Brexanolone (SAGE-547 injection) in post-partum depression: a randomised controlled trial.

BACKGROUND: Post-partum depression is a serious mood disorder in women that might be triggered by peripartum fluctuations in reproductive hormones. This phase 2 study investigated brexanolone (USAN; formerly SAGE-547 injection), an intravenous formulation of allopregnanolone, a positive allosteric modulator of γ-aminobutyric acid (GABAA) receptors, for the treatment of post-partum depression. METHODS: For this double-blind, randomised, placebo-controlled trial, we enrolled self-referred or physician-referred female inpatients (≤6 months post partum) with severe post-partum depression (Hamilto

Telephone support for women during pregnancy and the first six weeks postpartum.

BACKGROUND: Telephone communication is increasingly being accepted as a useful form of support within health care. There is some evidence that telephone support may be of benefit in specific areas of maternity care such as to support breastfeeding and for women at risk of depression. There is a plethora of telephone-based interventions currently being used in maternity care. It is therefore timely to examine which interventions may be of benefit, which are ineffective, and which may be harmful. OBJECTIVES: To assess the effects of telephone support during pregnancy and the first six weeks pos

Postnatal depression in mothers bringing infants to the emergency department.

OBJECTIVE: To determine the prevalence of postnatal depression (PND) in mothers of young infants presenting to the emergency department (ED). DESIGN, SETTING AND PARTICIPANTS: Prospective observational study of the prevalence of PND in mothers of infants aged 14 days to 6 months presenting with non-time-critical conditions to the ED of a large tertiary paediatric hospital. MAIN OUTCOME MEASURES: We assessed PND by applying a self-administered validated screening tool, the Edinburgh Postnatal Depression Scale (EPDS). Mothers of patients were approached before clinician consultation when a soc

Universal prevention of depression in women postnatally: cluster randomized trial evidence in primary care.

BACKGROUND: To test whether receiving care from a health visitor (HV) trained in identification and psychological intervention methods prevents depression 6-18 months postnatally in women who are not depressed 6 weeks postnatally. METHOD: The study was a prospective cluster trial, randomized by GP practice, with follow-up for 18 months in 101 primary care teams in the Trent area of England. The participants were women scoring <12 on the postal Edinburgh Postnatal Depression Scale (EPDS) at 6 weeks postnatally (1474 intervention and 767 control women). Intervention HVs (n=89, 63 clusters) were

Home-based support for disadvantaged adult mothers.

BACKGROUND: Babies born to socio-economically disadvantaged mothers are at higher risk of a range of problems in infancy. Home visiting programs are thought to improve outcomes, both for mothers and children, largely through advice and support. OBJECTIVES: To assess the effectiveness of home visiting programmes for women who have recently given birth and who are socially or economically disadvantaged. SEARCH STRATEGY: We searched the following electronic databases: The Cochrane Central Register of Controlled Trials (CENTRAL) (Issue 3, 2006); MEDLINE (1966 to March 2006); EMBASE (1980 to 2006

Effect of antenatal breast milk expression or postnatal dietary intervention on breastfeeding outcomes and human milk composition in women with excess weight: protocol for the Ready, Set, Nourish study, a randomized controlled trial.

BACKGROUND: Human milk is recommended as the ideal source of nutrients intake for infants. It provides optimal nutrition and critical health benefits for both infants and mothers. However, women with excess weight are less likely to initiate and sustain breastfeeding. This study aims to assess how a prenatal and a postnatal intervention can improve breastfeeding outcomes, human milk composition, and infant growth in women with excess weight. METHODS: This three-arm randomized controlled trial will include 180 pregnant women with pre-pregnancy BMI ≥ 25 kg/m2 recruited from central Arkansas. Th

What is the best mode of delivery in nulliparous, singleton, term, vertex pregnancies.

With approximately 145 million births occurring worldwide each year-over 30 million by cesarean delivery (CD), the need for evaluation of maternal and perinatal outcomes in different delivery scenarios is more pressing than ever. Recently, in a meta-analysis of the available randomized controlled trials, planned CD was associated with significantly decreased rates of low umbilical artery pH, and neonatal complications such as birth trauma, tube feeding, and hypotonia when compared to planned vaginal delivery (VD). Among singleton pregnancies, planned CD was associated with a significantly lowe

A Prospective Longitudinal Quality Initiative toward Improved Enhanced Recovery after Cesarean Pathways.

OBJECTIVE: This study aimed to evaluate whether enhanced recovery after cesarean (ERAC) pathways reduces inpatient and outpatient opioid use, pain scores and improves the indicators of postoperative recovery. STUDY DESIGN: This is a prospective, longitudinal, quality improvement study of all patients older than 18 undergoing an uncomplicated cesarean delivery (CD) at an academic medical center. We excluded complicated CD, patients with chronic pain disorders, chronic opioid use, acute postpartum depression, or mothers whose neonate demised before their discharge. Lastly, we excluded non-Engli

Maternal screening for early postnatal vulnerability.

