[Nasal glial heterotopia in children: a review of the literature and a series of our own observations].

UNLABELLED: Nasal glial heterotopia (nasal glioma) is a rare congenital benign accumulation of glial tissue displaced into the nasal cavity or paranasal sinuses. OBJECTIVE: To analyze our own clinical cases of nasal glioma (features of the clinic, diagnosis and treatment) and compare them with data from foreign literature. MATERIAL AND METHODS: A retrospective review of medical histories with a diagnosis of nasal glial heterotopia was conducted from 2017 to November 2023 at the pediatric otorhinolaryngological department of the National Medical Research Center for Otorhinolaryngology. Compla

Neuroendoscopic management of a third ventricle hydatid cyst presenting with obstructive hydrocephalus in a 4-year-old female: a case report and review of the literature.

BACKGROUND: Hydatidosis is a parasitic disease caused by the larvae of Echinococcus granulosus, typically acquired through ingestion of contaminated food. While hydatid cysts commonly develop in the liver and the lungs, the central nervous system is rarely affected, and the cyst predominantly develops within the cerebral parenchyma. However, the ventricular system involvement remains extremely rare. Among the cases of intraventricular hydatid cysts, the lateral ventricles are the mostly affected, while the involvement of the third ventricle remains exceedingly rare, with only five cases previo

Arterial spin labeling detects trapped labeled spins in flow-diverted aneurysms and it reflects intra-aneurysmal flow stasis.

BackgroundThis study evaluated arterial spin labeling (ASL) imaging findings in flow-diverted aneurysms (FD-ANs).MethodsTwenty-one patients with internal carotid artery aneurysms treated with flow-diverter stent (FDS) were included. Time-of-flight magnetic resonance angiography (TOF-MRA) and ASL were performed before and after follow-up digital subtraction angiography (DSA) on the day, 6 months, and/or 1 year after treatment. Two radiologists rated the clarity of the depiction of FD-ANs for MRA on a 3-point scale and the ASL signal intensity of the aneurysm as negative or positive. A neurosurg

A case with a trend of QT interval prolongation due to the introduction of methadone to a pancreatic cancer patient on levofloxacin.

BACKGROUND: As methadone can prevent the development of opioid resistance, it has application in alleviating cancer-related pain that proves challenging to manage with other opioids. QT interval prolongation is a serious side effect of methadone treatment, with some reported deaths. In particular, owing to the increased risk of QT interval prolongation, caution should be exercised when using it in combination with drugs that also prolong the QT interval. CASE PRESENTATION: This study presents a case in which methadone was introduced to a patient (a man in his 60s) already using levofloxacin,

Do Not Forget the Spine MRI in Children With Arthrogryposis Multiplex Congenita: High Prevalence of Tethered Spinal Cord and Preliminary Clinical Findings Following Detethering.

BACKGROUND: Arthrogryposis multiplex congenita (AMC) encompasses congenital conditions with joint contractures in 2 or more joints. Patients with AMC may have scoliosis and neural axis malformations. The study aimed to determine the prevalence of tethered spinal cord (TSC), characterized by a low-lying conus medullaris, and secondarily, present preliminary findings following surgical untethering in children with AMC. METHODS: Patients 18 years of age and younger with a diagnosis of AMC and a spine MRI were identified. The presence of a TSC was defined as a low-lying conus with termination at

Rapid Ventricular Pacing for Clipping of Intracranial Aneurysms: A Single-centre Retrospective Case Series.

OBJECTIVE: Multiple strategies exist to facilitate microdissection and obliteration of intracranial aneurysms during microsurgical clipping. Rapid ventricular pacing (RVP) can be used to induce controlled transient hypotension to facilitate aneurysm manipulation. We report the indications and outcomes of intraoperative RVP for clipping of ruptured and unruptured complex aneurysms. METHODS: We completed a retrospective review of adult patients who underwent RVP-facilitated elective and emergent microsurgical aneurysm clipping by a single senior neurosurgeon between 2016 and 2023. Intraoperativ

Surgical management of sporadic and schwannomatosis-associated pelvic schwannomas.

Pelvic schwannomas are rare tumors that may occur either sporadically or in the context of schwannomatosis. We retrospectively reviewed the charts of patients harboring a pelvic schwannoma under conservative management or operated at our reference center between 2016 and 2023. All patients were operated by a multidisciplinary team, combining a vascular surgeon and a neurosurgeon. Twenty-four patients harboring 33 pelvic tumors were included in the cohort, including 12 patients with sporadic lesions, 2 patients with NF2-related schwannomatosis, and 10 patients with NF2-independent schwannomatos

Cranial Versus Cervical Spine Magnetic Resonance Imaging in Adult Chiari Malformation Type I Diagnostics: Is There a Difference in Tonsil Ectopia Length?

