Value of laparotomy in the diagnosis of obscure gastrointestinal haemorrhage.

Over a nine year period a total of 137 patients were investigated for obscure gastrointestinal bleeding on one surgical unit. In 20 patients visceral angiography strongly suggested the presence of caecal or right colonic angiodysplasia. These patients were treated by an appropriate colectomy and they are not considered further in this study. Similarly lesions of the small bowel detected by preoperative investigations are not considered here. Fifty five patients were offered diagnostic laparotomy after the failure of other investigations to establish a diagnosis. Two patients refused. A diagnos

Surgical perspectives of symptomatic omphalomesenteric duct remnants: Differences between infancy and beyond.

BACKGROUND: The clinical manifestations of omphalomesenteric duct remnant (OMDR) can vary with the age at diagnosis, from asymptomatic incidental findings to symptoms related to gastrointestinal complications. The lifelong complication rates are reported as 4%-34%, and complications are more common in patients younger than 2 years of age. The authors attempted to identify different clinical features and management for the various pediatric age groups. AIM: To find surgical perspectives for the pediatric age-related variants of OMDR and make recommendations for optimal management. METHODS: Th

Evaluating surgical strategies for small bowel obstruction in virgin abdomen: A comparative analysis of laparoscopic and open approaches.

Small bowel obstruction in virgin abdomen (SBO-VA) is rarely encountered and there is no standard treatment strategy. Even though nonoperati … s, open surgery was conducted. Adhesions (60%) were the most common underlying etiology followed by Meckel diverticulum (17.4%). Pathological examination revealed malignancy in 4 (11.43%) patients. The diagnostic accuracy of computed tomography performed preoperatively was calculated as 34.3%. In the laparoscopic surgery group, the length of incision, Clavien-Dindo C …

Heterotopic Pancreas: Histopathologic Features, Imaging Findings, and Complications.

Heterotopic pancreas is a congenital anomaly in which pancreatic tissue is anatomically separate from the main gland. The most common locations of this displacement include the upper gastrointestinal tract-specifically, the stomach, duodenum, and proximal jejunum. Less common sites are the esophagus, ileum, Meckel diverticulum, biliary tree, mesentery, and spleen. Uncomplicated heterotopic pancreas is typically asymptomatic, with the lesion being discovered incidentally during an unrelated surgery, during an imaging examination, or at autopsy. The most common computed tomographic appearance of…

Single-Instrument Thoracic and Abdominal Surgery in Children: Minimizing Minimally Invasive Surgery.

Introduction: The use of single-port surgery is widely accepted in pediatric surgery, but the majority of reports are on its use for appendectomy or inguinal hernia repair using multiple instruments. The aim of this report is to demonstrate that both thoracic and abdominal single-instrument procedures are feasible and safe in children. Materials and Methods: The following cases were managed in our department for the past 12 months. Two types of telescopes (10- and 5-mm) with inbuilt working channels were used in all cases. Results: The 10-mm endoscope with a 6-mm inbuilt channel was used to pa

The role of dexamethasone in decreasing early recurrence of acute intussusception in children: A retrospective study.

BACKGROUND: Early recurrent intussusception (RI) is a concern after a successful pneumatic reduction. Steroids have been suggested as a treatment that decreases early RI. The purpose of this study was to examine the role of dexamethasone in decreasing early RI. METHODS: A retrospective review of 174 pediatric patients that underwent successful pneumatic reduction was conducted. Two groups were identified: group 1 that received intravenous dexamethasone (0.5mg/kg/dose) on diagnosis or immediately after the reduction maneuver, and group 2 that were not given dexamethasone. RESULTS: There were

[Mechanical ileus in children with no prior history of abdominal surgery].

In children with no prior history of abdominal surgery and no signs of intussusception or incarcerated inguinal hernia, mechanical ileus may have a congenital cause such as malrotation with volvulus or a persistent omphalomesenteric duct. Acquired causes include sigmoid volvulus. We present two cases of mechanical ileus in children. The first case involved a 6-year-old boy who presented with acute abdominal pain and vomiting. An emergency laparotomy was performed, with resection of the omphalomesenteric duct. Recovery was uneventful. The other case concerned a 9-year-old boy who presented with

Evisceration of small bowel after cauterization of an umbilical mass.

The omphalomesenteric duct (OMD), a temporary structure essential to fetal development, normally involutes completely by week 8 or 9 of gestation. On occasion, the OMD persists, the clinical presentations of which vary widely. We describe a case of a 6-week-old male with a patent OMD remnant that was initially treated as an umbilical granuloma, which then potentially allowed for prolapse of the small bowel through the umbilical ring. The patient required resection of the incarcerated bowel but had an otherwise uneventful and complete recovery.

Acute abdomen. The role of laparoscopy.

The child with an acute abdomen presents the perfect opportunity to take advantage of the benefits of minimal-access surgery. There are a wide variety of conditions, acquired and congenital, for which minimal-access techniques provide the distinct advantages of a cost-effective diagnosis and therapy with minimal discomfort for the patient. In the present report, the management of children with abdominal pain of unclear etiology is outlined, along with discussions of the value of minimal-access surgery in appendicitis and other conditions. Several algorithms are provided as a guideline for sugg

Idiopathic localized dilatation of the ileum in adults: findings on barium studies.

OBJECTIVE: Idiopathic localized dilatation of the ileum is a rare entity characterized by a sharply demarcated segmental dilatation of the small bowel that is in line with the lumen. It is probably congenital in origin. Although more commonly diagnosed in children, it presents in adults as occult gastrointestinal (GI) bleeding or less often with abdominal pain. On pathologic examination, the mucosa may be ulcerated, but otherwise the wall is relatively normal. We analyzed the radiographic findings in nine previously unreported cases of this condition in adults. MATERIALS AND METHODS: Collabor

False-positive Meckel's scan in a case of infected ovarian cyst.

Abdominal scanning using sodium pertechnetate Tc 99m is useful in demonstrating Meckel's diverticula containing ectopic gastric mucosa. However, other structures like the uterus may also concentrate 99mTc. We present a case which showed a focal area of increased uptake in the pelvis due to an infected ovarian cyst.

Gastrin-producing cells in ectopic gastric mucosa of developmental and metaplastic origins.

Immunocytochemical techniques using antigastrin antibody were employed to localize G cells in ectopic gastric mucosa of metaplastic and congenital origins and to compare their distribution with that in normal gastric mucosa. Five examples of Barrett's esophagus, 8 Meckel's diverticula, and 2 small bowel duplications were studied. Although G cells were absent in the gastric mucosa from all cases of Barett's esophagus, four Meckel's diverticula and one small bowel duplication contained G cells. In all instances of congenitally derived ectopic gastric mucosa where G cells were demonstrable, the g
