Carbonic anhydrase inhibitors. Inhibition of the beta-class enzyme from the yeast Saccharomyces cerevisiae with anions.

The protein encoded by the Nce103 gene of Saccharomyces cerevisiae, a beta-carbonic anhydrase (CA, EC 4.2.1.1) designated as scCA, has been cloned, purified, characterized kinetically, and investigated for its inhibition with a series simple, inorganic anions such as halogenides, pseudohalogenides, bicarbonate, carbonate, nitrate, nitrite, hydrogen sulfide, bisulfite, perchlorate, sulfate, and some of its isosteric species. The enzyme showed high CO(2) hydrase activity, with a k(cat) of 9.4x10(5) s(-1) and k(cat)/K(m) of 9.8x10(7) M(-1) s(-1). scCA was weakly inhibited by metal poisons (cyanid

Optic nerve sheath diameter correlates with the presence and severity of acute mountain sickness: evidence for increased intracranial pressure.

Increased intracranial pressure is suspected in the pathogenesis of acute mountain sickness (AMS), but no studies have correlated it with the presence or severity of AMS. We sought to determine whether increased optic nerve sheath diameter, a surrogate measure of intracranial pressure, is associated with the presence and severity of AMS. We performed a cross-sectional study of travelers ascending through Pheriche, Nepal (4,240 m), from March 3 to May 14, 2006. AMS was assessed using the Lake Louise score. Optic nerve sheath diameter was measured by ultrasound. Ultrasound exams were performed a

Scleritis as the initial clinical manifestation of graft-versus-host disease after allogenic bone marrow transplantation.

PURPOSE: To describe a case of scleritis with choroidal detachment as the initial clinical manifestation of graft-versus-host disease after allogenic bone marrow transplantation. DESIGN: Interventional case report. METHODS: Review of clinical records. RESULTS: A 50-year-old man developed symptoms of mild dry eye syndrome 20 days after receiving a one-antigen-mismatched allogenic bone marrow transplant. Five days later, he developed orbital pain, moderate visual loss, and a sterile orbital abscess on the left side. The orbital inflammatory collection was drained, and symptoms improved. On da

Effect of hydrochloric acid, pepsin, or taurocholate on bioelectric properties of gastric squamous mucosa in horses.

OBJECTIVE: To determine the effect of pH with or without pepsin or taurocholic acid on the bioelectric properties of gastric squamous mucosa in horses. SAMPLE POPULATION: Gastric tissues obtained from 16 adult horses that did not have evidence of gastric disease. PROCEDURE: Bioelectric properties of squamous mucosa were determined, using modified Ussing chambers. Tissues then were exposed to mucosal pepsin (1 mg/ml) or taurocholic acid (2.5 mM) under neutral (pH 74) or acidic (pH 1.7) conditions. RESULTS: Exposure of mucosal sheets to an acidic pH resulted in an immediate and sustained decr

Visual field prognosis in early glaucoma. A long-term clinical follow-up.

Fifty-four eyes of 45 patients with ocular hypertension developed early glaucomatous optic disc or visual field changes after a mean follow-up of 5.1 years without therapy. These changes initiated antiglaucomatous treatment, and the eyes have been followed further on therapy for 2-14 years (mean 8.0 years) in a retrospective study. The mean intraocular pressure (IOP) was lowered from 28.9 to 19.3 mmHg. Half of the eyes were still without field defects at the end of the study, whereas 7 eyes developed extensive field damage noticeable to the patients. The eyes with deteriorating visual fields c

Early diagnosis and treatment of acute heart failure in prehospital and emergency settings. Part 1 of the International Expert Opinion Series on acute heart failure management.

Acute heart failure (AHF) is diagnosed in about 0.5% of all patients seen by emergency medical systems (EMS) and represents about 1% of emer … patients with poor diuretic response for dose augmentation or drug escalation with the addition of acetazolamide or thiazides. For selected patients, vasodilators (especially for acute cardiogenic pulmonary edema phenotype) or inotropes/vasopressors (for those with cardiogenic shock) can be needed. Oxygen therapy should be provided to patients with air-room Sp …

How to reduce the complication rate of multiple burr holes surgery in moyamoya angiopathy.

PURPOSE: This study is aimed at analyzing clinical outcome, absence of stroke recurrence, revascularization, and complications and long-term … sonance angiography (MRA) with perfusion or single-photon emission computed tomography (SPECT) with acetazolamide. RESULTS: In total, 21 consecutive patients (6 children and 15 adults) were included with a mean age of 7.4 years in the pediatric group and 36.9 years in the adult group. Initial presentation was permanent ischemic stroke in 15 cases, transient is …

Renal Mechanisms for Hypercalciuria Induced by Metabolic Acidosis.

BACKGROUND: In metabolic acidosis, a negative calcium balance is induced by decreased renal tubular calcium reabsorption. This occurs indepe … ansporters TRPV5 and 28K calcium-binding protein in the DCT-CNT were also downregulated by NH4Cl or acetazolamide administration in mice. KEY MESSAGES: Both paracellular and transcellular calcium transport pathways in the kidney are regulated by metabolic acidosis and lead to renal calcium wasting. In the PT, claudin-2 is downregulated by acidic pH. In the TAL …

Ophthalmic manifestations of Cryptococcus gattii species complex: a case series and review of the literature.

AIM: To report 4 cases of Cryptococcus gattii (C. gattii) species complex infection with diverse ophthalmic manifestations, and to review th … n with serial lumbar punctures. CSF diversion procedures should be considered for refractory cases. Acetazolamide and mannitol are associated with high complication rates, and adjuvant corticosteroids have demonstrated higher mortality rates; these treatments should be avoided. CONCLUSION: Permanent visual loss represents a devastating yet potentially preventa …
