Metabolic pathway analysis of hyperuricaemia patients with hyperlipidaemia based on high-throughput mass spectrometry: a case‒control study.

BACKGROUND: Both hyperuricaemia and hyperlipidaemia are common metabolic diseases that are closely related to each other, and both are independent risk factors for the development of a variety of diseases. HUA combined with hyperlipidaemia increases the risk of nonalcoholic fatty liver disease and coronary heart disease. This study aimed to investigate the relationship between HUA and hyperlipidaemia and study the metabolic pathway changes in patients with HUA associated with hyperlipidaemia using metabolomics. METHODS: This was a case‒control study. The prevalence of hyperlipidaemia in HUA p

Update on Importance of Diet in Gout.

Gout is an inflammatory arthritis caused by deposition of monosodium urate crystals within synovial joints. Although it is most well-known for its arthritis, gout has an intimate relationship with many other cardiovascular and metabolic conditions. Current recommendations support aggressive medical therapy to treat gout, whereas dietary counseling has become less emphasized. This article argues for the absolute importance of dietary counseling in gout and proves why this counseling may impact the long term well-being of a patient with gout.

Allopurinol use and risk of non-fatal acute myocardial infarction.

OBJECTIVES: To quantify the risk of non-fatal acute myocardial infarction (AMI) among users of allopurinol. METHODS: We carried out a population-based case-control study over the period 2001-2007 in patients aged 40-90 years. Patients who had prescriptions of allopurinol or an episode of AMI before the start date of follow-up were excluded from the main analysis. Allopurinol initiators were classified as current users if their last prescription ended in the 30-day window before the recorded date of AMI for cases and a random date for controls. The association between use of allopurinol and no

Allopurinol initiation and all-cause mortality in the general population.

BACKGROUND: Allopurinol is the most commonly used urate-lowering therapy, with rare but potentially fatal adverse effects. However, its impact on overall mortality remains largely unknown. In this study, we evaluated the impact of allopurinol initiation on the risk of mortality among individuals with hyperuricaemia and among those with gout in the general population. METHODS: We conducted an incident user cohort study with propensity score matching using a UK general population database. The study population included individuals aged ≥40 years who had a record of hyperuricaemia (serum urate l

The efficacy and safety of febuxostat for urate lowering in gout patients ≥65 years of age.

BACKGROUND: The incidence of gout rises with increasing age. Management of elderly (≥65 years) gout patients can be challenging due to high rates of comorbidities, such as renal impairment and cardiovascular disease, and concomitant medication use. However, there is little data specifically addressing the efficacy and safety of available urate-lowering therapies (ULT) in the elderly. The objective of this post hoc analysis was to examine the efficacy and safety of ULT with febuxostat or allopurinol in a subset of elderly subjects enrolled in the CONFIRMS trial. METHODS: Hyperuricemic (serum u

Hyperuricaemia and gout.

Gout is increasing in prevalence throughout the world, particularly in developed countries. The causes are dietary--purine-rich foods, high saturated fats, fructose-containing drinks and alcohol. Gout is also drug-related and associated with increased obesity, hypertension, insulin resistance and metabolic syndrome. Although very readily treated, there is evidence that physicians fail to optimise the treatment and achieve low enough serum urate levels, while patients fail to comply with the treatment and dietary advice. Standard treatment of acute attacks is with non-steroidal anti-inflammator

Gout: a clinical syndrome illustrated and discussed.

Gout is an acute inflammatory arthritis with the potency to fully destroy the integrity of the joint leading to severe disability. Besides joint destruction, gout is often associated with an accelerated atherosclerosis culminating in an increased risk of cardiovascular disease. The current existing therapy modalities allow an efficient treatment that not only controls local inflammation but might also have an effect on the generalised features that surround this condition. Here we discuss the modes of clinical appearance, how we are nowadays supposed to treat gout and the current knowledge abo

Frequency, risk, and cost of gout-related episodes among the elderly: does serum uric acid level matter?

OBJECTIVE: We examined the association between serum uric acid (SUA) level and the frequency, risk, and cost of gout flares among the elderly. METHODS: Data were extracted from the Integrated Healthcare Information Services claims database (1999-2005). Patients were included if they had gout, were aged 65 years and older and had both medical and pharmacy benefits, and electronic laboratory data. Patients with gout and gouty episodes were identified using algorithms based on ICD-9-CM codes and medications. Logistic regression and negative binomial regressions were used to study the relationshi

Serum uric acid level as an independent component of the metabolic syndrome in type 2 diabetic blacks.

CONTEXT: No consensus has been achieved on the components included in the definition of Metabolic Syndrome (MS). Uric acid and Gamma glutamyl transpeptidase are however newer markers not included in previous studies. OBJECTIVES: This study was carried out to determine the prevalence of MS in Diabetes Mellitus, the correlation between hyperuricaemia and MS as well as make a case for the inclusion of serum Uric acid level as a new marker for MS. METHODOLOGY: Fasting venous sample from the cubital vein of 77 females and 44 males diagnosed NIDDM patients for enzymatic determination of serum lipi

Functional consequences of a novel uromodulin mutation in a family with familial juvenile hyperuricaemic nephropathy.

BACKGROUND: Familial juvenile hyperuricaemic nephropathy (FJHN) is an autosomal-dominant disorder featuring hyperuricaemia, low fractional urate excretion, interstitial nephritis and chronic renal failure. The responsible gene UMOD was recently identified. UMOD encodes for uromodulin or Tamm-Horsfall glycoprotein, the most abundant protein in normal urine. We encountered a family with FJHN and identified a novel UMOD mutation in exon 6. METHODS: We sequenced the gene in all family members, identified the mutation, and verified its presence in the affected members. We next performed functional

From Efficacy to Process: Uric-Acid-Lowering Effect of Limosilactobacillus reuteri W6-12 and Its Production Process Optimization.

Clinical chemical drugs for hyperuricemia (HUA) often cause side effects including liver and kidney injury. Developing safe and effective probiotic preparations for HUA intervention is of considerable application value. However, the large-scale production of probiotics faces two major bottlenecks: low viable cell density during fermentation and poor survival after lyophilization. This study integrated in vivo efficacy evaluation and mechanism exploration with the production process optimization for Limosilactobacillus reuteri W6-12. We first verified its hypouricemic effect in vivo, then adopt…

Distinct clinical profiles of methotrexate intolerance and hepatotoxicity in psoriatic arthritis.

INTRODUCTION: Methotrexate (MTX) is the most widely used conventional DMARD for psoriatic arthritis (PsA), but intolerance and hepatotoxicit … rs of intolerance were identified. Hepatotoxicity was linked to male sex, dactylitis, hypertension, hyperuricemia, and treatment-refractory PsA. In multivariable analysis, hepatotoxicity was independently associated with arterial hypertension (OR 4.40; p 0.007) and treatment-refractory PsA (OR 6.23; p 0.016), and inversely associated with intolerance (OR 0.09; …

Population Pharmacokinetic Analysis and Modelling of Serum Uric Acid Dynamics in Patients Treated with Favipiravir.

Background: Hyperuricemia is an adverse effect frequently observed during favipiravir treatment. The time course, from uric acid elevation to recovery, and quantitative relationship between drug exposure and changes in serum uric acid levels remain insufficiently characterized. We investigated the pharmacodynamic mechanism of uric acid elevation and described its time course by population pharmacokinetic and pharmacodynamic modelling. Methods: Patients who received favipiravir for coronavirus disease 2019 or severe fever with thrombocytopenia syndrome were retrospectively evaluated. The pharma…
