Intramedullary abscess resulting from disseminated cryptococcosis despite immune restoration in a patient with AIDS.

We report on a case of cryptococcal intramedullary abscess, which occurred three years after a disseminated cryptococcosis and two years after a lymph node cryptococcal recurrence in a HIV-infected patient who exhibited a long-standing immune restoration. At the time of diagnosis, CD4(+) lymphocyte-count was 640x10(6)/l and HIV viral load was undetectable. Spinal involvement is rare during cryptococcosis of the central nervous system. As far as we are aware, there is only one case of proven intramedullary cryptococcal abscess reported in the literature and this case is then the second one. The

Stereotactic laser-guided approach to distal middle cerebral artery aneurysms.

The Steiner-Lindquist laser intra-operative guidance method is described and its application in the surgical treatment of mycotic aneurysm is presented. Three illustrative cases of distal middle cerebral artery aneurysms are reported. The authors argue that all ruptured mycotic aneurysms should be treated surgically. The laser-guided stereotactic approach is presented as simple, flexible and reliable method to improve safety and accuracy of the operation in these cases. In our experience it compares favorably with previously described stereotactic techniques.

Staphylococcal meningitis can present as an abscess of a single lateral ventricle.

Ventricular obstruction and hydrocephalus are recognized complications of neurosurgical procedures and meningitis that has been previously treated. The confinement of bacterial meningitis solely to a lateral ventricle in an otherwise healthy individual, however, is rare. I describe a case in which a ventricular abscess occurred as the presenting manifestation of staphylococcal meningitis in a man who had no history of head trauma or neurosurgery.

[Abscess of the pyramidal apex].

The authors report on one case of an abscess at the pyramidal apex. The 52-year old male patient presented with pareses of the fifth and seventh cranial nerves and hypacusis on the right side. After diagnostic procedures (CT-scan, carotid angiography), a tumor at the apex of the right pyramid was expected. During surgery a large encapsulated mass was found containing pus. A bacterial agent could not be isolated. The abscess bordered on the mucosal lining of the sphenoid sinus and on the cells of mastoid bone. The starting point of an abscess at the pyramidal apex is most commonly an otitis med

Epidemiologic features of an intracranial abscessation/suppurative meningoencephalitis complex in white-tailed deer.

Case records of 683 white-tailed deer (Odocoileus virginianus) submitted to the Southeastern Cooperative Wildlife Disease Study (Georgia, USA) for diagnostic purposes from 1971 to 1989 were reviewed for the occurrence of pyogenic infections of the central nervous system, specifically intracranial abscessation or suppurative meningoencephalitis. These conditions, either alone or in combination, were diagnosed in 24 of 683 (4%) deer. Thirteen genera of bacteria were isolated; the most frequent species was Actinomyces pyogenes. The disease was strongly sex biased (P less than 0.01); 88% of the ca

[Correlation between CT findings and clinical course in viral and bacterial meningoencephalitis].

The computed axial tomograms (CAT) of 50 patients with viral or bacterial meningoencephalitis are correlated with the clinical course, the laboratory data, and the EEG changes. The diagnostic value of CAT in the early diagnosis of herpes simplex encephalitis is of specific importance. Hypodense regions are demonstrated after the fifth day of illness in the temporal lobe. All other viral or bacterial meningoencephalitis cases have no specific changes on CAT examination compared with clinical or laboratory data. However, in localised encephalitis CAT may reveal hypodense regions. For follow-up s

Hyperbaric oxygen in the treatment of rhinocerebral mucormycosis.

Hyperbaric oxygen therapy was utilized in a case of fulminant mucormycosis of the maxilla, orbit and temporal bone. The patient had refused radical surgery and death seemed iminent in spite of aggressive medical management of diabetic ketoacidosis, amphotericin B and wide surgical drainage of the maxillary and ethmoid sinuses with orbital decompression. Hyperbaric oxygen was instituted on the following theoretical premises: 1. It would provide oxygenation of tissues distal to occluded arteries, thereby increasing local survival and decreasing acidosis, 2. resultant lessening of acidosis would

Hypernatremic hemorrhagic encephalopathy.

