Alcohol use and adolescent pregnancy.

Reports indicate that 33.4% of adolescents engage in heavy episodic alcohol consumption, and that 34.8% of adolescents are sexually active by the age of 15 without using any form of contraception. Combined, these activities can lead to adolescent pregnancy complicated by fetal alcohol syndrome (FAS), a rare disorder that is totally preventable by abstinence from alcohol during pregnancy. This article offers some primary prevention strategies that nurses can adopt in working with adolescents and communities to help prevent FAS.

Binge-like alcohol exposure of neonatal rats via intragastric intubation induces both Purkinje cell loss and cortical astrogliosis.

Binge-like alcohol exposure in neonatal rats on postnatal days 4 to 9 via artificial rearing results in a well-documented transient astrogliosis in the cerebral cortex. A recent study, which replicated the astrogliosis using artificial rearing, found that alcohol administered via daily exposure cycles in a vapor inhalation chamber on postnatal days 4 to 9 failed to elicit the effect, thus suggesting that the gliosis was an interactive effect of the artificial rearing administration and not specific to alcohol. The present study evaluated the effects in an intragastric intubation model that rep

Heeding the alcoholic beverage warning label during pregnancy: multiparae versus nulliparae.

OBJECTIVE: We compared the impact of the Federal Alcoholic Beverage Warning Label on multiparae (women with at least one previous live birth) and nulliparae (women with no previous live births). The label, implemented on November 18, 1989, urges women not to drink during pregnancy because of the risk of birth defects. If multiparae drank during prior pregnancies, delivering apparently normal babies, we hypothesized that the warning might be less salient for them. METHOD: We studied 17,456 inner city black gravidas seen between September 1986 and September 1993 at one antenatal clinic. Time se

Alterations of neocortical neuronal responses to acetylcholine and GABA in rats born to alcohol-dependent mothers.

Alcohol is known to be a CNS teratogenic factor interfering with neuronal and synaptic maturation. The purpose of this microiontophoretic study was to explore GABAergic and cholinergic central mechanisms in adult rats exposed to alcohol in the third phase of prenatal life (ADM), when their mothers were subjected to alcohol physical dependence induction (9.6 g/kg/day). Responses to acetylcholine and GABA were recorded in frontal and somatosensory cortical neurons. Adult rats, whose mothers had been administered placebo with identical procedures, were used as a control (C). Cholinergic responses

Effects of moderate alcohol consumption during pregnancy on child development at 18 and 42 months.

The aim of this study was to estimate the association between moderate alcohol consumption in pregnancy and child development to the age of 3.5 years. Furthermore, the aim was to compare development indices at 18 and 42 months of age. This study is an extension of the Danish participation in the EuroMac study. In a two-stage sampling, pregnant women in a well-defined region (Odense, Denmark) were recruited to a follow-up study according to their reported drinking habits in the first trimester of pregnancy. All among 2880 pregnant women who reported an alcohol consumption early in pregnancy (12

Low to moderate maternal alcohol use before and during pregnancy, and neurobehavioural outcome in the newborn infant.

Of 2002 randomly selected pregnant women recruited prospectively over a three-year period for an extensive questionnaire survey, a stratified subsample of 665 mothers was selected for mother-infant follow-up on the basis of pre-pregnancy alcohol intake. Infant outcome was assessed by detailed clinical examination and application of a modified Einstein Neonatal Behavioural Assessment Schedule (ENBAS) performed at 24 to 72 hours of age. Of the infant responses to 25 ENBAS items, only tonus showed a small but significant relationship to pre-pregnancy maternal alcohol intake. The authors conclude

Prenatal alcohol exposure and offspring growth at 18 months of age: the predictive validity of two measures of drinking.

This is an analysis of the effect of prenatal alcohol exposure on growth of the offspring at 18 months of age. In this prospective study, a cohort of women was interviewed at the end of each trimester of pregnancy, at delivery, and at 8 and 18 months. Offspring were examined at each follow-up point. Two drinking scales, average daily volume (ADV) and frequent heavy drinking (FHD), were used to explore the effect of different patterns of drinking. We found significant relationships between both prenatal FHD and ADV and offspring growth at 18 months. A significant and inverse relationship was fo

Feasibility and Acceptability of Maternal Choline Supplementation in Heavy Drinking Pregnant Women: A Randomized, Double-Blind, Placebo-Controlled Clinical Trial.

BACKGROUND: Choline, an essential nutrient, serves as a methyl-group donor for DNA methylation and is a constituent of the neurotransmitter acetylcholine and a precursor to major components of cell membranes. Findings from animal studies suggest that choline supplementation during pregnancy can mitigate adverse effects of prenatal alcohol exposure on growth and neurocognitive function. We conducted a randomized, double-blind exploratory trial to examine feasibility and acceptability of a choline supplementation intervention during pregnancy. METHODS: Seventy heavy drinkers, recruited in mid-p

Maternal Alcohol Use During Pregnancy and Associated Morbidities in Very Low Birth Weight Newborns.

