Development and optimization of Fe₃O₄@SiO₂-xanthine oxidase magnetic nanoparticles for rapid screening of XOD inhibitors from Trichosanthes anguina (snake gourd) with molecular docking approach.

Xanthine oxidase (XOD) inhibitors are central to hyperuricemia and gout management, but efficient, reusable enzyme-based screening platforms remain limited. In this study, XOD immobilized Fe₃O₄@SiO₂- nanoparticles (Fe₃O₄@SiO₂-XOD NPs) were developed and optimized as a reusable screening platform and applied to identify bioactive XOD inhibitors from Trichosanthes anguina (Snake Gourd). Fe₃O₄@SiO₂-XOD NPs were synthesized via glutaraldehyde-mediated crosslinking and characterized by FTIR, XRD, and SEM. Optimal immobilization was achieved at 15% glutaraldehyde, pH 7.5, 25 °C, and 1 h incubation. …

Transient neonatal diabetes mellitus as an early diagnostic clue to HNF1B-related disease - two case reports and a literature review.

BACKGROUND: Pathogenic variants in the HNF1B gene cause a multi system disorder encompassing organ abnormalities-primarily affecting the kid … ctrum. Although, in both cases, kidney abnormalities and extra-renal features (NDM, hypomagnesemia, hyperuricemia) were observed, both patients experienced a delay in diagnosis. On follow-up, serial oral glucose tolerance tests (OGTT), HbA1c assessments, and glucagon stimulation tests to date demonstrated preserved β-cell function, with the exception of hypergl …

Biological Therapies for Urate Lowering and Inflammation Control in Gout Management.

Gout remains the most prevalent inflammatory arthritis worldwide, driven by hyperuricemia and monosodium urate crystal-induced inflammation. Conventional therapies manage most patients effectively, but refractory disease and contraindications in special populations create unmet clinical needs. This gap is particularly evident among patients with advanced chronic kidney disease or organ transplants. Biologic therapies targeting key pathophysiologic mechanisms have emerged as specialized options for these difficult-to-treat cases. Pegloticase achieves sustained urate reduction in refractory gout…

Antithrombin in Hyperuricemic Nephropathy: Unveiling Its Noncanonical Renoprotective Roles and Therapeutic Repurposing Potential.

The pathogenesis of hyperuricemic nephropathy (HN) is increasingly linked to a self-perpetuating cycle of thromboinflammation. This process begins with hyperuricemia-induced endothelial dysfunction, which activates both the coagulation cascade and inflammatory pathways, notably the pyrin domain-containing protein 3 inflammasome. This creates a vicious cycle of which inflammation promotes coagulation and coagulation products further amplify inflammation, ultimately leading to renal microvascular thrombosis, ischemia, and progressive fibrosis. This review critically examines the therapeutic pote…

Dendrophenol modulates the LRRK1/PIK3AP1 signaling axis: a novel strategy to mitigate inflammation and pyroptosis in gouty arthritis.

BACKGROUND: Gouty arthritis (GA) is caused by hyperuricemia and the articular deposition of urate crystals, for which current treatments remain suboptimal. Considering the anti-inflammatory properties of Dendrobium extracts, especially Dendrophenol (Den), this study conducted an in-depth investigation into its effects on GA. METHODS: Through methylthiazolyldiphenyl-tetrazolium bromide (MTT) test, the impact of Den on bone marrow-derived primary macrophages (BMDMs) was assessed to ascertain its non-toxic dosage range. The GA cell model was established, subjected to Den (0.5 µM) treatment, and …

Spatial scaling of metagenomic diversity reveals ecological disruption in the gut microbiome of gout patients.

Gout, a painful inflammatory arthritis, is characterized by hyperuricemia and monosodium urate crystal deposition, with growing evidence linking its pathogenesis to gut microbiome dysbiosis. However, traditional diversity metrics fail to capture the complex spatial organization of microbial communities. This study addresses this gap by applying the novel metagenomic Diversity-Area Relationship (m-DAR) model to investigate scaling laws in the gout microbiome-quantifying how metagenomic diversity changes with the number of individuals sampled. Our analysis of gut microbiomes from gout patients a…

Design, synthesis, and evaluation of N-substituted indolyl-diazine derivatives as potent xanthine oxidase inhibitors.

Xanthine oxidase (XO) is a key enzyme in purine metabolism and a well-established target for treating hyperuricemia and gout. However, clinically used XO inhibitors are limited by suboptimal efficacy and adverse effects, underscoring the necessity for novel scaffolds. N-substituted indolyl-heterocycle scaffold has emerged as an effective and promising framework for XO inhibition, with isoxazole incorporation in our previous study shown to yield derivatives exhibiting favorable activity in vitro. In this study, we further investigated this scaffold by systematically introducing a series of six-…
