Group B streptococcal disease in nonpregnant adults: incidence, clinical characteristics, and outcome.

A retrospective review of 150 cases of group B streptococcal disease in nonpregnant adults over an 8-year period was performed in a single tertiary-care teaching hospital to determine the incidence, clinical spectrum, and outcome of the disease. Incidence increased from 0.53 cases per 1,000 admissions in the 1993-1994 period to 0.96 cases per 1,000 admissions in 1999-2000 (P=0.013, chi-square test for trend). Bacteremia also increased from 0.15 to 0.42 cases per 1,000 admissions over the same period of time (P=0.005, chi-square test for trend). The mean age of the patients was 61.4 years, and

Perinatal antibiotic usage and changes in colonization and resistance rates of group B streptococcus and other pathogens.

OBJECTIVE: To quantify current antibiotic usage during the perinatal period and impact on vaginal-rectal colonizing organism resistance rates. METHODS: Swabs were obtained for culture of group B streptococcus and other bacteria from a cohort of 1207 pregnant women in Calgary, Alberta, at 36 weeks' gestation. Those women who received antibiotics during labor or after pregnancy and a 10% subset who received no antibiotics had repeat cultures at 6 weeks postpartum. Cultured organisms were tested for sensitivity to several antibiotics. RESULTS: Group B streptococcus was identified in 235 women (

Implementation of guidelines for preventing early onset group B streptococcal infection.

Early onset Group B Streptococcus (EOGBS) disease, defined by an onset within the first 72 hours of life, occurs in 1.3 to 3.7 per 1,000 live births. The authors sought to determine the impact of the new CDC/AAP/ACOG guidelines on the prepartum screening practice, intrapartum management, incidence of EOGBS infection, and evaluation of neonates born to GBS colonized women at University Macdonald Women's Hospital (Cleveland, OH). A retrospective analysis by chart review was conducted from January 1, 1995 to December 31, 1997 of women identified as GBS colonized during prenatal screening. These w

Nitric oxide, prostaglandins, and impaired cerebral blood flow autoregulation in group B streptococcal neonatal meningitis.

Impaired autoregulation of cerebral blood flow (CBF) contributes to CNS damage during neonatal meningitis. We tested (i) the hypothesis that cerebrovascular autoregulation is impaired during early onset group B streptococcal (GBS) meningitis, (ii) whether this impairment is regulated by vasoactive mediators such as prostaglandins and (or) nitric oxide (NO), and (iii) whether this impairment is preventable by specific and (or) nonspecific inhibitors: dexamethasone, ibuprofen, and Nomega-nitro-L-arginine, a NO inhibitor. Sterile saline or 10(9) colony-forming units (cfu) of heat-killed GBS was i

Group B streptococcal disease in the era of intrapartum antibiotic prophylaxis.

BACKGROUND: Group B streptococcal infections are a leading cause of neonatal mortality, and they also affect pregnant women and the elderly. Many cases of the disease in newborns can be prevented by the administration of prophylactic intrapartum antibiotics. In the 1990s, prevention efforts increased. In 1996, consensus guidelines recommended use of either a risk-based or a screening-based approach to identify candidates for intrapartum antibiotics. To assess the effects of the preventive efforts, we analyzed trends in the incidence of group B streptococcal disease from 1993 to 1998. METHODS:

Characterization of cpsF and its product CMP-N-acetylneuraminic acid synthetase, a group B streptococcal enzyme that can function in K1 capsular polysaccharide biosynthesis in Escherichia coli.

Group B Streptococcus (GBS) is the foremost cause of neonatal sepsis and meningitis in the United States. A major virulence factor for GBS is its capsular polysaccharide, a high molecular weight polymer of branched oligosaccharide subunits. N-acetylneuraminic acid (Neu5Ac or sialic acid), at the end of the polysaccharide side chains, is critical to the virulence function of the capsular polysaccharide. Neu5Ac must be activated by CMP-Neu5Ac synthetase before it is incorporated into the polymer. We showed previously that a transposon mutant of a serotype III GBS strain which had no detectable c

[Necrotizing fasciitis of soft tissues of the face and neck].

Three cases of cervical necrotizing fasciitis are presented. One case was odontogenic in origin and two were due to pharyngeal infectious. Bacteria cultured represented multiple bacterial species. Airway control was necessary early, as was wide surgical exploration of the fascial spaces of the neck with re-exploration as necessary. Intensive medical support was crucial to prevent or treat complications. Cervical necrotizing fasciitis has an overall mortality rate of 30 per cent at the University of Bern.

The changing spectrum of neonatal meningitis over a fifteen-year period.

One hundred seventy-seven cases of neonatal meningitis treated at the University of Texas Medical Branch at Galveston over a 15-year period (1974-1988) were reviewed. Over this period, the incidence of bacterial meningitis decreased, the incidence of aseptic meningitis remained stable, and the diagnosis of enteroviral meningitis increased in frequency. During 1984-1988, enterovirus was the most common cause of meningitis in neonates older than seven days and accounted for one third of all cases of neonatal meningitis. Half of all neonates with bacterial meningitis had negative blood cultures.

