Effect of Real-Ear Adjusted Stimuli on Vestibular Evoked Myogenic Potential Variability in Children and Young Adults.

OBJECTIVES: There is large variability in cervical and ocular vestibular evoked myogenic potential (c- and oVEMP) amplitudes. One potential source of variability is differences in ear canal shape and size. Real ear-to-coupler difference (RECD) values are used to measure the acoustic environment of an individual's ear canal. RECD may be a useful measure to calibrate air conducted VEMP stimuli, which are elicited at high intensities and may put patients at risk of unsafe sound exposure. A recommendation for avoiding unsafe exposure is to use a 125 dB SPL stimulus for individuals with an equivale

The Importance of Frailty in Older Adults With Benign Paroxysmal Positioning Vertigo.

BACKGROUND AND PURPOSE: Even though Benign Paroxysmal Positioning Vertigo (BPPV) is one of the most reported vestibular disorders, its interaction with frailty and postural control in older adults is hardly or not investigated. METHODS: Thirty-seven older adults (≥65 years) with a diagnosis of BPPV (oaBPPV) (mean age 73.13 (4.8)) were compared to 22 age-, weight-, and height-matched controls (mean age 73.5 (4.5)). Modified Fried criteria were used to assess frailty. Postural control was assessed with the timed chair stand test, mini Balance Systems Evaluation test (mini-BESTest), a Clinical T

Episodic Ataxias: Primary and Secondary Etiologies, Treatment, and Classification Approaches.

BACKGROUND: Episodic ataxia (EA), characterized by recurrent attacks of cerebellar dysfunction, is the manifestation of a group of rare autosomal dominant inherited disorders. EA1 and EA2 are most frequently encountered, caused by mutations in KCNA1 and CACNA1A. EA3-8 are reported in rare families. Advances in genetic testing have broadened the KCNA1 and CACNA1A phenotypes, and detected EA as an unusual presentation of several other genetic disorders. Additionally, there are various secondary causes of EA and mimicking disorders. Together, these can pose diagnostic challenges for neurologists.

The Effects of 30 Minutes of Artificial Gravity on Cognitive and Sensorimotor Performance in a Spaceflight Analog Environment.

The altered vestibular signaling and somatosensory unloading of microgravity result in sensory reweighting and adaptation to conflicting sensory inputs. Aftereffects of these adaptive changes are evident postflight as impairments in behaviors such as balance and gait. Microgravity also induces fluid shifts toward the head and an upward shift of the brain within the skull; these changes are well-replicated in strict head-down tilt bed rest (HDBR), a spaceflight analog environment. Artificial gravity (AG) is a potential countermeasure to mitigate these effects of microgravity. A previous study d

Simultaneous transcutaneous electrical nerve stimulation mitigates simulator sickness symptoms in healthy adults: a crossover study.

BACKGROUND: Flight simulators have been used to train pilots to experience and recognize spatial disorientation, a condition in which pilots incorrectly perceive the position, location, and movement of their aircrafts. However, during or after simulator training, simulator sickness (SS) may develop. Spatial disorientation and SS share common symptoms and signs and may involve a similar mechanism of dys-synchronization of neural inputs from the vestibular, visual, and proprioceptive systems. Transcutaneous electrical nerve stimulation (TENS), a maneuver used for pain control, was found to influ

Comprehensive Evaluation of Healthy Volunteers Using Multi-Modality Brain Injury Assessments: An Exploratory, Observational Study.

Introduction: Even though mild traumatic brain injury is common and can result in persistent symptoms, traditional measurement tools can be insensitive in detecting functional deficits after injury. Some newer assessments do not have well-established norms, and little is known about how these measures perform over time or how cross-domain assessments correlate with one another. We conducted an exploratory study to measure the distribution, stability, and correlation of results from assessments used in mild traumatic brain injury in healthy, community-dwelling adults. Materials and Methods: In

Intracapsular decompression or radical resection followed by Gamma Knife surgery for patients harboring a large vestibular schwannoma.

OBJECT: Microsurgery is the primary treatment used for patients harboring a large vestibular schwannoma (VS). However, its outcome may lead to hearing impairment and facial nerve dysfunction particularly when resection is extended outside the tumor capsule. When surgery for a large VS consists of intracapsular resection and decompression, better preservation of facial and hearing function are obtained. In this study, the authors compared outcomes of intracapsular decompression followed by Gamma Knife surgery (GKS) with outcomes of standard microsurgery followed by radiosurgery. METHODS: Betwe

Paving the Way Toward Distinguishing Fallers From Non-fallers in Bilateral Vestibulopathy: A Wide Pilot Observation.

