Leveraging artificial intelligence in neurosurgery-unveiling ChatGPT for neurosurgical discharge summaries and operative reports.

PURPOSE: Chat generative pre-trained transformer (GPT) is a novel large pre-trained natural language processing software that can enable scientific writing amongst a litany of other features. Given this, there is a growing interest in exploring the use of ChatGPT models as a modality to facilitate/assist in the provision of clinical care. METHODS: We investigated the time taken for the composition of neurosurgical discharge summaries and operative reports at a major University hospital. In so doing, we compared currently employed speech recognition software (i.e., SpeaKING) vs novel ChatGPT f

The Recent Technologies to Curb the Second-Wave of COVID-19 Pandemic.

Different epidemics, specially Coronavirus, have caused critical misfortunes in various fields like monetary deprivation, survival conditions, thus diminishing the overall individual fulfillment. Various worldwide associations and different hierarchies of government fraternity are endeavoring to offer the necessary assistance in eliminating the infection impacts but unfortunately standing up to the non-appearance of resources and expertise. In contrast to all other pandemics, Coronavirus has proven to exhibit numerous requirements such that curated appropriation and determination of innovation

Adverse and Hypersensitivity Reactions to Prescription Nonsteroidal Anti-Inflammatory Agents in a Large Health Care System.

BACKGROUND: Nonsteroidal anti-inflammatory drugs (NSAIDs) are among the most frequently used medications in the United States. NSAID use can be limited by adverse drug reactions (ADRs), including hypersensitivity reactions (HSRs). OBJECTIVE: We aimed to use electronic health record data to determine the incidence and predictors of HSRs to prescription NSAIDs. METHODS: We performed a retrospective cohort study of all adult outpatients in a large health care system prescribed diclofenac, indomethacin, nabumetone, or piroxicam between January 1, 2004, and September 30, 2012. The primary outcome

Premature Clopidogrel Discontinuation After Drug-Eluting Stent Placement in a Large Urban Safety-Net Hospital.

Premature clopidogrel discontinuation is an important cause of stent thrombosis, myocardial infarction, and death after drug-eluting stent (DES) placement. Previous studies of clopidogrel nonadherence after DES placement have had short follow-up and have relied on self-reported adherence. All patients who underwent DES placement from January 2008 to December 2011 at a single safety-net hospital and received medications through the county-subsidized health plan were considered for inclusion; those with <1-year follow-up were excluded. We retrospectively collected 1-year refill data from a large

Provider barriers prevent recommended sexually transmitted disease screening of HIV-infected men who have sex with men.

BACKGROUND: HIV-infected men who have sex with men (MSM) are at increased risk for transmitting and acquiring sexually transmitted diseases (STDs). Guidelines recommend at least annual screening of HIV-infected MSM for syphilis and for chlamydia and gonorrhea at exposed anatomical sites, to protect their health and their sexual partners' health. Despite these guidelines, STD screening has been suboptimal, with very low nongenital chlamydia and gonorrhea testing rates. Our objective was to better understand barriers encountered by HIV care providers in adhering to STD screening guidelines for H

Re-thinking diagnostic classification of the dysarthrias: a developmental perspective.

Acquired childhood dysarthria (ACD) receives little attention in the research literature in contrast with the adult correlate of the disorder. Speech language pathologists working in this field find diagnosis and management challenging, arguably because there is no child-based dysarthria diagnostic classification. Clinicians are either dependent upon developmental speech models that are not specific to dysarthria and that ignore the neural basis of the disorder, or on adult-based neurobehavioural classification systems. Here we consider the necessary elements for developing a clinically useful

Rule-Based Versus Generative Extraction of Psychological Symptoms From Forensic Medical Certificates: A Feasibility Study in the French ORFeAD Network.

Forensic medical certificates for victims of interpersonal violence describe psychological symptoms in unstructured prose, rarely explicit and requiring inference. To assess the feasibility of two extraction pipelines-rule-based and a locally served generative large language model-in a French multicentric forensic network, and to identify variables usable without human verification. 110 randomly selected 2021 certificates from 12 ORFeAD units were processed by both pipelines: rule-based (segmentation, lexicons, regular expressions, negation detection) and generative (Llama 3 8B, 4-bit, Ollama,

ISAR-Mamba: A Dual-Stream Gated Mamba with Hierarchical Spatial Summaries for ISAR Image Captioning.

Inverse Synthetic Aperture Radar (ISAR) imagery plays a crucial role in space situational awareness, yet its interpretation remains largely confined to tasks such as classification and segmentation, lacking the ability to generate detailed natural language captions. To address this limitation, this paper proposes ISARCap 1.0, the first dataset specifically designed for ISAR image captioning, covering 38 classes of space targets and comprising 102,965 simulated images and 514,825 image-text pairs. On this basis, this paper proposes ISAR-Mamba, a dual-stream gated state space model for ISAR imag

Spatio-Temporal and Retrieval-Augmented Modeling for Chest X-Ray Report Generation.

Chest X-ray report generation has attracted increasing research attention. However, most existing methods neglect the temporal information and typically generate reports conditioned on a fixed number of images. In this paper, we propose STREAM: Spatio-Temporal and REtrieval-Augmented Modelling for automatic chest X-ray report generation. It mimics clinical diagnosis by integrating current and historical studies to interpret the present condition (temporal), with each study containing images from multi-views (spatial). Concretely, our STREAM is built upon an encoder-decoder architecture, utiliz

Ex2Vec: Enhancing assembly code semantics with end-to-end execution-aware embeddings.

