A Clinico-Histopathological Study of Nail Dermatoses in a Tertiary Care Hospital.

Introduction Nail disorders account for an important component of all dermatological conditions. Nail abnormalities can result from local pathology or systemic diseases. Pathologies can lead to pain and impaired fine touch and are aesthetically distressing. Clinical assessment of nail pathologies is seldom accurate; moreover, the limited available investigative modalities make it difficult to correctly diagnose the disorders. Nail biopsies provide crucial histological information, especially for nail-limited dermatoses, though they are infrequently used and technically challenging. Proper biop

Outcomes of in-office versus operating room insertion of tympanostomy tubes in children.

OBJECTIVE: Tympanostomy tube insertion in children is commonly performed under general anesthesia, but there has been increasing interest in office-based alternatives. Although initial research comparing in-office versus operating room (OR) insertion of tubes looks promising, there are scant data available on long-term outcomes. The objective of this study is to compare long-term outcomes of tympanostomy tubes placed in-office versus the OR, with emphasis on the duration of tube function. METHODS: We reviewed electronic medical records in an academic pediatric otolaryngology practice of child

Practical Review of Mechanical Ventilation in Adults and Children in the Operating Room and Emergency Department.

BACKGROUND: During general anesthesia, mechanical ventilation can cause pulmonary damage through mechanism of ventilator-induced lung injury, which is a major cause of post-operative pulmonary complications, which varies between 5 and 33% and increases the 30-day mortality of the surgical patient significantly. OBJECTIVE: The aim of this review is to analyze different variables which played a key role in the safe application of mechanical ventilation in the operating room and emergency setting. METHODS: Also, we wanted to analyze different types of the population that underwent intraoperativ

All-payer Spending on Common Hospital-based Services in California.

BACKGROUND: Hospital-based care accounts for one third of US health spending or over $1 trillion annually, yet a detailed all-payer assessment of what services contribute to this spending is not available. STUDY DESIGN: Cross-sectional and longitudinal evaluation of hospital financial statements from acute-care general hospitals in California between fiscal years 2007 and 2016. The amounts spent on 41 different revenue centers were included. The primary outcome was state-level and hospital-level spending for each revenue center including decomposing growth trends into changes in volume and pr

Microsurgical Salvage of Acute Lower Limb Ischemia after Iatrogenic Femoral Injury during Orthopedic Surgery in a Pediatric Patient.

Acute lower limb ischemia (ALLI) in the pediatric population is rare but may lead to limb loss and life-long complications. We report the technique and outcomes of microsurgical salvage of ALLI in a preterm newborn after open reduction of congenital hip dysplasia. A 2-month-old girl was born premature at week 36 with congenital bilateral hip dysplasia. The initial management attempted was conservative and entailed placement of bilateral Von Rosen splints. The treatment was successful on the left side, but the patient had recurrent unstable hip dislocation on the right side. Subsequently, 2 att

[Assessment of lumbar puncture skills in students, interns and residents attending hospital internship].

Although lumbar puncture is recognized as a great contributor to the diagnosis of some neurological diseases, the modalities of learning this procedure are still poorly defined in training programs for students attending their hospital internships. Apprehension related to the lack of experience and the fear of failure accelerates the abandonment of the practice. This study aims to assess lumbar puncture skills in the students at the Faculty of Medicine in Nouakchott as well as their subjective experience of this procedure. We conducted a survey of TCEM and DCEM 4 interns, residents and student

Transthoracic cardioversion of atrial fibrillation and flutter: standard techniques and new advances.

Direct current electric shocks have been used to terminate atrial arrhythmias (cardioversion) in humans since the 1960s. The likelihood of successful cardioversion and maintenance of sinus rhythm is increased if the left atrium is not markedly enlarged and fibrotic, if there is no marked left atrial hypertension (e.g., mitral stenosis), and if the arrhythmia is not long-standing. To minimize the risk of thromboembolic phenomena, therapeutic anticoagulation should be established for at least 3 weeks before and for 4 weeks after cardioversion; coumadin is usually used for this purpose. A more re

The Use of a Biologically Transparent Illumination Guide to Detect Gastric Tube Tip Position in Young Children From Diverse Clinical Backgrounds.

To confirm that the tip of a gastric tube is properly positioned within the stomach, commonly used methods include simple X-ray examination, the auscultation of gastric bubble sounds, and pH testing of aspirated fluids. However, these methods have limitations, such as concerns regarding radiation exposure and accuracy. We utilized the Tumguide® (Otsuka Pharmaceutical Factory, Inc., Tokushima, Japan), a gastric tube tip verification system that employs biologically transparent (BT) red light-emitting diode (LED) light, for gastric tube insertion in five pediatric patients with conditions consid

Child with progressive limb weakness caused by a neuroenteric cyst in the cervical segment: A case report.

