An Unusual Etiology of Cauda Equina Syndrome in an AIDS Patient: A Case Report.

Patients with advanced HIV/AIDS are at high risk for Primary central nervous system lymphoma (PCNSL) due to impaired immune surveillance and HIV-driven B-cell proliferation. While intracranial lesions are well recognized, spinal involvement remains underrecognized due to the scarcity of well-documented reported cases in the current literature. In this report, we describe the case of a 35-year-old male with a new HIV diagnosis who experienced acute bilateral lower extremity weakness and urinary retention during hospitalization for fever, perianal abscess, and urinary tract infection. Initial CD

Subacute anterior spontaneous spinal epidural hematoma at the thoracolumbar junction: A diagnostic confounder following necrotizing staphylococcal pneumonia.

Spontaneous spinal epidural hematoma (SSEH) is a rare cause of acute spinal cord compression, accounting for less than 1% of spinal space-occupying lesions. Its ventral localization is exceptionally uncommon due to the firm adherence of the dura to the posterior longitudinal ligament. We report the case of a 61-year-old woman with type 1 diabetes and a remote history of necrotizing Staphylococcus aureus pneumonia (treated 5 months prior) who presented with a 1-week history of progressive paraparesis and marked biological inflammatory markers (Leukocytes: 19,800/mm³; Eosinophils: 6800/mm³). Cefepime-induced encephalopathy following pancreaticoduodenectomy: a case report of a rare postoperative complication in gastrointestinal surgery.

We report a rare case of cefepime-induced encephalopathy (CE) after pancreaticoduodenectomy. An 83-year-old female patient underwent pancreaticoduodenectomy for a distal bile duct tumor. On postoperative day (POD) 6, she developed a pseudoaneurysm secondary to postoperative pancreatic fistula, for which interventional radiology (IVR) treatment was performed. A liver abscess developed after IVR treatment on POD 15. Multidrug-resistant P. aeruginosa was detected in a drain culture, prompting a change of antibiotic therapy to cefepime on POD 30. Antifungal therapy was initiated for fungemia; howe

Treatment of cerebral ventriculitis with a new self-irrigating catheter system: narrative review and case series.

Cerebral ventriculitis is a life-threatening condition that requires prompt and effective pharmacological intervention. The continuous irrigation of the cerebral ventricles with fluid and its drainage is a system to remove toxic substances and infectious residues in the ventricles; this system is called IRRAflow®. We used this kind of ventricular irrigation/drainage system to treat two patients with post-surgical cerebral ventriculitis and a patient with bacterial meningitis complicated with ventriculitis. In this case series, we discuss the management of these three cases of cerebral ventricu

The Value of Screening for Bicuspid Aortic Valve in First Degree Family Members.

A 54-year-old male with a history of hypertension, diabetes, and sleep apnea presented with a two-week history of dyspnea. The patient was hypoxic with bilateral leg edema. Initial workup showed elevated troponin at 0.15 ng/mL, brain natriuretic peptide of 720 pg/mL, and hyponatremia. Chest X-ray revealed lungs infiltrates with possible pneumonia. An electrocardiogram showed sinus tachycardia and ST depression in septal leads. He received diuretics and antibiotics for fluid overload and pneumonia. Blood culture showed methicillin-sensitive staphylococcus aureus (MSSA). Transthoracic echocardio

Epidural abscess presenting as severe depression with suicidal ideations: Case report.

BACKGROUND: Epidural abscess (EDA) is an uncommon form of intracranial infection that generally presents with fever, headache, and focal neurologic deficit. Imaging generally reveals a lentiform collection with diffusion restriction on diffusion weighted image. We present an interesting case in which a patient with EDA presented with three weeks of depression with suicidal ideations. The patient displayed no notable infectious signs and the imaging was suggestive of chronic subdural hematoma (SDH) rather than EDA. CASE DESCRIPTION: The patient is a 57-year-old man with past medical history si

Novel bi-allelic splice mutations in CARD9 causing adult-onset Candida endophthalmitis.

CARD9 deficiency (CANDF2; OMIM# 212050) is an autosomal-recessive monogenic inborn error of immunity conferring susceptibility to invasive fungal diseases, including the very distinct syndrome of spontaneous central nervous system candidiasis, in which opportunistic yeast of the genus Candida infect the central nervous system (either brain parenchyma and/or meninges) in the absence of trauma, chemotherapy or underlying systemic disease. We present a patient with spontaneous endophthalmitis of the right eye due to Candida albicans; further investigations revealed concomitant cerebral abscess. S

Opportunistic infections in patients with pulmonary alveolar proteinosis.

OBJECTIVES: To describe the demographics, clinical manifestations, treatment, and outcomes of patients with pulmonary alveolar proteinosis (PAP) who developed an opportunistic infection with Nocardia spp., mycobacteria or fungal pathogens. METHODS: A case of PAP and Nocardia spp. brain abscess is described. A comprehensive review of the English-language literature was conducted to identify all reported cases of PAP and opportunistic infections between 1950 and July, 2010. Candida parapsilosis meningitis associated with shunt infection in an adult male.

Candida parapsilosis is a very rare cause of meningitis. Though several cases have now been reported in neonates and children, only one has been described in an adult. We report on a 55-year-old male that was admitted due to altered mental status. He had recent sinus drainage and polypectomy, craniotomy with drainage of brain abscess, and ventriculo-peritoneal shunt placement. On admission, imaging studies showed no evidence of shunt dysfunction but did reveal extensive white matter decreased attenuation. Microscopic examination of the first 10 daily cerebrospinal fluid (CSF) cultures revealed…

Otitis media observed in unexpected natural death of infants.

Intracranial complications due to otitis media such as brain abscess and leptomeningitis are well known as a cause of death. In recent years, encountering those serious intracranial complications in forensic medical practice is extremely rare. However, we rarely encounter autopsy cases with otitis media of which pathological damage is mild or moderate. We investigated 11 autopsy cases (6 cases of pneumonia and 5 cases of SIDS) in unexpected natural death of infants under one year old, and found 3 cases with otitis media. The tympanic cavity was investigated by chiseling a petrosal part of the …

Fronto-ethmoid sinus mucocele: a case report.

BACKGROUND: Mucoceles are epithelium-lined cavities in the paranasal sinuses filled with mucus. They develop because of scarring and obstruction of the sinus ostium, whether from chronic sinusitis, trauma, or surgery. They commonly erode the bony sinus wall and can have serious complications of brain and orbital invasion, with potential for abscess and rupture. CASE REPORT: A 39-year-old woman had diplopia in left gaze and a hard nodule above the right eye for 1 year. She recently noted a bulging right eye, nasal congestion, and occasional headaches. She was 9 years post-sinus surgery. All as