[Streptococcus group B septicemia. Analysis of 18 cases in adults and 10 in newborn infants].

From 1976 to 1981, 28 episodes of group B streptococcus (GBS) septicemia were identified in our hospital (CHUV, University Hospital Lausanne), 18 in 17 adults and 10 in newborns. The latter had acute respiratory distress syndrome (8 cases) or meningitis (2 cases). In adults the skin was the main source of infection (6 diabetic foot, 4 acute cellulitis complicating chronic skin diseases, 2 infections secondary to diagnostic procedures (capillary and ascitic taps) and 1 meningitis secondary to neurosurgery). The other sources of infection were 1 pharyngitis, 1 pneumonia and 1 pyelonephritis. Ele

Action of EDTA-Tris and antimicrobial agent combinations on selected pathogenic bacteria.

The concentration of EDTA-Tris (3.22 mM EDTA and 0.05 M Tris) used as a lavage to treat otitis externa, cystitis or other persistent infections in dogs and cats, was found to prevent the growth of Pseudomonas aeruginosa, Staphylococcus aureus and beta streptococci when present in growth media. Pseudomonas aeruginosa was rapidly lysed in this solution. Escherichia coli and Proteus vulgaris grew in the presence of EDTA-Tris, but to a lesser extent than the controls without these compounds. Minimal inhibitory concentrations (MIC) for 8 antimicrobial agents with and without EDTA-Tris were determin

Multicenter Diagnostic Accuracy Evaluation of the Luminex Aries Real-Time PCR Assay for Group B Streptococcus Detection in Lim Broth-Enriched Samples.

The vertical transmission of group B Streptococcus (GBS) strains causing neonatal sepsis is one of the leading reasons for neonatal mortality worldwide. The gold standard for GBS detection is enriched culture with or without the aid of chromogenic agars. Given the high risk for morbidity and mortality in this population, high assay sensitivity is required to prevent the personal and economic costs of GBS disease. Nucleic acid amplification tests (NAATs) allow for objective determination of GBS colonization with a sensitivity and a specificity higher than those of traditional culture methods. I

Group B streptococcus late-onset disease: 2003-2010.

BACKGROUND: There is insufficient population-based data on group B streptococcus (GBS) late-onset disease (LOD). Risk factors and routes of GBS transmission are poorly understood. METHODS: A prospective, cohort study was conducted to collect incidence data on LOD and evaluate GBS infections over an 8-year period (2003-2010). Starting from January 2007, maternal rectovaginal and breast milk cultures were routinely collected on confirmation of the LOD diagnosis to assess maternal GBS culture status. RESULTS: The incidence rate of LOD was 0.32 per 1000 live births (1.4 and 0.24 per 1000 live bi

Inappropriate use of vancomycin for preventing perinatal group B streptococcal (GBS) disease in laboring patients.

OBJECTIVE: The 2002 CDC guidelines for the prevention of perinatal group B streptococcus (GBS) stipulate that vancomycin is reserved for penicillin-allergic women at high risk for beta-lactam anaphylaxis with resistance to clindamycin or erythromycin. Our objective was to evaluate practitioner adherence to these guidelines. METHODS: This is a retrospective chart review of patients admitted to labor and delivery who received vancomycin for GBS prophylaxis from January 1st, 2005 to June 1st, 2007. Identification and documentation of allergic reactions to beta lactams and performance of GBS sens

[New trends in research on group B streptococci (GBS) infections and sexual transmitted diseases].

The adherence to and invasion of A549 cells by serotype VIII GBS (group B streptococci) were significantly greater than those of serotype III GBS for both the invasive strain and vaginal isolates. Serotype III strains of GBS stimulated significantly greater tumor necrosis factor alpha (TNF-alpha), interleukin-10 (IL-10), and interleukin-12 (IL-12) production than did serotype VIII strains. Interleukin-4 (IL-4) and interleukin-8 (IL-8) production in response to serotype VIII was significantly higher than in response to serotype III. Large, identical, tandem repeating units in the C protein alpha antigen gene, bca, of group B streptococci.

Group B Streptococcus (GBS) is the leading cause of neonatal sepsis and meningitis in the United States. The surface-associated C protein alpha antigen of GBS is thought to have a role in both virulence and immunity. We previously cloned the C protein alpha antigen structural gene (named bca for group B, C protein, alpha) into Escherichia coli. Western blots of both the native alpha antigen and the cloned gene product demonstrate a regularly laddered pattern of heterogeneous polypeptides. The nucleotide sequence of the bca locus reveals an open reading frame of 3060 nucleotides encoding a prec

Population-based risk factors for neonatal group B streptococcal disease: results of a cohort study in metropolitan Atlanta.

To determine risk factors for neonatal group B streptococcal (GBS) disease, a cohort study was conducted in Atlanta of infants with invasive GBS disease during 1982 and 1983. Laboratory review detected 71 infants with early-onset disease (1.09 cases/1000 live births) and 37 infants with late-onset disease (0.57 cases/1000 live births). Compared with the 64,858 births in Atlanta in the same period, infants with early-onset GBS disease were more often black, less than 2500 g, and born to teenage mothers. A history of miscarriage increased a woman's risk of delivering an infant with early-onset d