Binary code similarity detection (BSCD), whose goal is to identify and analyze similar or identical functions in compiled binaries, is an essential task in computer security. Recent methods leveraging deep neural networks (DNN) for numerical vector representation of code have achieved significant success. However, these methods primarily adapt techniques from masked language modeling (MLM), encoding code instructions by predicting missing values from an instruction context, which limits their ability to fully capture execution semantics. In this paper, we propose Ex2vec, an innovative end-to-e

A novel paradigm for measuring prediction abilities in a rat model using a speech-sound discrimination task.

The ability to predict and respond to upcoming stimuli is a critical skill for all animals, including humans. Prediction operates largely below conscious awareness to allow an individual to recall previously encountered stimuli and prepare an appropriate response, especially in language. The ability to predict upcoming words within typical speech patterns aids fluent comprehension, as conversational speech occurs quickly. Individuals with certain neurodevelopmental disorders such as autism and dyslexia have deficits in their ability to generate and use predictions. Rodent models are often used

Alexia and Agraphia from 1861 to 1965.

Studies of alexia and agraphia have played historically important roles in efforts to understand the relation between brain and behavior. In the second half of the 19th century, works by Paul Broca and Carl Wernicke led to the concept of delimited cortical centers in the left cerebral hemisphere concerned with discrete aspects of spoken and written language. These specialized centers were linked by white matter pathways. Charlton Bastian, Jean-Martin Charcot, Sigmund Exner, and Jules Dejerine championed center-pathway models of reading and writing. Dejerine played a dominant role, rejecting th

Higher Medical Urgency Is Associated With Greater Psychosocial Risk Tolerance in Liver Transplant Listing and Selection.

BACKGROUND: The Model for End-Stage Liver Disease (MELD) score guides liver transplant (LT) allocation, but transplant programs also evaluate psychosocial risks that may affect outcomes such as graft survival, adherence, and follow-up. How medical urgency influences consideration of these risks remains unclear. METHODS: We conducted a retrospective analysis of 2959 adults listed for LT at a large academic center (2012-2023). Using a validated large language model-based techniques, we extracted 23 predefined psychosocial factors from clinical documentation. We compared the prevalence of these

Is Toxoplasma gondii a potential risk factor for Alzheimer's disease? A systematic review and meta-analysis.

Toxoplasmosis is a major public health concern due to neurotropic nature and role in the development of mental and behavioral disorders. Alzheimer's disease (AD) is an important nervous disease that results in the reduction of the amount of beta-amyloid plaque deposition and irreversible loss of neurons in the brain. Although a few studies evaluated the association between AD and toxoplasmosis, the present study as a systematic review and meta-analysis of published studies investigated the possible association between Toxoplasma gondii (T. gondii) and AD. A systematic literature search was con

PANCDetect: Early Detection of Pancreatic Cancer from Multimodal EHR data with LLM Embeddings.

BACKGROUND: Pancreatic cancer (PANC) is often diagnosed at late stages due to the absence of specific early symptoms, resulting in one of the highest cancer mortality rates. While imaging modalities such as MRI and CT offer high diagnostic accuracy, their population-wide application is however impractical due to the cost. Electronic health records (EHRs) provide a routine, easily accessible, longitudinal and scalable data source for risk prediction, particularly for diseases with no specific symptom such as PANC. METHOD: We introduce PANCDetect, a multimodal framework that leverages large lan

CODER: Data-efficient activation of mathematical reasoning via difficulty-aware core-set.

Large language models (LLMs) demonstrate remarkable performance in mathematical reasoning tasks, yet their fine-tuning process typically relies on substantial volumes of high-quality annotated data. Under conditions of limited computational resources and sparse data, they are highly susceptible to overfitting and catastrophic forgetting. Inspired by the classic textbooks used in human education from primary to secondary school, which embody a natural difficulty progression, this work suggests that constructing hierarchical data covering key knowledge nodes can effectively facilitate the activa…

AI-enabled and comparator digital interventions for adolescent mental health (2020-2025): early detection, human-AI collaboration, and ethical governance-a PRISMA-ScR scoping review.

INTRODUCTION: Mental health difficulties among adolescents and youth (ages 10-24) are increasing globally, yet early intervention remains impeded by stigma, service shortages, and cultural barriers. AI-enabled technologies including chatbots, automated screening tools, and machine learning-based predictive models have been proposed as scalable approaches to address these gaps, alongside non-AI digital interventions such as therapist-guided internet-delivered cognitive behavioral therapy (iCBT) and smartphone-based CBT applications, which are examined as comparator evidence given their stronger

Word Learning Ability Varies Across Contexts and Time: A Longitudinal Study of Primary School Children with Developmental Language Disorder.

BACKGROUND AND AIMS: Children with developmental language disorder (DLD) have difficulty learning new words, but we know little about whether and to what extent their word-learning ability improves over time. Our primary goal was to compare the rate of development of word learning abilities, specifically the encoding of new lexical-semantic information, in children with DLD and their age-mates with typical language development (TLD). Secondary goals were to examine variation in outcomes according to the aspect of word knowledge under consideration, the word-learning context, and the cognitive

Harmonisation of later-life cognitive function across national contexts: results from the Harmonized Cognitive Assessment Protocols.

BACKGROUND: The Harmonized Cognitive Assessment Protocol (HCAP) is an innovative instrument for cross-national comparisons of later-life cognitive function, yet its suitability across diverse populations is unknown. We aimed to harmonise general and domain-specific cognitive scores from HCAP studies across six countries, and evaluate reliability and criterion validity of the resulting harmonised scores. METHODS: We statistically harmonised general and domain-specific cognitive function scores across publicly available HCAP partner studies in China, England, India, Mexico, South Africa, and th
