Host-Salmonella interaction: human trials.

Human clinical trials, including experimental challenges of volunteers with pathogenic Salmonella enterica serovar Typhi, small phase I and II trials that monitor the immune responses to vaccines, and large-scale controlled field trials that assess vaccine efficacy under conditions of natural challenge, have helped elucidate the interactions between Salmonella typhi and human hosts.

Inefficacy of the commercial live oral Ty 21a vaccine in the prevention of typhoid fever.

The efficacy of the live oral typhoid fever vaccine Ty 21a marketed in Switzerland in 1981 was investigated. Forty-seven Swiss travellers to Third World countries contracted typhoid fever in 1982 and 1983; 20 (43%) of these persons had been vaccinated with Ty 21a. The incidence of typhoid fever in vaccinees was 1.1 per 10,000 doses of Ty 21a vaccine sold, which was similar to the incidence of 0.9 per 10,000 doses for the ineffective killed Ty 2 vaccine. Comparison of the incidence of vaccination in patients with typhoid fever in 1983 and in healthy travellers to India revealed vaccination effi

Controlled field trial of a high-dose oral killed typhoid vaccine in India.

A controlled field trial of a modified vaccine containing Salmonella typhi, TY 2 strain, in a dose of 400 x 10(9) killed organisms per capsule, was carried out in schoolchildren aged 6-17 years. The children were followed up for 6-8 months. There were 66 cases of typhoid fever in the vaccine group (7312 children) and 54 in the control group (7292 children who had received a placebo). This difference is not statistically significant. The results confirm that, as found in earlier trials, oral typhoid vaccine, in the dosages used so far, gives no protection.

[Premorbid prognosis and diagnosis of toxinfection according to the enzymatic status of blood lymphocytes and neutrophils].

On a model of an infectious process caused by intraperitoneal injection of S. paratyphi B to mice there were elaborated diagnosis of paratyphoid infection and premorbid prognosis of the disease by cytochemical indices. For the diagnosis of the infectious process the most informative of the cytochemical signs under study proved to be changes in the activity of succinic and alpha-glucerophosphoric dehydrogenases in the lymphocytes and particularly in neutrophils. Prognosis of the outcome of the infection was possible in some of the animals before the inoculation of the infection by the activity

Vi antigen from Salmonella typhosa and immunity against typhoid fever. 11. Safety and antigenicity in humans.

Safety and antigenicity of a purified preparation of Salmonella typhosa Vi antigen was evaluated in human volunteers. Dosages of Vi antigen at 25, 50, and 100 mug were less toxic than U.S. standard typhoid vaccine (lot 6A) containing 5 X 10(8) bacteria per dosage. Vi antigen in comparison with the standard typhoid vaccine induced higher hemagglutinating antibody but lower bactericidal antibody responses.

Fever: reciprocal shift in brain sodium to calcium ratio as the set-point temperature rises.

A bacterial pyrogen acts on the brain by disturbing the natural balance between two essential cations in the cerebral region involved in thermoregulation. After typhoid vaccine is administered to the unanesthetized cat, (45)Ca(2+) efflux into the third cerebral ventricle increases while (22)Na(+) is retained in hypothalamic tissue at the same time that the set-point temperature begins to rise. The subsequent rates of (22)Na(+) and (45)Ca(2+) efflux parallel the course of the bacterial fever but in a reciprocal fashion. This supports the theory that a change in the set-point temperature is dete

SEROLOGICAL RESPONSES OF MAN TO TYPHOID VACCINES.

Study of the serological responses in man to typhoid vaccines tested in several WHO-sponsored controlled field trials in British Guiana, Poland and Yugoslavia has not indicated any correlation between the level of Vi and O antibodies elicited and the effectiveness of the vaccines. Instead, the more effective vaccines have elicited significantly greater H-antibody responses; but a variety of factors suggests that it may in fact not be the antibody that confers immunity to the disease. The phagocytic test does not appear promising as a technique for determining the effectiveness of a typhoid vac
