A 5-year-old boy had initial symptoms of behavioral changes, nausea, vomiting, headache, weight loss, and progressive vision failure. Brain MRI revealed abnormal signal intensity in both optic nerves, the optic chiasm, the right medial temporal lobe, and tissues surrounding the right supraclinoid internal carotid artery with associated leptomeningeal and spinal cord enhancement. After nondiagnostic dural and spinal arachnoid biopsies, a temporal lobe biopsy was diagnostic for a rare malignant peripheral nerve sheath tumor.

A meningoencephalocele is a herniation of meninges and brain out of the cranial fossa through a bony defect. Cerebrospinal fluid leakage may occur due to a defect in the wall of the meningoencephalocele. The defect may be traumatic, iatrogenic, or may appear spontaneously. In this report, the authors present an unrecognized transethmoidal meningoencephalocele that resulted in a cerebrospinal fluid leak during dacryocystorhinostomy.

A case is reported of a patient with bilateral central retinal artery occlusion secondary to giant cell arteritis. After treatment the vision recovered in one eye but remained poor in the other eye. Treatments employed are discussed with particular attention drawn to differences in the treatment of each eye and suggestions are made for treatment of similar cases.

A 30-year-old woman had bilateral anterior ischemic optic neuropathy after undergoing large-volume liposuction. Visual function remained stable over a four-month follow-up, with decreased visual acuity and marked constriction of the visual fields. To our knowledge, this is the second reported case of ischemic optic neuropathy in this setting.

The authors assess the efficacy of preoperative decrease of intraocular pressure by diacarb in 130 patients (134 eyes) with senile and complicated cataracts and cataracts combined with primary open-angle glaucoma. Diacarb administered after the proposed protocols helps notably decrease the initial intraocular pressure, thus preventing intraoperative complications in the course of extracapsular cataract extraction with implantation of intraocular lenses.

PURPOSE: The purpose of this study was to evaluate the clinical relevance of regional cerebral blood flow by single photon emission computerized imaging (SPECT) in patients undergoing carotid endarterectomy (CEA).
METHOD: Eighty-four patients were included in this study; 74 were undergoing CEA. All 74 operative cases had SPECT scans before and after surgery. The first 30 patients undergoing CEA also had computed tomography scans for comparison. Findings were correlated during surgery with carotid stump pressures.
