AIM: Orifice stenosis remained to be a common complication of hypospadias repair. We had modified the preputial island flap urethroplasty by folding and everting the distal end of the pedicle graft flap to prevent the neo-orifice from stenosis.
METHODS: Sixteen patients had undergone hypospadias repair using a modified onlay island flap technique. A urethral catheter was retained for 8 days to 10 days after operation.
RESULTS: Satisfactory results were seen in all the patients with a cosmetically fine appearance. One patient had a urinary tract infection and another, urethrocutaneous fistula

UNLABELLED: The male urethra with its anatomic and tissue characteristics demands a differential approach in its reconstruction. OBJECTIVES: 856 cases of hypospadias, operated over a period of 7 years, using various methods of one-stage repair, are reviewed in order to assess their functional and cosmetic results and their rate of complications.
PATIENTS AND METHODS: 856 boys with an average age of 3.5 years were classified according to the classification proposed by Duckett (anterior, middle and posterior hypospadias). Magpi, Mathieu's (flip-flap technique) and pedicled flaps tubularized or as onlay were utilized, depending upon the severity of the anomaly.

The aim of the investigation was to classify urethral variability as seen on micturating cystograms of 193 young girls and to study the physiological and pathological significance of the various shapes. It was shown that significant abnormalities were associated with all types of urethral configuration. Comparison of urethral shapes and clinical symptoms (dysuria, enuresis) did not show any particular appearance. Ultrasound diagnosis of bilateral hydronephrosis, hydroureters, and megacystis together with oligohydramnios secondary to urethral stenosis was made in a male fetus at 29 weeks' gestation. In utero decompression of the obstructed urinary tract and expansion of the amniotic fluid was achieved by placing an external bladder-amniotic fluid shunt using two catheters, one inserted into the fetal bladder and the other placed in the amniotic sac. The infant was delivered by cesarean section at 35 weeks' gestation and had a favorable outcome.

Uroflow diagrams, obtained by plotting the mean flow rate at 1-second intervals for controls and patients with bladder outflow obstruction, have been studied in 60 healthy controls and 100 patients. Standard patterns were obtained for male and female controls in 3 different age groups and for patients with various types of lower urinary tract obstruction. Outflow diagrams in age and sex-matched controls were compared to those obtained from patients, indicating the nature and the site of obstruction.

Vesicourethral reconstruction after radical retropubic prostatectomy was done by the Vest technique in 36 patients and by direct vesicourethral anastomosis in 100 patients. Complications resulting from the two methods of vesicourethral reconstruction were similar. Incontinence after radical retropublic prostatectomy appears not to be related to the method of vesicourethral reconstruction but occurs because of damage during surgery or postoperative scarring of the distal sphincteric mechanism.

Good results have been obtained following the treatment of incomplete urethral rupture while the results of complete urethral rupture treated in this hospital by the conventional method have been very poor. This poor result is comparable to those from other centers using similar methods. A review of various treatment methods was undertaken and reasons are given for advocating the method of initial suprapubic cystostomy, followed by delay urethroplasty.
