[Surgical technics of kidney transplantation].

The renal transplantation operative technique obeys simple rules which must allow for surgical revision or a new transplantation. Ideally, the first transplantation is performed in the right iliac fossa, in a low, retroperitoneal position. A side-to-end arterial anastomosis is performed onto the external iliac artery, and an end-to-side venous anastomosis is performed onto the external iliac vein. In the case of a right kidney, the renal vein is elongated by means of a vena cava patch graft and the ureter is reimplanted into the bladder according to Gregoir's technique, intubated by a stent. Monitoring of emission as direct intraoperative control for nerve sparing retroperitoneal lymphadenectomy.

An intraoperative test to identify emission relevant lumbar postganglionic nerves during nerve sparing retroperitoneal lymph node dissection is presented. The neurophysiological course of the ejaculation into the posterior urethra, the emission, is described. A retroperitoneal nerve sparing procedure was done for nonseminomatous testicular tumors bilaterally in 7 patients with pathological stage I disease and unilaterally in 4 with pathological stage IIa disease. While the isolated lumbar nerves were electrostimulated, the seminal vesicles and bladder neck were monitored by suprapubic transves

Movement of abdominal structures on magnetic resonance imaging during positioning changes related to lateral lumbar spine surgery: a morphometric study: Clinical article.

OBJECT: The minimally invasive lateral interbody fusion of the lumbar spine through a retroperitoneal transpsoas approach has become increasingly used. Although preoperative imaging is performed supine, the procedure is performed with the patient in the lateral decubitus position. The authors measured the changes in location of the psoas muscle, aorta, inferior vena cava (IVC), iliac vessels, and kidneys with regard to the fixed lumbar spine when moving from a supine to a lateral decubitus position. METHODS: Unenhanced lumbar MRI scans were performed using a 3T magnet in 10 skeletally mature

Laparoscopic retroperitoneal lymph node dissection in testicular cancer: feasibility and safety at a tertiary center.

OBJECTIVE: To bear the evidence that laparoscopy is the possible new treatment of choice in the management of retroperitoneal disease in patients with testicular cancer. The surgical management of retroperitoneal lymph nodes is a crucial step in the multidisciplinary treatment of testicular germ cell tumors. Laparoscopic retroperitoneal approach poses technical challenges in urologic oncology. This study reports on the safety, efficacy, and short-term oncological outcomes of laparoscopic retroperitoneal lymph node dissection at a tertiary referral hospital. METHOD: Prospective, observational,

Can Contralateral Template Dissection be Avoided in Post-chemotherapy Retroperitoneal Lymph Node Dissection for Non-seminomatous Germ Cell Tumors?

BACKGROUND: Bilateral retroperitoneal lymph node dissection (RPLND) has been the standard of care in non-seminomatous germ cell tumor (NSGCT) with residual nodes following chemotherapy. Extensive dissection leads to increased morbidity, especially sexual dysfunction. Hence, avoiding contralateral RPLND in select patients can reduce morbidity without affecting oncological outcomes. This article explores the feasibility of one such option. PATIENTS AND METHODS: Patients diagnosed with testicular NSGCT with metastasis to the ipsilateral retroperitoneal (RP) lymph node template measuring ≤ 5 cm,

Risk factor analysis of intravesical recurrence after retroperitoneoscopic nephroureterectomy for upper tract urothelial carcinoma.

BACKGROUND: One of the major concerns of patients with upper tract urothelial carcinoma (UTUC) treated with nephroureterectomy is intravesical recurrence (IVR). The purpose of the present study was to investigate the predictive risk factors for IVR after retroperitoneoscopic nephroureterectomy (RNU) for UTUC. METHODS: Clinicopathological and surgical information were collected from the medical records of 73 patients treated with RNU for non-metastatic UTUC, without a history of or concomitant bladder cancer. The association between IVR after RNU and clinicopathological and surgery-related fac

Transabdominal open abdominal aortic aneurysm repair is associated with higher rates of late reintervention and readmission compared with the retroperitoneal approach.

