Enhancing surveillance of sexually transmitted infections in England with gender identity and behavioural data: The GUMCAD STI Surveillance System.

BACKGROUND: There has been an increasing trend in bacterial sexually transmitted infections (STIs) in England since the early 2000s. Since 2008, surveillance of STIs in England has been conducted using the Genitourinary Medicine Clinic Activity Dataset (hereafter referred to as 'GUMCAD'), a depersonalised dataset of all attendances at all publicly-commissioned sexual health services (SHS). The aim of is this article is to describe and evaluate the piloting and rollout of an enhanced specification of GUMCAD at SHS in England. METHODS: GUMCAD was enhanced in 2019 to allow SHS to report the gend

Resilience at the Intersection of Race/Ethnicity, Sex, and Sexual Orientation among U.S. Adults.

Resilience, a key protective factor for mental health, may vary across intersecting identities, but this remains underexplored. This study applied Intersectional Multilevel Analysis of Individual Heterogeneity and Discriminatory Accuracy to examine resilience inequities at the intersection of race/ethnicity, sex, and sexual orientation among U.S. adults in a predominantly urban and suburban sample. Data came from 6543 adults in the National Wellbeing Survey 2024. Resilience was measured using 6-item Brief Resilience Scale (range 1-5), where higher scores indicate greater resilience. Participan

A case of Large Cell Neuroendocrine Carcinoma (LCNEC) treated with tarlatamab.

UNLABELLED: Transgender and gender-diverse young adults, most of whom identify as lesbian, gay, bisexual, or queer, encounter considerable health, social, and systemic inequalities, leading to higher rates of trauma exposure, mental health difficulties, and suicidal thoughts and behaviours compared to their heterosexual and cisgender peers. The complex relationship between sexual orientation, gender identity, and suicide has received considerable attention, primarily from quantitative research focused on risk narratives, rather than an individual's lived experiences or specific healthcare need

Mapping Diversity, Equity, and Inclusion Statements in Graduate Medical Education: Protocol for a Scoping Review Across Specialties.

BACKGROUND: Persistent disparities in diversity, equity, and inclusion (DEI) remain across US residency programs despite recent accreditation requirements. In this context, DEI disparities refer to inequities in the representation, recruitment, advancement, and educational experiences of individuals from underrepresented and marginalized groups. Numerous specialty-specific studies have evaluated DEI content on residency program websites, but these studies differ in their methodologies, assessed content, and reporting practices. There is currently no comprehensive synthesis of how DEI content i

Developing culturally responsive sexual health interventions: recommendations from Middle Eastern and North African diaspora youth in Ontario, Canada.

INTRODUCTION: Sex and sexual health practices of Middle Eastern and North African (MENA) youth in Canada are under-researched, even as the community grows rapidly due to high immigration rates. Through a community-based research approach, we examined access to sexual health services among MENA youth living in Ontario, Canada, and identified interventions to strengthen uptake of sexual health services. METHODS: Fifty-six MENA youth were recruited through purposive and snowball sampling. Participants completed a brief sociodemographic survey and joined subgroups across five sexual and gender id

Investigation of an outbreak of Shigella sonnei bla CTX-M-3 among men in England, 2025-26, using long- and short-read whole-genome sequencing.

Background. Recent outbreaks of gastrointestinal symptoms among gay, bisexual and other men who have sex with men (GBMSM) have been caused by multidrug-resistant (MDR) Shigella sonnei carrying bla CTX-M-27 or bla CTX-M-15. To date, the bla CTX-M-3 variant has not been associated with sexually transmitted shigellosis in the UK.Methods. Routine surveillance identified an outbreak S. sonnei among men in England in June 2025. Short-read sequencing data were analysed to identify antimicrobial resistance (AMR) determinants and to investigate the phylogenetic context of the outbreak. Long-read Oxford

Mental Health and Drug Use Patterns Among Men Who Have Sex with Men (MSM) Engaging in Chemsex in the UK.

Background: Chemsex is a subtype of sexualized drug use which is increasingly more prevalent amongst gay, bisexual, and other men who have sex with men (MSM). This use of psychoactive substances in sexual contexts poses an emerging public health concern, since it has been associated with an array of health risks, including mental health risks. Objective: As Chemsex appears to be increasing in the UK, this study aimed to explore Chemsex patterns and mental health amongst Chemsex users in the UK. Methods: Chemsex/drug use patterns and the prevalence of coexisting depression and anxiety, based on

Compliance with gender-neutral sexual behavior-based blood donor screening questions.

