Measurement of Radiological Parameters of Distal Radius Fracture Using the Ulnar Axis Compared with the Radial Axis.

Background: The long axis of the radius is a standard reference for measuring radiological parameters in distal radius fractures. However, in patients with severe comminution and anatomical variation with distal radius bowing, it is challenging to assess using the long axis of the radius. The long axis of the ulna can be used as an alternative reference. The aim of this study is to assess the reliability and level of agreement using the long axis of the ulna as an alternative reference in comparison to the long axis of the radius. Methods: Posteroanterior (PA) radiography of the wrist in patie

Bilateral posterior fracture dislocation of the shoulder.

The authors report a case of shoulder bilateral posterior fracture dislocation in a 42-year-old man with osteogenesis imperfecta history. The importance of the communition, the large size of articular surface involved and the poor quality of the bone indicate a bilateral total shoulder arthroplasty. At midterm outcome, a good range of motion of both shoulders was registered.

[Reconstruction of the orbit floor using a resorbable polydioxanone (PDS degree) cup. Analysis of a series of 71 cases].

A 71 patients series is presented, concerning primary orbital floor reconstruction by a polydioxanone cup (PDS degree) which has been used for five years. The technical data and surgical procedure are exposed. No infection either intolerance manifestation has been observed until the materiel was resorbed. Two early displacements of the implant are reported, due to a technical fault during the insertion of the cup. Only one late enophtalmy was observed for a patient with a large orbital floor defect. The loss a the rigidity of the polydioxanone seems to be relatively fast (probably 2-3 months)

[The rotationally stable screw-anchor with trochanteric stabilizing plate (RoSA/TSP) : First results in unstable trochanteric femur fractures].

BACKGROUND: In unstable trochanteric fractures, the extramedullary rotationally stable screw-anchor (RoSA) combines the benefits of the load and rotational stability of the blade with the advantages of the screw (pull-out resistance, compression capability) in a single load carrier, and was designed to prevent femoral neck shortening by using an additional locked trochanteric stabilizing plate (TSP). OBJECTIVES: The aim of the current prospective cohort study was the clinical evaluation of the RoSA/TSP system regarding the mechanical re-operation rate and the amount of postoperative femoral n

Fractures of the lesser tuberosity of the humerus.

BACKGROUND: Fractures of the lesser tuberosity are rare injuries, and little is known of their epidemiology. Operative treatment is generally recommended for displaced fractures; however, the outcome of this method of treatment has not previously been studied. The aims of our study were to determine the approximate incidence of lesser tuberosity fractures, as well as the functional outcome following operative treatment in a consecutive series of patients. METHODS: Over an eight-year period, we studied the demographic details of a consecutive series of twenty-two adult patients who had a fract

Short versus long proximal femoral nail anti-rotation-II (PFNA-II) in the management of unstable intertrochanteric fractures.

OBJECTIVES: Unstable intertrochanteric (IT) fractures, particularly in elderly patients with low bone mineral density, pose significant treatment challenges. Proximal femoral nail anti-rotation-II (PFNA-II) is widely used, but the optimal implant length (short vs. long) remains debated. The objective of this study was to compare the clinical and functional outcomes of short versus long PFNA-II implants in unstable IT fractures. METHODS: A prospective comparative study was conducted at a tertiary hospital from November 2018 to November 2020. Adult patients (age ≥18) with recent (≤3 weeks) unst

Tension Suture Fixation of Olecranon Fractures.

BACKGROUND: Mayo type-IIA olecranon fractures are characterized by a transverse or short oblique fracture without articular comminution or ulnohumeral instability. Traditionally, these fractures are treated with a tension-band wiring technique. Despite good rates of fracture union, tension-band wiring is associated with a reoperation rate of 30% to 60%, usually for removal of prominent metalwork. The tension suture technique was developed as an alternative method of fixing Mayo type-IIA fractures using only high-tensile braided nonabsorbable number-2 sutures, with the aim of reducing the reope

A detailed account of Christopher Reeve's spinal cord injury, its treatment, and its impact upon research 30 years later.

On the screen, Christopher Reeve was Superman. Off-screen, Reeve was an avid equestrian who experienced arguably one of the most famous horse riding accidents to date when a fall from the saddle led to comminuted C1 and C2 fractures with an associated spinal cord injury that left him quadriplegic. Reeve publicly endured a grueling rehabilitation including personalized training plans and experimental treatments coupled with severe depression. With his family and friends by his side, Reeve shocked everyone when he was able to lift a finger on his left hand and feel the hugs of his family several

Influence of Screw Angulation on the Mechanical Properties on a Polyaxial Locking Plate Fixation.

Polyaxial locking systems are widely used for strategic surgical placement, particularly in cases of osteoporotic bones, comminuted fractures, or when avoiding pre-existing prosthetics. However, studies suggest that polyaxiality negatively impacts system stiffness. We hypothesize that a new plate design, combining a narrow plate with asymmetric holes and polyaxial capabilities, could outperform narrow plates with symmetric holes. Three configurations were tested: Group 1 with six orthogonal screws, and Groups 2 and 3 with polyaxiality in the longitudinal and transverse axes, respectively. A bi

Ablation, Osteoplasty, Reinforcement, and Internal Fixation as a New Alternative First-Line Management for Osteolytic Pelvic Metastases.

Many orthopaedic procedures for osteolytic metastases are performed using surgical techniques and implants that are used in arthroplasties or trauma surgeries. There is a need for development of skeletal metastasis-specific procedures. Massive osteolytic metastases in periacetabular regions are managed with open surgical procedures when radiation and antiresorptive agents fail to prevent development and progression of the lesion. An ideal procedure for osteolytic metastases would decrease cancer burden and stabilize weakened bones for continued ambulatory oncologic care without delay. Therefor

Hybrid versus single internal fixation strategies in minimally invasive surgery for multiple rib fractures: a retrospective comparative analysis.

BACKGROUND: Surgical stabilization of rib fractures typically employs a single type of internal fixation material. While this "Single Strategy" offers the advantage of procedural consistency and facilitates a rapid learning curve for surgeons, it often fails to address the heterogeneity of multiple rib fractures (MRF). Factors such as fracture location, segment length, degree of comminution, and associated intrathoracic visceral injuries frequently render a single material insufficient for all fracture types within a single patient. Consequently, we adopted a clinical approach utilizing multip

Fixation of 4-part fractures of the proximal humerus: Can we identify radiological criteria that support locking plates or IM nailing? Comparative, retrospective study of 107 cases.

INTRODUCTION: No objective criteria exist to help surgeons choose between IM nailing and plate fixation for 4-part fractures of the proximal humerus. The goal of this study was to identify radiological criteria that would make one technique a better choice than the other. MATERIAL AND METHODS: This was a comparative, multicentre, retrospective study of 54 cases of antegrade nailing and 53 cases of plating performed between 1st January 2009 and 31 December 2011 for 4-part fractures of the proximal humerus. All patients had a minimum radiological and clinical follow-up of 18 months. The functio
