OBJECTIVE: To evaluate the incidence, clinical features, and prognosis of pulmonary complications associated with toxic epidermal necrolysis

SETTING: Dermatology intensive care unit in Mondor Hospital, France.
PATIENTS: 41 consecutive patients.
INTERVENTIONS: On admission, then daily, respiratory evaluation was based on clinical examination, chest X-ray, and arterial blood gas analysis. BACKGROUND: Craniospinal irradiation (CSI) is commonly required for pediatric patients with central nervous system malignancies. Traditionally, CSI is given using x-rays to deliver radiation to the brain and spine, exposing normal anterior structures, including heart, to unnecessary radiation.
OBSERVATIONS: We present a patient treated with x-ray CSI for medulloblastoma with spinal metastasis (3600 cGy CSI with focal boost to 5000 cGy), who subsequently developed significant cardiac toxicity, likely related to radiation exposure.

METHODS: 50 patients with recalcitrant heel pain and a plantar calcaneal spur on the X-ray received in a controlled, prospective and randomized study low-energetic extracorporal shock-wave-therapy.
