Because we are concerned with the teaching of preventive medicine to residents in family medicine, we decided to implement a vaccination stu … for each vaccine studied. We found no statistically significant difference for vaccination against whooping cough, measles and mumps; there was a significant difference for vaccination against tetanus, diphtheria, polio and rubeola (P:0.01).

Pertussis Presenting as New-Onset Uncontrolled Asthma in Children: A Series of Three Cases.

Wheezing in children is most often attributed to asthma; however, infectious etiologies, such as Bordetella pertussis, may present with atypical manifestations, including wheezing, particularly in older children. Such presentations pose a diagnostic challenge and frequently result in misdiagnosis and delayed treatment. We describe three nonatopic school-aged children who presented with prolonged, predominantly nocturnal cough and wheezing and were initially managed as cases of new-onset uncontrolled asthma without clinical improvement. All were subsequently diagnosed with pertussis using multi

Pneumomediastinum as a complication of critical pertussis.

BACKGROUND AND AIMS: Pertussis is a common and potentially serious disease affecting mainly infants and young children. In its non-classic presentation, pertussis can be clinically indistinguishable from other respiratory illnesses. Pertussis today often remains underdiagnosed in adults. Our aims was to report a complicated cases of pertussis. RESULTS: A case of serologically confirmed pertussis occurred in an 18-year-old man presenting with pneumomediastinum, subcutaneous emphysema in the neck and chest, and persistent attacks of coughing with apnea that required treatment in the intensive c

Preliminary X-ray crystallographic analysis of holotoxin from Bordetella pertussis.

Pertussis (whooping cough) is a serious infectious disease caused by the bacterium Bordetella pertussis. One of the major virulence factors is a protein known as pertussis toxin, which is composed of six subunits, with a total molecular weight of 106,000. Enzymatic transfer of ADP-ribose from NAD to a family of GTP-binding proteins is effected by the largest subunit (S1 or the A monomer), while binding of host cells and entry of S1 to the interior is a function of the other subunits (the B oligomer). The holotoxin crystallizes in the orthorhombic space group P2(1)2(1)2(1), with unit cell dimen…

Global Incidence of Pertussis After the COVID-19 Pandemic.

IMPORTANCE: Pertussis, or whooping cough, is caused by the Bordetella pertussis bacterium. It induces prolonged cough in all age groups and is a severe, life-threatening disease in young infants. OBSERVATIONS: In an online workshop organized by the International Bordetella Society on November 12, 2024, most participating countries reported very low pertussis incidence during and immediately following the COVID-19 pandemic. Since that time, many countries have seen large outbreaks of pertussis, particularly in adolescents. Before the pandemic, several countries, especially those using acellula…

An Expert Perspective on a New Recombinant Pertussis-Only Vaccine and Its Potential Clinical and Programmatic Advantages in Australia, New Zealand, and Beyond.

Despite high childhood coverage and booster doses of pertussis vaccination across the life course, pertussis (whooping cough) outbreaks continue to occur, including in Australia and New Zealand. In January 2026, the European Medicines Agency (EMA) granted marketing authorisation to a new pertussis-only booster vaccine (VacPertagenTM): the first time in two decades that a new acellular pertussis vaccine has been approved in the European Union. Anticipating future licensure of the vaccine in Australia and New Zealand, an expert panel was convened to discuss the clinical and programmatic advantag…

Challenges and solutions for a rational vaccine design for TB-endemic regions.

Vaccines have been successful for global eradication or control of dreaded diseases such as smallpox, diphtheria, tetanus, yellow fever, whooping cough, polio, and measles. Unfortunately, this success has not been achieved for controlling tuberculosis (TB) worldwide. Bacillus Calmette Guérin (BCG) is the only available vaccine against TB. Paradoxically, BCG has deciphered success in the Western world but has failed in TB-endemic areas. In this article, we highlight and discuss the aspects of immunity responsible for controlling Mycobacterium tuberculosis infection and factors responsible for t…

Use of an in-house approach to study the three-dimensional structures of various outer membrane proteins: structure of the alcaligin outer membrane transporter FauA from Bordetella pertussis.

Bordetella pertussis is the bacterial agent of whooping cough in humans. Under iron-limiting conditions, it produces the siderophore alcaligin. Released to the extracellular environment, alcaligin chelates iron, which is then taken up as a ferric alcaligin complex via the FauA outer membrane transporter. FauA belongs to a family of TonB-dependent outer membrane transporters that function using energy derived from the proton motive force. Using an in-house protocol for membrane-protein expression, purification and crystallization, FauA was crystallized in its apo form together with three other …

Conserved Patterns of Symmetric Inversion in the Genome Evolution of Bordetella Respiratory Pathogens.