BACKGROUND: We hypothesized that maternal alcohol use occurs in pregnancies that end prematurely and that in utero alcohol exposure is associated with an increased risk of morbidities of premature newborns. METHODS: In an observational study of mothers who delivered very low birth weight newborns (VLBW) ≤1,500 g, maternal alcohol use was determined via a standardized administered questionnaire. We compared the effect of maternal drinking on the odds of developing late-onset sepsis (LOS), bronchopulmonary dysplasia (BPD), death, BPD or death, days on oxygen or any morbidity (either LOS, BPD or

Inconsistent report of pre-pregnancy-recognition alcohol use by Latinas.

OBJECTIVE: This study compared pregnant Latinas' report of alcohol use for the 3-month period before pregnancy recognition with two different methods of data collection, in an attempt to identify opportunities for improved screening. METHODS: Data were collected for 53 pregnant Latinas who have ever drunk alcohol and who were receiving services at Women, Infants, and Children (WIC) clinics in San Diego, California. This study compared alcohol use reported during in-depth research interviews conducted in homes, to responses to an alcohol screening question administered by WIC staff. RESULTS:

Use of record linkage to examine alcohol use in pregnancy.

BACKGROUND: To date, no population-level data have been published examining the obstetric and neonatal outcomes for women with an alcohol-related hospital admission during pregnancy compared with the general obstetric population. This information is critical to planning and implementing appropriate services. METHODS: Antenatal and delivery admissions to New South Wales (NSW) hospitals from the NSW Inpatient Statistics Collection were linked to birth information from the NSW Midwives Data Collection over a 5-year period (1998-2002). Birth admissions were flagged as positive for maternal alcoho

A register-based study of long-term health and social care costs among children with prenatal alcohol exposure.

Prenatal alcohol exposure (PAE) associated with fetal alcohol spectrum disorders (FASD) often remain underdiagnosed. They globally cause a wide range of health and social problems leading to high costs. To outline cumulative health and social care costs in children related with PAE with and without diagnosed FASD, we followed 427 children with PAE until the age of 20 years, and 1795 controls born 1992-2001 until the year 2016. All hospital care and out-of-home care episodes, including placements in foster or residential care, were analyzed, and their costs estimated. Age-dependent patterns of …

Maternal circulating miRNAs that predict infant FASD outcomes influence placental maturation.

Prenatal alcohol exposure (PAE), like other pregnancy complications, can result in placental insufficiency and fetal growth restriction, although the linking causal mechanisms are unclear. We previously identified 11 gestationally elevated maternal circulating miRNAs (HEamiRNAs) that predicted infant growth deficits following PAE. Here, we investigated whether these HEamiRNAs contribute to the pathology of PAE, by inhibiting trophoblast epithelial-mesenchymal transition (EMT), a pathway critical for placental development. We now report for the first time that PAE inhibits expression of placent

Pre-administration of G9a/GLP inhibitor during synaptogenesis prevents postnatal ethanol-induced LTP deficits and neurobehavioral abnormalities in adult mice.

It has been widely accepted that deficits in neuronal plasticity underlie the cognitive abnormalities observed in fetal alcohol spectrum disorder (FASD). Exposure of rodents to acute ethanol on postnatal day 7 (P7), which is equivalent to the third trimester of fetal development in human, induces long-term potentiation (LTP) and memory deficits in adult animals. However, the molecular mechanisms underlying these deficits are not well understood. Recently, we found that histone H3 dimethylation (H3K9me2), which is mediated by G9a (lysine dimethyltransferase), is responsible for the neurodegener

Mutations in the Bone Morphogenetic Protein signaling pathway sensitize zebrafish and humans to ethanol-induced jaw malformations.

UNLABELLED: Fetal Alcohol Spectrum Disorders (FASD) describe ethanol-induced developmental defects including craniofacial malformations. While ethanol-sensitive genetic mutations contribute to facial malformations, the impacted cellular mechanisms remain unknown. Bmp signaling is a key regulator of epithelial morphogenesis driving facial development, providing a possible ethanol-sensitive mechanism. We found that zebrafish mutants for Bmp signaling components are ethanol-sensitive and affect anterior pharyngeal endoderm shape and gene expression, indicating ethanol-induced malformations of the…

Quantification of Ethanol Levels in Zebrafish Embryos Using Head Space Gas Chromatography.

Fetal Alcohol Spectrum Disorders (FASD) describe a highly variable continuum of ethanol-induced developmental defects, including facial dysmorphologies and neurological impairments. With a complex pathology, FASD affects approximately 1 in 100 children born in the United States each year. Due to the highly variable nature of FASD, animal models have proven critical in our current mechanistic understanding of ethanol-induced development defects. An increasing number of laboratories has focused on using zebrafish to examine ethanol-induced developmental defects. Zebrafish produce large numbers o…
