid deficiency, as well as HHC. Vitamin supplementation and a gluten-free diet resulted in complete healing of the lesions. DISCUSSION: This

The role of B vitamins in the management of heart failure.

Heart failure (HF) is the leading cause of morbidity and mortality in industrialized countries, creating a significant burden on both the healthcare system and quality of life. Research efforts continue to explore new pharmaceutical or surgically based approaches to HF management, but the role of nutrition as an adjunct therapy has been largely ignored. Elderly age, anorexia, malabsorption, premature satiety, and disease severity are among the factors identified as contributing to reduced nutrient intakes in patients with HF. These factors suggest that patients with HF are at increased risk of

Vitamin B6 deficiency: a potential cause of refractory seizures in adults.

OBJECTIVE: In children, vitamin B(6) (pyridoxine) deficiency has been described as a cause of seizures that are refractory to conventional antiepileptic medications. We describe the clinical presentation of 3 adults with refractory seizures (later diagnosed with vitamin B(6) deficiency) that resolved after pyridoxine treatment. DESIGN: Case series. SETTING: Tertiary care surgical intensive care unit. PATIENTS: In the first case, a 54-year-old male with history of alcoholic cirrhosis developed new-onset seizures refractory to phenytoin and levetiracetam 8 days after liver transplantation. In

Trypsin inhibition: a potential cause of cobalamin deficiency common to the pathogenesis of Alzheimer-type dementia and AIDS dementia complex?

There is increasing evidence for an association between Alzheimer-type dementia (AD) and nutritionally independent cobalamin deficiency. Furthermore, low serum cobalamin values occur in a kindred with familial Alzheimer's disease (FAD) and histopathological confirmation of AD neuropathology. The Cobalamin deficiency could be either a consequence or cause of amyloidogenesis. Cobalamin deficiency is also associated with the acquired immunodeficiency syndrome (AIDS). A common pathogenic mechanism may exist for AIDS dementia complex (ADC) and AD, but there is no explanation at present for these as

Oral Cyanocobalamin Treatment in Infants Under 1 Year With Nutritional Vitamin B12 Deficiency: A Retrospective Comparison of 500 and 1000 µg Dose Regimens.

BACKGROUND: Vitamin B12 deficiency is a significant health problem in infancy, particularly in developing countries. Oral treatment has emerged as an alternative to intramuscular therapy, but optimal dosing in infants remains unclear. METHODS: This retrospective study included 93 infants aged 1 to 12 months diagnosed with nutritional vitamin B12 deficiency. Patients received either 500 or 1000 µg oral cyanocobalamin. Laboratory parameters were evaluated at baseline, 2 months, and 4 months. RESULTS: Both treatment groups demonstrated significant increases in serum vitamin B12 levels and decre

Intramuscular Cyanocobalamin Treatment in Patients with Corpus Atrophic Gastritis and Vitamin B12 Deficiency: Efficacy and Predictors of Increased Requirement-A Monocentric Longitudinal Real-Life Cohort Study.

Background and Objectives: Corpus atrophic gastritis (CAG) is associated with vitamin B12 deficiency due to impaired gastric acid and intrinsic factor secretion. Untreated vitamin B12 deficiency can lead to pernicious anemia, severe neurological consequences, and acute cardiocerebral-vascular events. Timely vitamin B12 supplementation is relevant; however, the dosage of intramuscular (IM) vitamin B12 supplementation has not been standardized to date. The objective was to assess the efficacy of a 1st and 2nd treatment schedule of IM-cyanocobalamin treatment in CAG patients with vitamin B12 defi

Fortification, folate and vitamin B12 balance, and the nervous system. Is folic acid excess potentially harmful?

The benefits of folic acid fortification are well known but less understood or acknowledged is the growing evidence of potential harms to the nervous system from excess folate, primarily because of a combination of fortification and/or high and sustained intake of folic acid supplements. We have summarised the historical, clinical, epidemiological and experimental evidence accumulated over the last 60 years in the pre and postfortification eras, which consistently and substantially suggests that excess folate, in particular in folic acid users, can have direct harms to the nervous system, espe

Intake of B vitamins and impairment in physical function in older adults.

BACKGROUND: The effect of vitamin B intake on physical function is not well known. OBJECTIVE: To examine the prospective association of the intake of vitamins B6, B12 and folate with physical function impairment in older adults. METHODS: We performed a prospective cohort study with 1630 participants from the Seniors-ENRICA study, a cohort of community-dwelling adults aged ≥60 years who were free of physical function impairment at baseline. In 2008-2010, nutrient intake was obtained through a validated computer-assisted face-to-face diet history. Study participants were followed-up through 20

Studies of biomarker responses to intervention with vitamin B-12: a systematic review of randomized controlled trials.

BACKGROUND: Mild vitamin B-12 deficiency is common among older adults, but evidence for setting dietary recommendations is limited because most studies have administered vitamin B-12 via nonoral routes or at doses several hundred times higher than current recommendations. Furthermore, different biomarkers of vitamin B-12 status have not been systematically reviewed. OBJECTIVE: The aim was to assess the effectiveness of biomarkers of vitamin B-12 status through a systematic review of published randomized controlled trials of oral vitamin B-12 supplementation. DESIGN: Methods included a struct

Homocysteine lowering and cardiovascular disease risk: lost in translation.

Studies of the general population have suggested that high homocysteine levels are associated with cardiovascular morbidity and mortality. In chronic kidney disease, homocysteine levels rise, and cardiovascular risk increases with declining kidney function. While some studies in this population have found an association between elevated homocysteine and cardiovascular risk, others have noted that this association is largely attenuated by adjustment for kidney function, and several studies of patients with kidney failure have found that lower homocysteine levels predict mortality. Homocysteine