Whooping cough (pertussis), primarily caused by Bordetella pertussis, has resurged in the United States, and circulating strains exhibit considerable chromosome structural fluidity in the form of rearrangement and deletion. The genus Bordetella includes additional pathogenic species infecting various animals, some even causing pertussis-like respiratory disease in humans; however, investigation of their genome evolution has been limited. We studied chromosome structure in complete genome sequences from 167 Bordetella species isolates, as well as 469 B. pertussis isolates, to gain a generalized…

Seroepidemiological Assessment of Bordetella pertussis in Jahrom, Southern Iran: A Cross-Sectional Study.

BACKGROUND: Bordetella pertussis, the causative agent of whooping cough, is a significant contributor to recurrent persistent cough across all age groups, including vaccinated individuals. This seroepidemiological study aims to address the gap in understanding pertussis incidence by investigating its occurrence in individuals with persistent cough and describing the characteristics of affected patients admitted to clinical centers in Jahrom, Southwest Iran. METHODS: This cross-sectional study enrolled 110 patients with a cough persisting for at least 2 weeks, admitted to clinical centers in J…

Clinical evaluation of a new rapid immunochromatographic test for detection of Bordetella pertussis antigen.

A more rapid and less complicated test to diagnose pertussis is required in clinical settings. We need to detect Bordetella pertussis, which mainly causes pertussis, as early as possible, because pertussis is more likely to become severe in infants, and people around them can easily become a source of infection due to its strong infectivity. Nevertheless, methods that can detect B. pertussis rapidly and efficiently are lacking. Therefore, we developed a new immunochromatographic antigen kit (ICkit) for the early diagnosis of pertussis. The ICkit detects B. pertussis antigens in a nasopharyngea

[Hypotonic-hyporesponsive episode following acellular pertussis vaccination].

The introduction of routine vaccination against whooping cough caused a drastic decline in the incidence of this disease, but remains today a public health problem even in countries with high vaccination coverage. However, with this decrease in cases, there were an emergence of neurological severe adverse events such as the "hypotonic-hyporesponsive episodes" (HHE), characterized by sudden loss of muscle tone associated with poor response to stimuli and skin pallor or cyanosis. The HHE is a rare phenomenon after administration of pertussis vaccine, but it is essential for health workers recogn…

Bordetellae colonization oligosaccharide (b-Cool), a glycan crucial for nasal colonization.

Respiratory tract infections pose considerable global health challenges. Upper airway colonization is pivotal to these infections, including those caused by Bordetella species. We identified an oligosaccharide, bordetellae colonization oligosaccharide (b-Cool), crucial for early nasal colonization of Bordetella bronchiseptica. We characterized the structure of b-Cool by LC-MS and NMR and found that it is prevalent across a diverse range of bordetellae, including Bordetella pertussis, which causes whooping cough in humans. A B. bronchiseptica mutant lacking b-Cool (Δb-Cool) showed significantly…

[Evolution of the vaccination timetable in Morocco].

Morocco has always been aware of the importance of vaccination and has been implementing an ambitious vaccination policy for several decades. The first vaccination campaigns started in the sixties. The Extended Vaccination Program (EVP) was launched in 1981 and restructured six years later, being renamed the National Immunization Program (NIP). Regular follow-up of vaccine efficacy has led to several changes in the national vaccination timetable, including new antigens and boosters. Protection against tuberculosis, diphtheria, tetanus, whooping cough, poliomyelitis, etc. has considerably impro…

Pertussis in infants, children, and adolescents: diagnosis, treatment, and prevention.

Pertussis, or "whooping cough," caused by the gram-negative pleomorphic bacillus Bordetella pertussis, is a highly contagious, potentially life-threatening respiratory tract illness that has re-emerged worldwide as a cause of substantial morbidity and mortality in infants, children, and adolescents, despite high vaccination rates. Increased awareness and reporting, in addition to the availability of better diagnostic tests, partially explain the recent resurgence of pertussis. However, waning immunity after childhood immunization has resulted in a growing pool of susceptible adolescents and ad…

Depopulation of the Northern Plains Natives.

Nine major epidemics of acute infectious diseases swept the Northern Plains of the Western Interior of Canada between 1774 and 1839. The Blackfeet, Plains Cree and Assiniboin, Atsina and Saulteaux who exploited the Plains were differentially exposed to these epidemics of smallpox, measles, whooping cough and influenza. Mortality estimates from these epidemics were used in assessing the degree to which a series of epidemics contributed to depopulation of the Plains Natives. A criteria was established to determine an epidemic from a depopulation epidemic, which involved among other factors, the …
