Visual system degeneration induced by blast overpressure.

The effect of blast overpressure on visual system pathology was studied in 14 male Sprague-Dawley rats weighing 360-432 g. Blast overpressure was simulated using a compressed-air driven shock tube, with the aim of studying a range of overpressures causing sublethal injury. Neither control (unexposed) rats nor rats exposed to 83 kiloPascals (kPa) overpressure showed evidence of visual system pathology. Neurological injury to brain visual pathways was observed in male rats surviving blast overpressure exposures of 104-110 kPa and 129-173 kPa. Optic nerve fiber degeneration was ipsilateral to the

[Sternal Fracture as a Risk Factor for Blunt Cardiac Injury].

Motor vehicle collisions are the leading cause of blunt chest trauma, including blunt cardiac injury (BCI) and sternal fracture (SF). Blunt cardiac injury occurs in about 20% of blunt chest trauma reaching to 76% in polytrauma patients. Reported SF rates range from 1.6% to 42%. This review evaluates the link between SF and BCI with focus on incidence, diagnosis and implications for patient management. A systematic search of the scientific literature in online databases over the past 15 years was conducted. The final set included 31 studies, predominantly retrospective, which were subsequently

Development of novel thoracic retractor for resuscitative thoracotomy.

BACKGROUND: Resuscitative thoracotomy (RT) is a critical intervention for patients in traumatic cardiac arrest or hemorrhagic shock, where survival is highly dependent on the time required to perform the procedure. Despite its urgency, RT is still conducted using traditional thoracic retractors originally designed for scheduled surgeries, which pose challenges in emergency settings. To address these limitations, we developed a novel thoracic retractor optimized for RT and evaluated its performance compared to a conventional model. METHODS: The novel retractor was designed with an arrow-shaped

Epitranscriptomic Analysis of the Ventral Hippocampus in a Mouse Model of Post-Traumatic Stress Disorder Following Deep Brain Stimulation Treatment of the Basolateral Amygdala.

BACKGROUND: Basolateral amygdala (BLA) deep brain stimulation (DBS) has been shown to alleviate the symptoms of post-traumatic stress disorder (PTSD), but the specific mechanisms remain incompletely understood. The hippocampus, a brain region closely connected to the amygdala, plays a key role in the pathological processes of PTSD. The N6-methyladenosine (m6A) methylation of RNAs in the hippocampus is known to play a significant role in regulating the brain's response to stress and emotional disorders. METHODS: This study aimed to comprehensively analyze the roles of transcriptome-wide m6A mo

Complex Limb Injuries: Limb Salvage Versus Amputation-A Mini Review and Meta-Analysis.

Introduction: The management of complex limb injuries can be very challenging, and it demands a multidisciplinary approach to treatment. Amputation and limb reconstruction are the two options that clinicians must choose. This study aims to comprehensively synthesize existing tools and resources from the literature that can assist clinicians in the decision-making process. Evaluation: The initial resuscitation and the prehospital care are the first important steps in the management of these injuries, while the immediate transfer to trauma centers is recommended for complex cases. After the stab

Single-Nucleus Transcriptome Profiling from the Hippocampus of a PTSD Mouse Model and CBD-Treated Cohorts.

Post-traumatic stress disorder (PTSD) is the most common psychiatric disorder after a catastrophic event; however, the efficacious treatment options remain insufficient. Increasing evidence suggests that cannabidiol (CBD) exhibits optimal therapeutic effects for treating PTSD. To elucidate the cell-type-specific transcriptomic pathology of PTSD and the mechanisms of CBD against this disease, we conducted single-nucleus RNA sequencing (snRNA-seq) in the hippocampus of PTSD-modeled mice and CBD-treated cohorts. We constructed a mouse model by adding electric foot shocks following exposure to sin

A better understanding of why murine models of trauma do not recapitulate the human syndrome.

OBJECTIVE: Genomic analyses from blood leukocytes have concluded that mouse injury poorly reflects human trauma at the leukocyte transcriptome. Concerns have focused on the modest severity of murine injury models, differences in murine compared with human age, dissimilar circulating leukocyte populations between species, and whether similar signaling pathways are involved. We sought to examine whether the transcriptomic response to severe trauma in mice could be explained by these extrinsic factors, by utilizing an increasing severity of murine trauma and shock in young and aged mice over time

Heat acclimation and cross-tolerance against novel stressors: genomic-physiological linkage.

Heat acclimation (AC) is a "within lifetime" reversible phenotypic adaptation, enhancing thermotolerance and heat endurance via a transition to "efficient" cellular performance when acclimatory homeostasis is reached. An inseparable outcome of AC is the development of cross-tolerance (C-T) against novel stressors. This chapter focuses on central plasticity and the molecular-physiological linkage of acclimatory and C-T responses. A drop in temperature thresholds (T-Tsh) for activation of heat-dissipation mechanisms and an elevated T-Tsh for thermal injury development imply autonomic nervous sys

Indications for mechanical ventilation in adults with acute respiratory failure.

