[Effect of age on the natural history of the shoulder: a clinical and radiological study in the elderly].

PURPOSE OF THE STUDY: Classically, the shoulder joint is thought to age more by its tendons than by its cartilages, the incidence of rotator cuff tears being considered to increase with age. The purpose of this work was to assess the natural history of the shoulder joint based on a clinical and radiological study of 200 patients aged 70 to 101 years. MATERIAL AND METHODS: The cohort was composed of 48 men (34%) and 152 women (76%), aged 84.25 +/- 6.7 years. All patients underwent a physical examination and the Constant score was established. The radiographic examination included an AP view in

[What information do we need to treat the injured shoulder of a hockey player?].

Successful patient care of the injured shoulder of a hockey player involves interdisciplinary cupertino. Images allow an interpretation of a static dimension but may not be sufficient enough to develop a surgical treatment strategy, since all injuries to the shoulder have its origin in a dynamic pathology with some kind of external impact. This article classifies various traumatic shoulder problems and the mechanism of injury, it outlines clinical tests and stresses key facts an imaging report should include in order to plan conservative or surgical treatment. Basic treatment options as perfor

Anterior internal impingement of the shoulder in rugby players and other overhead athletes.

BACKGROUND/OBJECTIVE: Impingement syndromes are a common cause of shoulder pain in overhead athletes. Anterior internal impingement is a recently suggested mechanism for activity-related pain of the shoulder. Impingement syndromes were initially described to occur due to repetitive or excessive contact between the rotator cuff and other structures in the shoulder. METHODS: This is a retrospective, clinical case study reporting 54 consecutive cases of anterior internal impingement in overhead athletes, of which 28 (51.2%) cases were of rugby players. All had undergone physiotherapy without rel

Clinical and structural results of arthroscopic repair of isolated subscapularis tear.

BACKGROUND: Even though the frequency of arthroscopic repair of isolated lesions of the subscapularis tendon has increased, few studies have presented clinical and anatomical outcomes of this treatment. We hypothesized that, after an arthroscopic repair, structural outcomes in the muscle have an influence on functional results. METHODS: A retrospective study was performed on twenty-two patients who had undergone arthroscopic repair of an isolated tear of the subscapularis tendon and had a mean follow-up of thirty-six months. Patients were evaluated preoperatively and postoperatively with use

The impact of the underlying pathology and previous surgery on the long-term functional outcomes of the Delta Xtend reverse total shoulder arthroplasty: a prospective cohort study with a minimum follow-up of 11 years.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is a successful treatment option for rotator cuff tear arthropathy (CTA). Since its introduction, the range of indications has been expanded while implant designs have been enhanced. The purpose of this study is to evaluate the long-term clinical, radiographic, and patient-reported outcomes of the Delta Xtend rTSA. MATERIALS AND METHODS: In this prospective cohort study, 108 consecutive patients who underwent implantation of rTSA using the DePuy Delta Xtend in 2008 were included. Patients were divided into 4 indication groups: CTA (60%),

Immediate and short-term effects of kinesiotaping on muscular activity, mobility, strength and pain after rotator cuff surgery: a crossover clinical trial.

BACKGROUND: Kinesiotape (KT) is widely used in musculoskeletal rehabilitation as an adjuvant to treatment, but minimal evidence supports its use. The aim of this study is to determine the immediate and short-term effects of shoulder KT on muscular activity, mobility, strength and pain after rotator cuff surgery. METHODS: Thirty-nine subjects who underwent shoulder rotator cuff surgery were tested 6 and 12 weeks post-surgery, without tape, with KT and with a sham tape (ST). KT and ST were applied in a randomized order. For each condition, the muscular activity of the upper trapezius, three par

The high frequency of superior labrum, biceps tendon, and superior rotator cuff pathologies in patients with subscapularis tears: A cohort study.

BACKGROUND: The purpose of this study was to assess the frequency of superior labrum anterior posterior (SLAP) lesions, long head of biceps tendon (LHBT) pathologies, and superior rotator cuff tears accompanying subscapularis tears. We hypothesised that LHBT lesions, superior rotator cuff tears, and especially SLAP lesions were very frequent with subscapularis tears. METHODS: The digital files of patients who underwent shoulder arthroscopy were reviewed retrospectively. One hundred and eleven patients with subscapularis tears evident on surgery videos were examined. Superior labrum, LHBT, and

Effect of tamoxifen on fatty degeneration and atrophy of rotator cuff muscles in chronic rotator cuff tear: An animal model study.