Patients with bilateral vestibulopathy (BVP) present with unsteadiness during standing and walking, limiting their activities of daily life and, more importantly, resulting in an increased risk of falling. In BVP patients, falls are considered as one of the major complications, with patients having a 31-fold increased risk of falling compared to healthy subjects. Thus, highlighting objective measures that can easily and accurately assess the risk of falling in BVP patients is an important step in reducing the incidence of falls and the accompanying burdens. Therefore, this study investigated t

Evaluating the effectiveness and complications of the Retrosigmoid, Translabyrinthine and Middle Fossa approaches in vestibular Schwannoma surgical management: a comprehensive systematic review and meta-analysis of 6,889 patients.

Surgical resection stands as one of the potential therapeutic methods for vestibular schwannomas (VS). However, in the management of patients with VS, there is limited literature directly comparing the Retrosigmoid approach (RSA) with the Translabyrinthine Approach (TLA) and Middle Fossa Approach (MFA). This gap arises because the MFA is typically reserved for smaller intracanalicular tumors, while the RSA and TLA are preferred for larger lesions. Our objective was to assess and compare the outcomes and safety profile of these three different surgical approaches. A comprehensive search was per

Clinical Observations in Patients With Cystic Fibrosis-Related Diabetes and Self-Reported Ototoxicity Symptoms.

PURPOSE: Persons with cystic fibrosis (PwCF) are at high risk for ototoxicity due to the routine use of intravenous aminoglycoside (IV-AG) antibiotics in respiratory infection management. Additionally, factors that contribute to ototoxicity-related symptom development and severity in PwCF are unknown. Given the increased risk of ototoxicity in people with diabetes, we explored the association between cystic fibrosis-related diabetes (CFRD) and self-reported ototoxicity symptoms (tinnitus and vestibular problems) in PwCF treated with aminoglycosides. METHOD: PwCF (N = 39; 25 females, 14 males;

Deformation of avian middle ear structures under static pressure loads, and potential regulation mechanisms.

Static pressure changes can alter the configuration and mechanical behavior of the chain of ossicles, which may affect the acoustic transfer function. In mammals, the Eustachian tube plays an important role in restoring ambient middle ear pressure, hence restoring the acoustic transfer function and excluding barotrauma of the middle and inner ear. Ambient pressure fluctuations can be potentially extreme in birds and due to the simple structure of the avian middle ear (one ossicle, one muscle), regulation of the middle ear pressure via reflexive opening of the pharyngotympanic tube appears all

Effects of alcohol on body-sway patterns in human subjects.

The vestibulospinal aspects of vestibular function are commonly neglected in the evaluation of alcohol-induced intoxication. Thus, in the present study the effect of an acute intoxication with a low or moderate quantity of alcohol was examined with respect to the equilibrium in 30 healthy subjects. The blood alcohol concentration (BAC) was measured 30 min after the ingestion of the last alcohol, ranging between 0.22 and 1.59 per thousand. Stability of stance was quantified by static platform posturography in Romberg-test conditions with eyes open and eyes closed. Among other parameters, the av

Blood immune profiling predicts surgical anatomy and facial nerve outcome in Vestibular Schwannoma.

BACKGROUND: Surgical resection of vestibular schwannoma (VS) is frequently complicated by unpredictable tumor fibrosis and adhesion to the facial nerve and brainstem, yet validated preoperative blood-based biomarkers for these intraoperative features are lacking. We tested whether peripheral blood immune profiles predict the intraoperative surgical anatomy of VS. METHODS: In a retrospective cohort of 342 VS patients undergoing microsurgical resection via the retrosigmoid approach, high-dimensional peripheral blood flow cytometry quantified T, B, and NK cell subsets, memory/activation status,

A consistent vigor network in Parkinson's disease: a potential target for brain stimulation effects.

INTRODUCTION: In motor tasks with repetitive trials, motor vigor varies substantially from trial to trial in people with Parkinson's disease (PD). A key question is whether neural variability reflects behavioral variability or whether additional variability emerges as the neural signal is translated into movement. We aimed to identify a vigor-related neural network that could provide a candidate target for brain stimulation. METHODS: We analyzed individual-trial fMRI data from 18 participants with PD who performed an overlearned ballistic hand-squeeze task in OFF- and ON-medication sessions w