A 12-year-old juvenile diabetic was inadvertently given 500 ml of hypertonic saline intravenously. He developed hypernatremia, hyperosmolality, metabolic acidosis, and hyperglycemia. Seizures and stupor ensued, followed by coma and death. Computerized cranial tomography revealed numerous small subcortical hemorrhages that were verified postmortem.

Nocardiosis in children: report of three cases and review of the literature.

The clinical illnesses observed in three adolescents with Nocardia asteroides infection are described. The organism has been classified as a bacterium and placed in the Actinomycetaceae family. Sulfonamides remain the first choice for chemotherapy but alternative drugs are available and may be essential in successful therapy. Nocardial infections in children indicate the need for evaluation for an underlying immunological deficiency. Nocardia species should be considered as possible causative agents in immunocompromised patients with infection.

[Decompressive craniotomy and its effect on recovery and rehabilitation of the patient].

The rehabilitation of patients who have to be operated on because of severe craniocerebral trauma and in whom the roof of the cranium must be removed because of severe post-traumatic cerebral oedema, is extremely difficult. The slow rehabilitation and recovery of these patients, who also show psychological changes in most cases, must be attributed to the pathological intracranial conditions which have arisen in consequence of large defects in cranial bones. A causal correlation between severe pathological intracranial conditions, which have arisen not only from trauma but also from bone flap d

Nuclear magnetic resonance imaging of the brain in children.

Nuclear magnetic resonance imaging of the hydrogen nucleus provides a unique noninvasive display of proton dynamics in biologic tissues and fluids as well as internal anatomy in a sectional imaging format. No ionizing radiation is utilized. Our experience with NMR imaging of the brain in 14 pediatric patients is presented and compared with computed tomography. The major advantages of NMR over CT include its greater sensitivity to blood flow, edema, hemorrhage, and myelinization and its lack of beam-hardening artifacts. In addition, the potential for tissue characterization exists by determinat

Occam's razor... blunted.

Occam's razor, [When there are two competing theories making the same prediction, the simpler one is chosen], is a popular method in making clinical diagnosis. This theory gets 'blunted' in our case report, as our immunocompromised patient was found to have multiple non-related aetiologies for a single pathological event.

Linezolid activity against disseminated Listeria monocytogenes meningitis and central nervous system abscesses: focus on early drug myelotoxicity.

A very infrequent report of Listeria monocytogenes meningitis and multiple subtentorial abscesses (cerebellar, bulbar, and pontine in location), occurred in an immunocompetent man, and is described and discussed on the ground of the most recent literature evidences regarding the frequency, presentation, and outcome of this disease. The possibility to resort to combination linezolid therapy in patients who appear to be not responsive to first-line treatment of cerebral abscess of definite origin, and the appearance of anemia as an early untoward event of this novel oxazolidinone antibiotic, are

Rifampin monotherapy for multidrug-resistant Chryseobacterium indologenes meningitis unresponsive to trimethoprim-sulfamethoxazole: a case report and review of the literature.

Multidrug-resistant (MDR) Chryseobacterium indologenes is an emerging pathogen causing challenging central nervous system (CNS) infections, for which there are no standardized treatment guidelines. A systematic review of the literature indicates that such infections are extremely rare. Although trimethoprim-sulfamethoxazole (TMP-SMX) is the most frequently reported first-line therapy, its clinical efficacy is not universal, posing a significant therapeutic challenge. We present the case of a 25-year-old postpartum woman who developed MDR C. indologenes meningitis and a brain abscess after unde…

Variability in agitated saline echocardiography protocols: a cross-sectional analysis of bubble study techniques in echo labs across Alberta, Canada.

Intrapulmonary right-to-left shunts (RLSs) are a clinical concern in hereditary hemorrhagic telangiectasia (HHT), contributing to dyspnea, brain abscess, and ischemic stroke. HHT is a genetic disorder characterized by arteriovenous malformations (AVMs), with pulmonary AVMs (PAVMs) being a major source of intrapulmonary shunting. Screening for PAVMs is essential to prevent paradoxical embolization, guide management, and mitigate mortality risk. Agitated saline contrast echocardiography ("bubble echo") is the recommended method for detecting and grading RLS, as outlined in International HHT Guid…