Increased understanding of the mechanisms and effects of acute respiratory failure has not been accompanied by more precise criteria by which the clinician can determine when intubation should be carried out and invasive positive-pressure ventilation (IPPV) instituted in a given patient. The indications traditionally offered in reviews and textbooks have tended to be either so broad as not to be very helpful in an individual case, or of questionable clinical relevance and too cumbersome for practical use. This review updates the indications for IPPV in adult patients with acute respiratory fai

Evolving brain lesions in the first 12 hours after head injury: analysis of 37 comatose patients.

From January 1, 1990, to April 30, 1994, 412 patients were admitted to our intensive care unit in coma after head injuries. Our study group consisted of 37 patients who were retrospectively identified as harboring lesions or developing new lesions within a 12-hour period from the time of admission. We defined the evolution of a lesion as an increase or decrease in the size of an already present hematoma or as the appearance of a totally new lesion. There were 25 male and 12 female patients (mean age, 34.9 yr), and the cause of trauma was road traffic accidents in 32 patients. Nine patients pre

[Pharmacologic profile of protirelin tartrate].

Pharmacological interest in the tripeptide thyrotropin-releasing hormone (TRH) is due to the multiple effects it produces. In fact, apart from taking part in regulating the activity of the hypothalamo-pituitary-thyroid axis, TRH produces various neuropharmacological effects which indicate a biological role that is probably more important than that of a releasing hormone. Trials performed in animals have shown, for example, the dose-dependent capacity of TRH to induce analgesia, probably by interacting with the opioid peptide system. Motor activity is affected by TRH. In fact this tripeptide el

Is it all about the right? Echocardiographic metrics of right-sided venous congestion are frequent in trauma patients with hemorrhagic shock.

BACKGROUND: Detecting right-sided venous congestion (RSVC) is more difficult than left-sided venous congestion (LSVC). Point-of-care ultrasound (POCUS) can assess venous Doppler flow patterns in the hepatic (HV), portal (PV), and renal (RV) veins. RSVC can predict organ injury from volume overload in critically ill patients. The prevalence and feasibility of measuring RSVC are unknown in traumatic hemorrhagic shock (THS). METHODS: This is a retrospective review of quality improvement data regarding feasibility and utility of assessing RSVC in THS. All trauma patients who were transfused, had

Spontaneous L4/L5 Dislocation Associated With Diffuse Idiopathic Skeletal Hyperostosis in a Young Patient With Severe Obesity: A Case Report.

Diffuse idiopathic skeletal hyperostosis (DISH) is characterized by extensive ossification of the spinal ligaments and entheses, resulting in a rigid spine that behaves biomechanically like a long bone. Although spinal fractures and dislocations after minor trauma are well recognized in ankylosed spines, non-traumatic spontaneous instability is rare. We report a rare case of spontaneous L4/L5 dislocation in a young patient with severe obesity and DISH. A 40-year-old man presented with progressive low back pain, bilateral lower extremity weakness, and bladder-bowel dysfunction. He had initially

Octreotide Attenuates Anxiety-Like Behaviors and Cognitive Deficits in a Rat Model of Post-traumatic Stress Disorder.

Post-traumatic stress disorder (PTSD) is a long-lasting mental health condition characterized by persistent intrusive recollections, avoidance behaviors, mood and cognitive alterations, heightened arousal, and significant cognitive deficits, often accompanied by hypothalamic-pituitary-adrenal (HPA) axis dysregulation and neuroinflammation. This study evaluated the therapeutic potential of octreotide (OCT), a synthetic somatostatin analog, in a rat model of PTSD induced by foot-shock contextual fear conditioning. Male Wistar rats (n = 40) were randomly assigned to four groups (n = 10/group): Sa

HMGB-1 as a predictor of major bleeding requiring activation of a massive transfusion protocol in severe trauma.

Massive bleeding causes approximately 50% of deaths in patients with major trauma. Most patients die within 6 h of injury, which is preventable in at least 10% of cases. For these patients, early activation of the massive transfusion protocol (MTP) is a critical survival factor. With severe trauma, high-mobility group box 1 (HMGB-1, i.e., amphoterin) is released into the blood, and its levels correlate with the development of a systemic inflammatory response, traumatic coagulopathy, and fibrinolysis. Previous work has shown that higher levels of HMGB-1 are associated with a higher use of red b

The early change in pH values after out-of-hospital cardiac arrest is not associated with neurological outcome at hospital discharge.

BACKGROUND: The association between pH values and outcome for patients after out-of-hospital cardiac arrest (OHCA) was not fully elucidated; besides, the relationship of change in pH values and neurological outcome was unknown. The aim was to explore the association of pH values as well as change in pH values and neurological outcome for OHCA cardiac patients. METHODS: The adult patients with non-traumatic out-of-hospital cardiac arrest, shock-refractory ventricular fibrillation or pulseless ventricular tachycardia, and at least two arterial blood gases analysis recorded after admission were
