Acute Cuff Tear Repair Trial (ACCURATE): protocol for a multicentre, randomised, placebo-controlled trial on the efficacy of arthroscopic rotator cuff repair.

INTRODUCTION: Rotator cuff tear is a very common and disabling condition that can be related to acute trauma. Rotator cuff tear surgery is a well-established form of treatment in acute rotator cuff tears. Despite its widespread use and almost a gold standard position, the efficacy of an arthroscopic rotator cuff repair is still unknown. The objective of this trial is to investigate the difference in outcome between arthroscopic rotator cuff repair and inspection of the shoulder joint defined as placebo surgery in patients 45-70 years of age with an acute rotator tear related to trauma. METHOD

Enhanced function and quality of life following 5 months of exercise therapy for patients with irreparable rotator cuff tears - an intervention study.

BACKGROUND: Rotator cuff rupture is associated with dysfunction, pain and muscular weakness related to the upper extremity. Some evidence exists to support the beneficial effect of exercises but there is lack of evidence of which exercises imply the best effect and how physiotherapy should be administered. Therefore, the purpose of this study was to examine the effect of a neuromuscular exercise program for patients with irreparable rotator cuff rupture. METHODS: Based on sample-size calculations thirty patients with chronic irreparable rotator cuff tears (of at least m. supraspinatus and m.

Peripheral nerve entrapment and injury in the upper extremity.

Peripheral nerve injury of the upper extremity commonly occurs in patients who participate in recreational (e.g., sports) and occupational activities. Nerve injury should be considered when a patient experiences pain, weakness, or paresthesias in the absence of a known bone, soft tissue, or vascular injury. The onset of symptoms may be acute or insidious. Nerve injury may mimic other common musculoskeletal disorders. For example, aching lateral elbow pain may be a symptom of lateral epicondylitis or radial tunnel syndrome; patients who have shoulder pain and weakness with overhead elevation ma

Injectable Zein-Based Photothermal Composite Hydrogel Enhances Osteoporotic Tendon-Bone Healing.

Osteoporotic rotator cuff tears represent a major clinical challenge owing to impaired tendon-bone interface (TBI) healing. To address this issue, we engineered an injectable Zein-based composite hydrogel (ZDC hydrogel) using electron-beam irradiation and, incorporated niacin-loaded mesoporous carbon nanospheres to achieve photothermal regulation and niacin release. The ZDC hydrogel exhibited favorable self-assembly, mechanical properties, photothermal performance, degradability, and biosafety. In vitro, ZDC hydrogel extracts enhanced the osteogenic differentiation of MC3T3-E1 cells and the te

Synovial Fluid Biomarkers Reflect Pre- and Postoperative Clinical Features in Rotator Cuff Tears, Independent of Tear Size.

Synovitis is a major contributor to pain and dysfunction in rotator cuff tears (RCTs), yet it remains unclear whether routine synovial fluid (SF) parameters reflect clinical features independent of tear size, operation method, and patient age. This prospective cohort study aimed to evaluate the associations between routine SF parameters and clinical features before and after rotator cuff repair. Preoperative glenohumeral SF was aspirated from 109 patients undergoing arthroscopic repair, and interleukin-6, total protein levels, and differential white blood cell counts were analyzed. Clinical fe

Non-tobacco nicotine dependence is associated with increased postoperative pain and opioid requirement following arthroscopic rotator cuff repair.

BACKGROUND: The increasing prevalence of non-tobacco nicotine dependence (NTND) related to vaping has raised concerns about its potential impact on rotator cuff disease especially given the pain after repair. This study aims to compare the risk of various surgical outcomes in patients with NTND, as compared to a control group without NTND. METHODS: A retrospective cohort study using the TriNetX database examined patients with full-thickness rotator cuff tears treated arthroscopically from 2004 to 2021. A NTND patient cohort was compared to a non-NTND cohort. Patients were 1:1 propensity match
