Parietal oscillations code nonvisual reach targets relative to gaze and body.

Recent blood oxygenation level-dependent (BOLD) imaging work has suggested flexible coding frames for reach targets in human posterior parietal cortex, with a gaze-centered reference frame for visually guided reaches and a body-centered frame for proprioceptive reaches. However, BOLD activity, which reflects overall population activity, is insensitive to heterogeneous responses at the neuronal level and temporal dynamics between neurons. Neurons could synchronize in different frequency bands to form assemblies operating in different reference frames. Here we assessed the reference frames of os

Motion sickness in rally car co-drivers.

BACKGROUND: Car sickness is a frequent and potentially disabling problem, commonly related to a theory of sensory conflict, in particular visuo-vestibular, and between actual and anticipated sensory signals. This study aimed to evaluate predictors of motion sickness (MS) in rally car co-drivers exposed to various accelerations. METHODS: The subjects were 85 rally co-drivers (21 women) who filled in a questionnaire investigating MS symptoms in 4 situations: 1) special stages (competition itself); 2) special stages reconnaissance; 3) reading a book in the car; and 4) rear-seat passenger. The ma

The assessment of balance and dizziness in the TBI patient.

OBJECTIVES: All degrees of traumatic brain injury (TBI) are associated with balance dysfunction and/or dizziness. The clinician assessing and managing patients with TBI should become familiar with vestibular and non-vestibular causes of dizziness and imbalance, and be able to perform screening tests to determine when referral to a vestibular specialist is warranted. This chapter outlines the clinical pathways to be followed in history-taking, physical examination, and assessment. INTRODUCTION: Dizziness, vertigo, balance dysfunction and gait ataxia can have their origin in the vestibular syst

Modulation of corticomuscular coherence by peripheral stimuli.

The purpose of this study was to investigate the effects of peripheral afferent stimuli on the synchrony between brain and muscle activity as estimated by corticomuscular coherence (CMC). Electroencephalogram (EEG) from sensorimotor cortex and electromyogram (EMG) from two intrinsic hand muscles were recorded during a key grip motor task, and the modulation of CMC caused by afferent electrical and mechanical stimulation was measured. The particular stimuli used were graded single-pulse electrical stimuli, above threshold for perception and activating cutaneous afferents, applied to the dominan

Biomechanical factors and physical examination findings in osteoarthritis of the knee: associations with tissue abnormalities assessed by conventional radiography and high-resolution 3.0 Tesla magnetic resonance imaging.

INTRODUCTION: We aimed to explore the associations between knee osteoarthritis (OA)-related tissue abnormalities assessed by conventional radiography (CR) and by high-resolution 3.0 Tesla magnetic resonance imaging (MRI), as well as biomechanical factors and findings from physical examination in patients with knee OA. METHODS: This was an explorative cross-sectional study of 105 patients with knee OA. Index knees were imaged using CR and MRI. Multiple features from CR and MRI (cartilage, osteophytes, bone marrow lesions, effusion and synovitis) were related to biomechanical factors (quadricep

Spinal cord compression in two related Ursus arctos horribilis.

Two 15-yr-old grizzly bear littermates were evaluated within 9 mo of each other with the symptom of acute onset of progressive paraparesis and proprioceptive ataxia. The most significant clinical examination finding was pelvic limb paresis in both bears. Magnetic resonance examinations of both bears showed cranial thoracic spinal cord compression. The first bear had left-sided extradural, dorsolateral spinal cord compression at T3-T4. Vertebral canal stenosis was also observed at T2-T3. Images of the second bear showed lateral spinal cord compression from T2-T3 to T4-T5. Intervertebral disk di

Intact 2D-form recognition despite impaired tactile spatial acuity in complex regional pain syndrome type I.

Tactile acuity measured by 2-point discrimination performance is impaired in patients with complex regional pain syndrome type I (CRPS-I). This is mirrored by pain-associated shrinkage of the cortical representation of the affected limb. We investigated whether, also, more complex tactile performance assessed by a dynamic 2D-form perception task is disturbed in CRPS-I patients. Therefore, we developed a Braille-like recognition task (BT) for geometrical dot pattern identification by dynamic touch. We studied 47 healthy volunteers (Study I) and compared them to 16 CRPS-I patients (Study II). Be

Massive midline occipitotemporal resection of the skull for treatment of multilobular osteochondrosarcoma in two dogs.

CASE DESCRIPTION: Two 6-year-old male dogs were evaluated for removal of midline occipito-temporal multilobular osteochondrosarcomas. CLINICAL FINDINGS: Physical examination revealed mild ataxia in 1 dog and large masses of the central occipitotemporal portion of the skull in both dogs. Computed tomography, magnetic resonance imaging (MRI), or both revealed large bone-origin occipitotemporal masses with impingement of the brain and the sagittal and transverse venous sinuses. Three-dimensional contrast magnetic resonance image reconstruction delineated collateral venous circulation around the

The neural correlates of sign versus word production.