Research has highlighted the wide impact of maternal mental health problems during and beyond the postpartum period and the public health role of community health professionals in early detection of women who may be at risk. This paper aims to describe, explore and test an a priori hypothesised conceptual model of postnatal experience, identifying the relationships between postnatal mental vulnerability and postnatal adjustment to maternal roles and attitudes, marital/partner-relationship and sense of coherence. Three validated self-report questionnaires (WAST, MAMA, SOC) measuring the variabl

Prenatal antipsychotic exposure and neuromotor performance during infancy.

CONTEXT: Despite the expanding clinical utility of antipsychotics beyond psychotic disorders to include depressive, bipolar, and anxiety disorders, reproductive safety data regarding the neurodevelopmental sequelae of fetal antipsychotic exposure are scarce. OBJECTIVE: To examine whether intrauterine antipsychotic exposure is associated with deficits in neuromotor performance and habituation in 6-month-old infants. DESIGN, SETTING, AND PARTICIPANTS: A prospective controlled study was conducted from December 1999 through June 2008 at the Infant Development Laboratory of the Emory Psychologica

Endocrine aspects of postnatal mental disorders.

Biological research in postnatal mental illness has only a short history and few encouraging data have yet emerged. The most promising positive findings are perhaps preliminary evidence for an increase of postnatal depression in women with postpartum thyroid dysfunction, some evidence for an enhanced sensitivity to changes in progesterone levels in postnatal depression, and the presence of an opioid peptide with unknown function in the cerebrospinal fluid of women with puerperal psychosis. Although steroid hormones are generally thought to be aetiologically relevant since they freely enter the

A Rare Case of Arnold Chiari Malformation Type 1 Presenting With Features of Catatonia.

The Arnold Chiari malformation is a congenital neurological condition. It occurs due to a defect in the cerebellum. Our patient is a 19-year-old postpartum female who presented to our ER with headaches, crying spells, reduced interaction, poor self-care, and neglect of her newborn for the past five days. Before the onset of her presenting symptoms, the patient had complained of a severe headache in the back of the head and dizziness. Her baseline investigations were normal. On examination, the patient was noticed to have a fixed gaze, ambiguity, mutism, and rigidity. So, she was diagnosed with

Socioeconomic disadvantages and neural sensitivity to infant cry: role of maternal distress.

Socioeconomic disadvantage such as poverty can increase distress levels, which may further make low-income mothers more vulnerable to difficulties in the transition to parenthood. However, little is known about the neurobiological processes by which poverty and maternal distress are associated with risks for adaptations to motherhood. Thus, the current study examined the associations between income and neural responses to infant cry sounds among first-time new mothers (N = 28) during the early postpartum period. Lower income was associated with reduced responses to infant cry in the medial pre

The consequences of early-life adversity: neurobiological, behavioural and epigenetic adaptations.

During the perinatal period, the brain is particularly sensitive to remodelling by environmental factors. Adverse early-life experiences, such as stress exposure or suboptimal maternal care, can have long-lasting detrimental consequences for an individual. This phenomenon is often referred to as 'early-life programming' and is associated with an increased risk of disease. Typically, rodents exposed to prenatal stress or postnatal maternal deprivation display enhanced neuroendocrine responses to stress, increased levels of anxiety and depressive-like behaviours, and cognitive impairments. Some

Suicide Mortality During the Perinatal Period.

IMPORTANCE: The US has the highest maternal mortality rate among developed countries. The Centers for Disease Control and Prevention deems nearly all of these deaths preventable, especially those attributable to mental health conditions. Coordination between US health care and social service systems could help further characterize circumstances and risks associated with perinatal suicide mortality. OBJECTIVE: To examine contextual and individual precipitating circumstances and risks associated with perinatal suicide. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional observational study

Prenatal stress, partner support, and infant cortisol reactivity in low-income Mexican American families.

Maternal exposure to significant prenatal stress can negatively affect infant neurobiological development and increase the risk for developmental and health disturbances. These effects may be pronounced in low SES and ethnic minority families. We explored prenatal partner support as a buffer of the impact of prenatal stress on cortisol reactivity of infants born to low-income Mexican American women. Women (N=220; age 18-42; 84% Spanish-speaking; 89% foreign born; modal family income $10,000-$15,000) reported on economic stress and satisfaction with spousal/partner support during the prenatal p

Factors associated with depressive symptoms among postnatal women in Nepal.

BACKGROUND: Depression after childbirth affects both the mother and her infant. In South-Asia, maternal depression might also contribute to poor infant growth. Knowledge of risk factors could improve the health workers' recognition of depression. AIM: To examine possible risk factors for depression in the postnatal period among women in one clinical, one urban and one rural population in Lalitpur district, Nepal. METHOD: A total of 426 postnatal women were included in a cross-sectional structured interview study, 5-10 weeks after delivery. Depressive symptoms were measured by the Edinburgh P

Health-related quality of life determinants among Rwandan women after delivery: does antenatal care utilization matter? A cross-sectional study.

BACKGROUND: Despite the widespread use of antenatal care (ANC), its effectiveness in low-resource settings remains unclear. In this study, self-reported health-related quality of life (HRQoL) was used as an alternative to other maternal health measures previously used to measure the effectiveness of antenatal care. The main objective of this study was to determine whether adequate antenatal care utilization is positively associated with women's HRQoL. Furthermore, the associations between the HRQoL during the first year (1-13 months) after delivery and socio-economic and demographic factors we