Fatty degeneration of the rotator cuff muscles is an irreversible change resulting from chronic rotator cuff tear and is associated with poor clinical outcomes following rotator cuff repair. We evaluated the effect of Tamoxifen, a competitive estrogen receptor inhibitor, on fatty degeneration using a mouse model for chronic rotator cuff tear. Sixteen adult mice were divided into two diet groups (Tamoxifen vs. Regular) and subjected to surgical creation of a large rotator cuff tear and suprascapular nerve transection in their left shoulder with the right shoulder serving as a control. The rotat

Failure of operative treatment for glenohumeral instability: etiology and management.

Failure of primary shoulder stabilization procedures is often related to uncorrected anatomic pathology. Orthopaedic surgeons must recognize excessive capsular laxity or large glenohumeral bone defects preoperatively to avoid recurrence of instability. When history, physical examination, and radiographic evaluation are used in conjunction, patients at risk for failure can be identified. The instability severity index score permits precise identification of patients at risk. When treating patients in whom prior surgical intervention has failed, the success of revision procedures correlates to t

Incidence and treatment of postoperative stiffness following arthroscopic rotator cuff repair.

PURPOSE: The purpose of this study was to determine the incidence of clinically significant postoperative stiffness following arthroscopic rotator cuff repair. This study also sought to determine the clinical and surgical factors that were associated with higher rates of postoperative stiffness. Finally, we analyzed the result of arthroscopic lysis of adhesions and capsular release for treatment of patients who developed refractory postoperative stiffness 4 to 19 months (median, 8 months) following arthroscopic rotator cuff repair. METHODS: A retrospective review of a consecutive series of ar

[Biochemical analysis of the synovial fluid of the shoulder joint in patients with and without rotator cuff tears].

AIM: The biochemical changes associated with rotator cuff tearing are still unclear. The aim of this study is to assess whether concentrations of matrix metalloproteinase in the synovial fluid are specifically altered in shoulders with torn rotator cuff tendons. PATIENTS AND METHOD: Synovial fluid was extracted via arthroscopy in 21 patients with complete rotator cuff tears (RCTs). The control group was composed of 21 patients without complete tears. The catabolic cartilage metabolism markers MMP-1 (collagenase), MMP-3 (stromelysin1) and MMP-13 (collagenase3) were quantified by an ELISA test

Latissimus dorsi transfer for the treatment of irreparable rotator cuff tears.

BACKGROUND: Treatment of irreparable rotator cuff tears remains controversial. Latissimus dorsi transfer to the greater tuberosity has been proposed for the treatment of irreparable tears associated with severe functional impairment and chronic, disabling pain. METHODS: Sixty-seven patients with sixty-nine irreparable, full-thickness tears of at least two complete tendons were managed with latissimus dorsi transfer and were reviewed clinically and radiographically after an average of fifty-three months. The study group included fifty-two men and fifteen women, with an average age of sixty-one

Reconstruction of massive rotator cuff lesions with a synthetic interposition graft: a prospective study of 41 patients.

In the present prospective study, we report about our experience with massive rotator cuff tears treated by means of a non-resorbable transosseously fixated patch combined with subacromial decompression. Forty-one patients were followed clinically and radiographically for a mean duration of 43 months. Their mean preoperative Constant and Murley score improved (P<0.001) from 25.7 preoperatively to 72.1 postoperatively. Substantial pain relief and improvement in the performance of activities of daily living were obtained. Anatomically, the repair resulted in a mean acromiohumeral interval of 8.6

Pre-operative corticosteroid injections are associated with a dose-dependent risk for complications following anatomic and reverse total shoulderarthroplasty.

BACKGROUND: Corticosteroid injections (CSIs) are widely used for symptomatic relief in shoulder arthritis, yet concerns persist regarding their impact on post-operative outcomes following shoulder arthroplasty. This study evaluates the dose-dependent relationship between pre-operative CSIs and post-operative complications in patients undergoing anatomic or reverse total shoulder arthroplasty (TSA). METHODS: Using the PearlDiver insurance claims database (2010-2022), we identified patients undergoing TSA with at least one CSI in the preceding 5 years compared to those without any prior history

Broken Transverse Force Coupling as a Predictor of Poor Prognosis after Lower Trapezius Tendon Transfer.

BACKGROUD: Lower trapezius transfer (LTT) has emerged as a promising surgical option for irreparable posterosuperior massive rotator cuff tears (PSMRCTs), particularly for restoring external rotation and improving shoulder function. The transverse force couple, formed by the balanced action of the subscapularis anteriorly and the infraspinatus and teres minor posteriorly, is essential for maintaining shoulder stability and kinematics. However, outcomes remain variable, and predictors of suboptimal recovery are not fully understood. The objective of this study was to evaluate clinical and radio
