Motor cortex rTMS reduces acute pain provoked by laser stimulation in patients with chronic neuropathic pain.

OBJECTIVE: To assess the modulation of acute provoked pain by repetitive transcranial magnetic stimulation (rTMS) of the motor cortex in patients with chronic neuropathic pain. METHODS: In 32 patients with chronic neuropathic pain affecting one upper limb, laser-evoked potentials (LEPs) (N2 and P2 components) were recorded in response to laser stimulation of the painful or painless hand, before and after active or sham rTMS applied at 10Hz over the motor cortex corresponding to the painful hand. Laser-induced pain was scored on a visual analogue scale. RESULTS: Both active and sham rTMS redu

Central pain mechanisms following combined acid and capsaicin perfusion of the human oesophagus.

Visceral afferents originating from different gut-segments converge at the spinal level. We hypothesized that chemically-induced hyperalgesia in the oesophagus could provoke widespread visceral hypersensitivity and also influence descending modulatory pain pathways. Fifteen healthy volunteers were studied at baseline, 30, 60 and 90 min after randomized perfusion of the distal oesophagus with either saline or 180 ml 0.1M HCl+2mg capsaicin. Electro-stimulation of the oesophagus, 8 cm proximal to the perfusion site, rectosigmoid electrical stimulation and rectal mechanical and heat stimulations w

Utility of multimodal evoked potentials study in neurofibromatosis type 1 of childhood.

A group of 21 children affected by neurofibromatosis type 1 has been investigated with the aim of studying multimodal (visual, brainstem auditory, and somatosensory) evoked potentials and their correlations with neurologic, electroencephalographic, and cranial magnetic resonance imaging. In the present series, cranial magnetic resonance imaging and evoked potentials were the most frequently abnormal instrumental tests. In approximately two thirds of the cases at least one of the evoked potentials (particularly visual and auditory evoked potentials) was compromised, always without clinical sign

Depression of I waves in corticospinal volleys by sevoflurane, thiopental, and propofol.

UNLABELLED: Isoflurane depresses the number and amplitude of I waves of the motor-evoked potential produced by transcranial electrical stimulation of the motor cortex and thus affects components of the corticospinal volley that are believed to arise from Laminae III and V. This study extends these observations to sevoflurane (9 patients) and the two IV anesthetics, thiopental and propofol (10 sets of observations in 10 and 6 patients, respectively). The patients' ages ranged from 10 to 17 yr. Sevoflurane was administered to achieve end-tidal concentrations of 0.5%-3%. Thiopental and propofol w

Monitoring during supratentorial surgery.

Electroneurophysiological monitoring is employed during various supratentorial surgical procedures. EEG and evoked potential monitoring are used to detect and to facilitate the timely correction of cerebral ischemia during carotid endarterectomy and aneurysm surgery. Direct cortical recording and stimulation is used to identify areas or cortex that would be likely to produce clinical deficits if removed. Electrocorticography is used to identify epileptogenic cortex intraoperatively during surgical treatment of epilepsy.

[Motor-evoked potentials following electric and magneto-electric stimulation: the value and a comparison of both methods].

Motor evoked potentials (MEP) were recorded in a total of 145 patients with supratentorial (N = 29), infratentorial (N = 25) and spinal (N = 91) lesions affecting the descending pathways. In all cases potentials were evoked by electrical, in addition in 55 of them by electromagnetic stimulation of the motor cortex. The peripheral conduction time was determined in all patients by electrical stimulation of the cervical and lumbar nerve roots, respectively. This study was designed to compare both stimulation techniques (electrical vs. electromagnetic) regarding their significance in recording of

EEG and evoked potentials in the psychoses.

EEG and evoked potential (ER) findings in schizophrenic and affective psychoses were selectively reviewed. Both groups of psychoses appear to differ from normal in a number of EEG and ER characteristics; however, diagnostic specificity was suggested by relatively few findings. More encouraging result with respect to specificity have been obtained by subtyping the psychotic groups, e.g., process versus reactive schizophrenic, unipolar versus bipolar depression. Methodologic issues and directions for future research were discussed.

Lever positioning manipulation alters real-time brain activity in patients with lumbar disc herniation: An amplitude of low-frequency fluctuation and regional homogeneity study.

INTRODUCTION: Lumbar disk herniation (LDH) is the preeminent disease of lever positioning manipulation (LPM), a complex disorder involving a … ReHo values, which were altered after LPM. The default mode network, prefrontal cortex, and primary somatosensory cortex regions could predict real-time brain activity for sensory and emotional pain management in patients with LDH after LPM.

Causal evidence for increased theta and gamma phase consistency in a parieto-frontal network during the maintenance of visual attention.

Endogenous visuo-spatial attention is under the control of a fronto-parietal network of brain regions. One key node in this network, the int … dot motion displays. Individual MRI-guided TMS pulses targeted the left IPS, while the left primary somatosensory cortex (S1) served as an active control site. In separate blocks of trials, participants were cued to attend covertly to the motion display in one hemifield (left or right) and to report brief coherent motion targets. The perceptual load of the task was ma …

4-aminopyridine improves evoked potentials and ambulation in the taiep rat: A model of hypomyelination with atrophy of basal ganglia and cerebellum.

The taiep rat is a tubulin mutant with an early hypomyelination followed by progressive demyelination of the central nervous system due to a point mutation in the Tubb4a gene. It shows clinical, radiological, and pathological signs like those of the human leukodystrophy hypomyelination with atrophy of the basal ganglia and cerebellum (H-ABC). Taiep rats had tremor, ataxia, immobility episodes, epilepsy, and paralysis; the acronym of these signs given the name to this autosomal recessive trait. The aim of this study was to analyze the characteristics of somatosensory evoked potentials (SSEPs) a

The impact of HCN4 channels on CNS brain networks as a new target in pain development.

While it is well established that the isoform 2 of the hyperpolarization-activated cyclic nucleotide-gated cation channel (HCN2) plays an im … anifested as a thermal hypersensitive phenotype. This was mediated by the key regions hypothalamus, somatosensory cortex, cerebellum and the amygdala. As consequence, HCN4-KO mice were more anxious, and their brain-wide RS functional connectivity could not be modulated by thermal nociceptive stimulation.

Neural correlates of object size and object location during grasping actions.

The visuo-motor channel hypothesis (Jeannerod, 1981) postulates that grasping movements consist of a grip and a transport component differing in their reliance on intrinsic vs. extrinsic object properties (e.g. size vs. location, respectively). While recent neuroimaging studies have revealed separate brain areas implicated in grip and transport components within the parietal lobe, less is known about the neural processing of extrinsic and intrinsic properties of objects for grasping actions. We used functional magnetic resonance imaging adaptation to examine the cortical areas involved in proc

Retardation of fetal dendritic development induced by gestational hyperglycemia is associated with brain insulin/IGF-I signals.

Hyperglycemia is an essential risk factor for mothers and fetuses in gestational diabetes. Clinical observation has indicated that the offspring of mothers with diabetes shows impaired somatosensory function and IQ. However, only a few studies have explored the effects of hyperglycemia on fetal brain development. Neurodevelopment is susceptible to environmental conditions. Thus, this study aims to investigate the effects of maternal hyperglycemia on fetal brain development and to evaluate insulin and insulin-like growth factor-I (IGF-I) signals in fetal brain under hyperglycemia or controlled