RATIONALE: Neurenteric cyst is a rare congenital cystic lesion within the spinal canal that originates from the failure of the separation between the neural tube and the endoderm during the embryonic period. This disease is more commonly observed in the cervical and thoracic spinal cord. Pediatric cases are even rarer. The clinical manifestations are diverse. Early recognition and surgical intervention are crucial for the prognosis. PATIENT CONCERNS: The male child was 3 years and 5 months old. He presented for medical attention due to "neck pain accompanied by limited mobility for 10 days" a

Is transphyseal intramedullary fixation of the distal radius in pediatric fractures a safe procedure? An MRI study.

BACKGROUND: Radius and ulna fractures are very common in the pediatric population. Despite the use of pinning through the growth plate, which was proposed in the past and is still being used to treat these fractures, an instrumental validation to define this procedure as safe has not yet been done. Because of this, in the absence of reliable data regarding the passage of fixation devices through the growth plate, most surgical techniques used for treating radius and ulna fractures are based on absolute respect for the growth cartilage. We conducted an MRI pilot study to evaluate the presence o

Dental RECUR Randomized Trial to Prevent Caries Recurrence in Children.

The purpose of this study was to determine the efficacy of a dental nurse-delivered intervention-the Dental RECUR Brief Negotiated Interview for Oral Health (DR-BNI)-in reducing the recurrence of dental caries in children who have a primary tooth extracted. It was based on a 2-arm multicenter randomized controlled trial with blinded outcome assessment. Participants were 5- to 7-y-old children (n = 241) scheduled to have primary teeth extracted in 12 UK centers. Test intervention parents (n = 119) received DR-BNI led by trained dental nurses. DR-BNI is a 30-min structured conversation informed

[Clinical results of tissue-engineered vascular autografts seeded with autologous bone marrow cells].

UNLABELLED: In May 2000 we initiated clinical application of tissue-engineered (TE) vascular grafts seeded with cultured cells. However, cell culturing is time-consuming and xeno-serum must be used. To overcome these disadvantages, we started to use bone marrow cells (BMCs), readily available on the day of surgery, as a cell source. The aim of this study was to assess the safety and feasibility of this technique for creating pulmonary arteries. METHODS: Since August 2000, TE grafts seeded with autologous BMCs have been implanted in 42 patients. Bone marrow (5 ml/kg) was aspirated under genera

Identifying Risk Factors, Patient-Reported Experience and Outcome Measures, and Data Capture Tools for an Individualized Pain Prediction Tool in Pediatrics: Focus Group Study.

BACKGROUND: The perioperative period is a data-rich environment with potential for innovation through digital health tools and predictive analytics to optimize patients' health with targeted prehabilitation. Although some risk factors for postoperative pain following pediatric surgery are already known, the systematic use of preoperative information to guide personalized interventions is not yet widespread in clinical practice. OBJECTIVE: Our long-term goal is to reduce the incidence of persistent postsurgical pain (PPSP) and long-term opioid use in children by developing personalized pain ri

Accuracy of Non-Invasive Hemoglobin (nHgb) Monitoring in an AIS Population.

Local Problem: Needle phobia and fear of blood draws are very common in children and adolescents. Noninvasive hemoglobin (nHgb) monitoring in children was first introduced in the Intensive Care Unit (ICU) setting. Later, our total joint arthroplasty colleagues demonstrated that nHgB monitoring was more efficient, less expensive, and preferred by patients compared to invasive hemoglobin (iHgb) monitoring. Specific Aims: The goal of this Quality Initiative (QI) project is to compare the accuracy and reliability when comparing nHgb monitoring and iHgb (blood draw) in an Adolescent Idiopathic Scol

A prospective, open-label trial of clevidipine for controlled hypotension during posterior spinal fusion.

OBJECTIVES: Controlled hypotension is one means to limit or avoid the need for allogeneic blood products. Clevidipine is a short-acting, intravenous calcium channel antagonist with a half-life of 1 to 3 minutes due to rapid metabolism by non-specific blood and tissue esterases. To date, there are no prospective evaluations with clevidipine in the pediatric population. We prospectively evaluated the dosing requirements, efficacy, and safety of clevidipine for ontrolled hypotension during spinal surgery for neuromuscular scoliosis in the pediatric population. METHODS: Patients undergoing poster

Ultrasound-Based Assessment of Gastric Fluid Volume in the Pediatric Population Undergoing Elective Surgery: A Prospective, Randomized, Double-Blind Comparison of Carbohydrate-Rich Clear Fluid Versus Water at One- and Two-Hour Fasting Intervals.

BACKGROUND: Preoperative fasting guidelines have evolved from the traditional "nil per os after midnight" rule toward more liberal regimens. The European Society of Anaesthesiology and Intensive Care (ESAIC) and the Association of Pediatric Anesthetists of Great Britain and Ireland (APAGBI) now recommend a one-hour clear fluid fast for children, yet objective evidence quantifying the gastric fluid volume (GFV) resulting from carbohydrate-rich clear fluids at this abbreviated interval remains limited. AIMS: To evaluate the effect of one- versus two-hour fasting following the ingestion of 3 mL/
