Ossification of ligamentum flavum in Middle East Arabs: a hospital-based study.

OBJECTIVE: To investigate the frequency of ossification of the ligamentum flavum (OLF) in the spine among the Arab population in Kuwait using magnetic resonance imaging (MRI) surveillance of the whole spine. SUBJECTS AND METHODS: A consecutive series of 102 patients with low back pain were recruited from the outpatient clinic of Mubarak Al-Kabeer Hospital, Kuwait. MRI of the whole spine in the sagittal plane was obtained in at least two sequences (T(1) and T(2)). The OLF was defined as low signal intensity thickening of the ligament in both T(1) and T(2) sequences on the posterior margin of t

A partially ossified solid and cystic Rathke cleft cyst.

A Rathke cleft cyst is considered to arise from the remnants of the Rathke pouch, and it consists of single cuboidal or columnar epithelium including cilia and goblet cells, which secrete mucus into the cyst. Magnetic resonance imaging characteristically shows a thin membranous cystic wall that enhances with Gd, and homogeneous intensity of the content suggesting fluid collection. Cases with an irregularly thickened and/or calcified cyst wall, presumably due to chronic inflammation of the wall, are rare. A 21-year-old woman presented with an extremely rare case of a solid and cystic Rathke cle

Promoting fusion in minimally invasive lumbar interbody stabilization with low-dose bone morphogenic protein-2--but what is the cost?

BACKGROUND: Using bone morphogenic protein (BMP) to augment fusion in spine surgery is widespread and lends itself in particular to minimally invasive lumbar fusion, where the surface area for fusion is significantly less than the equivalent open procedure. PURPOSE: Here we described the use of very low-dose BMP in promoting fusion in minimally invasive lumbar interbody fixation but also highlight some of the potential complications of BMP-2 use and techniques available to reduce or avoid them. STUDY DESIGN: Prospective observational study of consecutive patients undergoing minimally invasiv

Clinicopathological study of "snake-eye appearance" in compressive myelopathy of the cervical spinal cord.

OBJECT: The goal of this study was to elucidate the pathophysiological features and clinical significance of the magnetic resonance imaging-documented small intramedullary high signal intensity known as "snake-eye appearance" (SEA) in cases of compressive myelopathy such as cervical spondylosis or ossification of the posterior longitudinal ligament. METHODS: One hundred forty-four patients with compression myelopathy who underwent surgery between 1998 and 2000 were selected. Intramedullary high signal intensity was found in 79 cases and was divided into two types, SEA and non-SEA (NSEA). The

[Computed tomography diagnosis of symptomatic ossification of the thoracic flaval ligaments].

We report two rare cases of prominent ossified ligamenta flava of the thoraco-lumbar junction. The first case presented with thoracic myelopathy diagnosed by spinal MRI. The definite diagnosis and the nature of the T10-T11 compressing process was established by spiral CT with multiaxial reconstruction and the patient was treated by laminectomy. In the second case the ossifications were found on a lateral lumbar X-ray and confirmed by spiral CT with multiplanar reconstructions. The pathology was essentially confined to the lateral portion of the T12-L1 ligamenta flava resulting in stenosis of t

Reconstruction of the thermally injured upper extremity.

A rational approach to the restoration of function of the upper extremity has been outlined for the burned patient. If these guidelines are followed and the joints of the upper extremity approached individually with a goal of restoring as much range of motion as possible, great functional improvement may be anticipated. The surgical procedures must be augmented with vigorous PT and strong patient motivation. With this combination improvement of function of the upper extremity can be provided that is gratifying to both the surgeon and the patient.

Age is not a contraindication: outcomes of distal biceps tendon repair in elderly patients.

BACKGROUND: Distal biceps tendon ruptures are increasingly common in the elderly, yet no studies have specifically examined surgical outcomes in patients aged more than 65 years. This represents a significant knowledge gap given the growing elderly population and increasing activity levels among older adults. The purpose of this study is to determine major and minor complication rates in patients aged 65 years andmore undergoing surgical repair of distal biceps tendon ruptures and to compare outcomes with a matched younger cohort. METHODS: This retrospective cohort study identified 163 patien

Effect of Keeping Caspar Retractor During Prestige LP Cervical Disc Implantation on Intra- and Postoperative Outcomes.

OBJECTIVES: Implantation of the Prestige LP prosthesis requires substantial endplate modification and insertion forces, yet the manufacturer's guide recommends relaxing or completely removing the Caspar retractor before implantation, which we observed to cause vertebral instability during endplate preparation and prosthesis insertion. This study aimed to evaluate the effect of retaining the Caspar retractor during Prestige LP cervical disc implantation. METHODS: A total of 55 patients who underwent cervical disc arthroplasty (CDA) between January and December 2025 were retrospectively reviewe

Surgical technique, outcomes and complications of conversion total hip arthroplasty in ankylosed hip operated through the posterior approach: A retrospective cohort study.

BACKGROUND: Conversion total hip arthroplasty (THA) in ankylosed hips is technically challenging because of distorted anatomy, soft-tissue contractures, and compromised bone quality. Flexion, abduction, and external rotation deformities, common in inflammatory conditions, further restrict visualization of the femoral neck and pubofemoral arch when using the posterior approach. This study outlines technical modifications to overcome these challenges and reports the clinical and radiological outcomes of conversion THA performed through a posterior approach in patients with ankylosed hips.