BACKGROUND: Chiari malformation type 1 (CM-1) diagnosis is based on measurement of the cerebellar tonsils on cranial or cervical spine magnetic resonance imaging (MRI). However, imaging parameters of cranial and cervical spine MRI could differ because spine MRI has greater resolution. METHODS: We conducted a retrospective chart review of 161 patients of a single neurosurgeon for adult CM-I consultation between February 2006 and March 2019. Patients were selected based on receiving both cranial and cervical spine MRI within a month of each other to determine tonsillar ectopia length for CM-1.

Dural Invasion as a Marker of Aggressive Pituitary Adenomas.

BACKGROUND: Predictive markers of aggressive pituitary tumors have not been consistently demonstrated. Dural invasion and invasion-associated proteins, including matrix metalloproteinase-9 (MMP9) and cofilin, have been proposed to predict aggressive behavior and recurrence, but findings to date have been inconsistent. OBJECTIVE: To assess whether microscopic dural invasion predicts aggressive pituitary adenoma behavior and whether MMP9 and cofilin expression correlates with pathological and clinical invasion markers. METHODS: We retrospectively studied 328 consecutive pituitary mass resectio

The impact of authors' medical specialty on publication patterns and published results of adjuvant radiotherapy for WHO grade 2 meningiomas-a systematic review.

BACKGROUND: The role of adjuvant radiotherapy after gross total resection (GTR) of WHO grade 2 meningioma remains unclear, and conflicting results have been published. We hypothesized that authors' medical specialties could be associated with reported findings on the role of adjuvant radiotherapy after GTR of WHO grade 2 meningiomas. METHOD: A systematic review was conducted in Embase and Medline databases, in addition to screening of all relevant bibliographies. Articles including patients aged 18 years or older, with histologically confirmed WHO grade 2 meningioma, were included. We extract

Feasibility, Safety and Impact on Overall Survival of Awake Resection for Newly Diagnosed Supratentorial IDH-Wildtype Glioblastomas in Adults.

BACKGROUND: Although awake resection using intraoperative cortico-subcortical functional brain mapping is the benchmark technique for diffuse gliomas within eloquent brain areas, it is still rarely proposed for IDH-wildtype glioblastomas. We have assessed the feasibility, safety, and efficacy of awake resection for IDH-wildtype glioblastomas. METHODS: Observational single-institution cohort (2012-2018) of 453 adult patients harboring supratentorial IDH-wildtype glioblastomas who benefited from awake resection, from asleep resection, or from a biopsy. Surgical Outcome of Large Solid Posterior Fossa Hemangioblastoma without Preoperative Embolization.

BACKGROUND: Large solid hemangioblastoma in the posterior fossa has an abundant blood supply as an arteriovenous malformation. The presence of adjacent vital neurovascular structures makes them vulnerable and difficult to operate. Complete surgical resection is always a challenge to the neurosurgeon. MATERIAL AND METHOD: We share the surgical difficulties and outcome in this case series of large solid hemangioblastomas without preoperative embolization as an adjunct. This study included five patients (three men and two women, with a mean age of 42.2 years). Preoperative embolization was attem

Rapid closure technique in suboccipital decompression.

OBJECTIVE: Suboccipital decompression has been established as standard therapeutic procedure for raised intracranial pressure caused by mass-effect associated pathologies in posterior fossa. Several different surgical techniques of dural closure have been postulated to achieve safe decompression. The aim of this study was to examine the differences between fibrin sealant patch (FSP) and dural reconstruction (DR) in suboccipital decompression for acute mass-effect lesions. METHODS: We retrospectively analyzed our institutional data of patients who underwent suboccipital decompression due to sp

A case series and review of the mononostril endoscopic transnasal transsphenoidal approach: Safe and effective in a low resource setting.

BACKGROUND: A transnasal transsphenoidal (TNTS) approach can be performed through a binostril or mononostril technique. The binostril technique is generally preferred, however the mononostril may be an underutilized approach with significant benefits. METHODS: All (n = 521) pituitary adenoma transsphenoidal surgeries performed from March 2008 until July 2017 at a university hospital in Indonesia were isolated. The majority (n = 512) were performed through a mononostril approach with no nasal speculum by a single experienced neurosurgeon. A PubMed literature review researching the differences