OBJECTIVE: Limited data exist comparing the transabdominal and retroperitoneal approaches to open abdominal aortic aneurysm (AAA) repair, especially late mortality and laparotomy-related reinterventions and readmissions. Therefore, we compared long-term rates of mortality, reintervention, and readmission after open AAA repair through a transabdominal compared with a retroperitoneal approach. METHODS: We identified all patients in the Vascular Quality Initiative (VQI) undergoing open AAA repair from 2003 to 2015. Patients with rupture or supraceliac clamp were excluded. We used the VQI linkage

A case report of successful treatment of secondary aortoenteric fistula complicated with gastrointestinal bleeding and retroperitoneal abscess in an elderly patient.

RATIONALE: The treatment of secondary aortoenteric fistula (SAEF) involves maintaining hemodynamic stability, infection control, revascularization, and surgical repair. Conventional open repair is associated with high mortality, whereas endovascular stent-graft repair is associated with recurrent infection or bleeding. PATIENT CONCERNS: We report the case of an 85-year-old man with SAEF who presented with gastrointestinal bleeding and retroperitoneal abscess. DIAGNOSES: He was misdiagnosed for 5 months. SAEF was eventually diagnosed by CT and gastroduodenoscopy. INTERVENTIONS: The patient u

Inferior Vena Cava Filter Resulting in Perforation and Massive Retroperitoneal Hematoma Presenting as Acute Onset of Lower Extremity Weakness.

Perforation of inferior vena cava (IVC) filter struts is a common incidental finding on postoperative computed tomography (CT) scans that is not associated with bleeding or major complications. However, in rare circumstances, it can be associated with hemorrhage requiring immediate removal. We present a case of a 62-year-old man who developed abdominal pain and right lower extremity weakness 2 weeks after treatment of a pulmonary embolism with IVC filter placement and anticoagulation. A CT scan revealed a large right-sided retroperitoneal hematoma with active extravasation from the IVC filter

Improving Minimally Invasive Adrenalectomy: Selection of Optimal Approach and Comparison of Outcomes.

INTRODUCTION: Minimally invasive adrenalectomy is commonly performed by either a transperitoneal laparoscopic (TLA) or posterior retroperitoneoscopic (PRA) approach. Our group described the technique for robot-assisted PRA (RAPRA) in 2010. Few studies are available that directly compare outcomes between the available operative approaches. We reviewed our results for minimally invasive adrenalectomy using the three different approaches over a 10-year period. METHODS: Between January 2005 and April 2015, 160 minimally invasive adrenalectomies were performed. Clinicopathologic data were prospect

Kinking of the upper ureter in CT urography: anatomic and clinical significance.

PURPOSE: Although the course of the ureter is described as a straight descent in the retroperitoneum, kinking of the upper ureter is often seen at imaging. The aim of this study was to investigate kinking of the ureter and its underlying anatomico-clinical significance. METHODS: We evaluated 176 computed tomography (CT) images and classified kinking into three grades: no/mild kinking as Grade 1, moderate as Grade 2, and severe as Grade 3. We defined the "crossing point" (CP) as where the ureter crosses over the gonadal vein and assessed its relation to the kinking. Fourteen halves from seven

Malignant mesothelioma of the tunica vaginalis: a rare case report and description of multimodal treatment.

Malignant mesothelioma is an uncommon neoplasm that develops from serous surfaces, and rarely from the tunica vaginalis. Although atypical in any location, paratesticular presentation is exceedingly infrequent as only 0.3% to 1.4% of mesothelioma cases arise from the tunica vaginalis. Fewer than 300 cases have been reported with very few descriptions of long term follow up and multimodal therapy. Here we describe a patient with 2 years of follow up for metastatic mesothelioma treated with orchiectomy, chemotherapy and robot-assisted laparoscopic retroperitoneal lymph node dissection.

Anatomical relationships of the anterior blood vessels to the lower lumbar intervertebral discs: analysis based on magnetic resonance imaging of patients in the prone position.

BACKGROUND: Intra-abdominal vascular injuries are rare during posterior lumbar spinal surgery, but they can result in major morbidity or mortality when they do occur. We are aware of no prior studies that have used prone patient positioning during magnetic resonance imaging for the purpose of characterizing the retroperitoneal iliac vasculature with respect to the intervertebral disc. The purpose of this study was to define the vascular anatomy adjacent to the lower lumbar spine with use of supine and prone magnetic resonance imaging. METHODS: A prospective observational study included thirty