BACKGROUND: In September 2022, Canadian Blood Services replaced the time-based deferral for gay and bisexual men who have sex with men (gbMSM) with sexual behavior-based questions. All donors who had sex with a new partner or >1 partner in the last 3 months and anal sex were deferred, permitting gbMSM with one regular partner to donate. We aimed to measure compliance with the new criteria post-implementation. STUDY DESIGN AND METHODS: Pre-implementation 42,999; post-implementation 41,157 donors were invited to complete an anonymous online sexual risks survey. Results were weighted reflecting

Disparities in primary Cesarean birth by sexual orientation: a population-based analysis across three longitudinal cohorts.

BACKGROUND: The study aim was to quantify differences in risk for Cesarean birth vary by sexual orientation. METHODS: We pooled data from singleton, live births in the Nurses' Health Studies 2 and 3, and the Growing Up Today Study (N = 102,298 pregnancies). We used generalized estimating equations to fit logistic models for risks of primary Cesarean birth comparing sexual minority (SM) to completely heterosexual participants, controlling for year and age. SM participants were further stratified into heterosexual with prior same-sex experience (i.e., identifying as heterosexual, but with same-

Differences in Discrimination, Violence, Mental Health, and Substance Use Outcomes by Cancer History Among LGBTQ+ Individuals in North Carolina.

BACKGROUND: Health inequities among lesbian, gay, bisexual, transgender, queer, and other sexual and gender minority populations (LGBTQ+) are driven by anti-LGBTQ+ stigma and discrimination. However, there is limited research on the manifestations of stigma and discrimination among LGBTQ+ cancer survivors. METHODS: Data were collected by the North Carolina Department of Health and Human Services via the 2023 North Carolina LGBTQ+ Health Needs Assessment. Differences in outcomes (i.e., discrimination, violence, homelessness, mental health, substance use, and overdose) between LGBTQ+ individual

Suboptimal Patient-Provider Communication About Undetectable = Untransmittable and HIV Transmission Risk in Australia and the US.

The Undetectable = Untransmittable (U = U) campaign aims to raise global awareness that people living with HIV whose viral load is undetectable cannot sexually transmit HIV. Healthcare providers are uniquely positioned to disseminate the U = U message. Our study explored patient-provider communication about U = U and HIV risk from the perspectives of gay, bisexual, and other men living with HIV (MLHIV) and healthcare providers engaged in HIV treatment and prevention service delivery. We conducted 40 semi-structured interviews with key informants recruited through HIV community-based and profes

A new STI in the city: MPOX in Barcelona. First outbreak (5/2022-5/2023) and subsequent resurgence.

BACKGROUND: In May 2022, after the suspension of the mobility restrictions due to the COVID-19 pandemic, the first outbreak of MPOX virus, transmitted from human to human, was detected outside of Africa, affecting mostly sexually active men who have sex with men. Our aim is to report the first outbreak of MPOX in Barcelona city in the period from 5/2022 to 5/2023 and the subsequent surge of cases in 8/2023. METHODS: We performed a descriptive study of all notified cases in city residents, obtained through epidemiological surveys. The analyses are presented for the hospitalized cases and the k

Anal Cancer Screening in People Living With HIV: A Pilot Study of Primary HPV Screening With Triage Use of p16/Ki67 Dual Stain.

BACKGROUND AND SETTING: People living with HIV, especially gay, bisexual, and other men who have sex with men, are at increased risk of anal cancer. A recent randomized controlled trial showed treating anal high-grade squamous intraepithelial lesions (HSIL) reduces anal cancer incidence, supporting development of screening programs. Given the transition from cytological to HPV testing in cervical cancer screening, HPV testing for anal cancer is worth investigating. However, because of its low specificity, additional biomarkers like the p16/Ki67 dual stain may improve specificity. METHODS: In

Diagnoses of sexually transmissible infections in HIV-serodiscordant male couples in Australia, Brazil and Thailand.

Background In an era of biomedical prevention of HIV transmission, prevention of sexually transmissible infections (STIs) remains a priority for male HIV-serodiscordant couples. Three common STIs in populations of gay and bisexual men are chlamydia, gonorrhoea and syphilis. Methods We examined the incidence of these three infections in 343 male HIV-serodiscordant couples in Australia, Brazil and Thailand. The couples participated in a prospective, observational cohort study that included regular STI testing and behavioural surveys for HIV-negative partners (HNP). Results HIV-positive partners

Minority stress mediates associations of sexual minority state policies and tobacco use among US sexual minority young adults.

INTRODUCTION: State policies surrounding sexual minority (SM) rights are associated with tobacco use among SM individuals. Research is scant regarding the role of distinct SM policy categories on SM young adults' (SMYAs) tobacco use and mechanisms explaining these associations. METHODS: We analyzed 2023 survey data from 1100 SMYAs (ages 18-34; 14.2 % gender minority; 66.1 % bisexual+, 29.1 % monosexual; 53.2 % racial/ethnic minority) with representation across 45 US states and DC.