The production of sign language involves two large articulators (the hands) moving through space and contacting the body. In contrast, speech production requires small movements of the tongue and vocal tract with no observable spatial contrasts. Nonetheless, both language types exhibit a sublexical layer of structure with similar properties (e.g., segments, syllables, feature hierarchies). To investigate which neural areas are involved in modality-independent language production and which are tied specifically to the input-output mechanisms of signed and spoken language, we reanalyzed PET data

The placebo effect of ankle taping in ankle instability.

PURPOSE: Recurrence of ankle sprains is common among athletes. Although ankle taping reduces the risk of injury, the mechanism underlying its effectiveness remains unclear. Anecdotal reports suggest a role of the belief among athletes that taping will protect them from injury. That is, taping may have a placebo effect. The purpose of the present study was to determine whether there was a placebo effect with ankle taping in individuals with ankle instability. METHODS: Thirty participants with ankle instability completed a hopping test and a modified star excursion balance test under three cond

Modulation of reflex responses in hand muscles during rhythmical finger tasks in a subject with writer's cramp.

The objective of this study was to examine phase- and task-dependent modulation of stretch reflexes during repetitive finger movements in writer's cramp, and compare them with normal controls from our previous study. A subject with writer's cramp conducted two rhythmic tasks, index finger abduction (RFA) and a pen-squeezing (RPS) task akin to handwriting. Stretch reflexes were evoked by mechanical perturbations at random phases of each task. Surface electromyograms (EMG) were recorded from two hand muscles, first dorsal interosseous (FDI) and flexor digitorum superficialis (FDS). The reflex re

Under-rated neuro-otological symptoms: Hoffman and Brookler 1978 revisited.

In 1978, Hoffman and Brookler published an article in The Laryngoscope to challenge prevailing views on the lack of diagnostic power of certain symptoms often reported by patients to neuro-otologists. Some of these 'under-rated neuro-otological symptoms' include complaints of non-rotational dizziness, blurred and double vision, and the development of visual motion hypersensitivity in patients with balance disorders. In this review, I revisit these visual symptoms in the light of new findings from our laboratory. Double vision due to skew eye deviation can indeed occur in peripheral vestibular

Sequence variation in GAA repeat expansions may cause differential phenotype display in Friedreich's ataxia.

Friedreich's ataxia, the most common autosomal recessive inherited ataxia, is characterized by progressive gait and limb ataxia. Friedreich's ataxia is known for its occurrence within the first or second decade of life and is associated with hypertrophic cardiomyopathy, and in some cases with diabetes. Genetically, it is identified by the expression of an unstable trinucleotide GAA repeat expansion located in the first intron of the X25 gene on chromosome 9. Two brothers with very late adult-onset ataxia, and their unaffected sister, were examined for the clinical presentation of FA and for th

[Metaphorical aspects of mistletoe therapy in patients' experiences--a qualitative study].

OBJECTIVE: Mistletoe therapy is an item of research because of its immunological features. Nevertheless, mistletoe should be also an item of research because of its semiotics. Long before R. Steiner proposed mistletoe as an anticancer drug, this plant seemed to have its meaning not by pharmacodynamics but by pharmacosemiotics and magic. Thus, metaphorical aspects of mistletoe therapy should be described by processing the transcript and paintings from a therapeutic group session of cancer patients. Especially the relation between individually and historically coded metaphors should be looked at

Motor function in a patient with bilateral lesions of the globus pallidus.

This study describes the long-term deficits of a patient who, after a toxic encephalopathy, sustained extensive bilateral damage to both segments of the globus pallidus (GP) and the right substantia nigra (SN). There were no signs of lesions of the pyramidal tracts or of other motor structures. The most obvious deficits were an abnormal gait with an exaggerated knee extension and a tendency to fall slowly, especially when pushed backward. In contrast, Romberg's test on an unstable platform was normal, as were long-latency leg reflexes induced by perturbations. Inadequate anticipatory and compe

[Post-traumatic anomalous innervation of the lateral rectus muscle by trigeminal fibers].

BACKGROUND: Congenital or acquired anomalous synkinetic movements are common in Neuro-Ophthalmology. Examples are the Stilling-Türk-Duane syndrome, the Marcus Gunn phenomenon or secondary oculomotor synkinesis in oculomotor nerve palsy. PATIENT AND METHOD: A 29-year-old woman with N.V-, N.VI-, N.VII- and N.VIII-lesion after blunt cranio-cerebral trauma developed a left sided trigemino-abducens synkinesis. The abduction of the left eye by gaze effort was limited to 30 degrees before the midline. With tight jaw closure the eye reached the midline. Eye muscle surgery was performed (recess-resect

[Striatal involvement on MRI in adrenomyeloneuropathy].

Adrenomyeloneuropathy (AMN), a clinical variant of child adrenoleukodystrophy (ALD), is an adult-onset progressive disorder which presents spastic paraparesis with peripheral nerve involvement and affects mainly the pyramidal tracts from the brainstem to the spinal cord. We report a case of AMN in which serial MRI showed unusual development of areas of high signal in the right striatum. The patient was in good health until the age of 12, when he began to lose his hair. At age 25 he started to have progressive gait disturbance and erectile impotence. In his first admission to our hospital at ag
