BACKGROUND AND OBJECTIVE: Treating geriatric osteoporotic distal femur fractures has always presented challenges, but developing biodegradable materials has brought new opportunities for therapeutic intervention. Despite this progress, there currently needs to be more evidence-based biomechanical guidelines for using dual plate fixation and biodegradable materials in treating osteoporotic comminuted distal femoral fractures. In this study, finite element analysis was conducted to evaluate the mechanical effectiveness of different implant materials (titanium alloys, biodegradable materials, and

BACKGROUND: Adult distal humeral fractures (DHF) comprise 2%-5% of all fractures and 30% of all elbow fractures. Treatment of DHF may be technically demanding due to fracture complexity and proximity of neurovascular structures. Open reduction and internal fixation (ORIF) are often the treatment of choice, but arthroplasty is considered in case of severe comminution or in elderly patients with poor bone quality. Ulnar nerve affection following surgical treatment of distal humerus fractures is a well-recognized complication.
AIM: To report the risk of ulnar nerve affection after surgery for ac

Lumbar spine pain accounts for 5 to 8% of athletic injuries. Although back pain is not the most common injury, it is one of the most challenging for the sports physician to diagnose and treat. Factors predisposing the young athlete to back injury include the growth spurt, abrupt increases in training intensity or frequency, improper technique, unsuitable sports equipment, and leg-length inequality. Poor strength of the back extensor and abdominal musculature, and inflexibility of the lumbar spine, hamstrings and hip flexor muscles may contribute to chronic low back pain. Excessive lifting and

BACKGROUND: The possible causal relationship between traumatic brain injury and subsequent meningioma development remains controversial. Although historical and anecdotal reports have suggested a post-traumatic origin of meningiomas, robust epidemiological evidence is lacking, and the distinction between causality and coincidence remains challenging.
CASE DESCRIPTION: We report the case of a 78-year-old man who sustained a severe frontal cranial trauma with a comminuted fracture and retained foreign body 40 years earlier, treated surgically by craniectomy. Decades later, the patient presented

Olecranon nonunion is an uncommon complication of proximal ulna fractures that can cause pain, deformity, and functional limitation. It usually arises from inadequate fixation, comminution, or impaired healing biology. Timely surgical intervention is often required to restore elbow function. A 34-year-old female, right-hand dominant homemaker presented with persistent deformity, weakness in extension, and difficulty performing overhead activities 4 months after a conservatively managed right elbow injury. Clinical examination revealed a palpable olecranon defect, and radiographs confirmed an a

A detailed review of 80 patients with severe naso-ethmoid-orbital injuries has facilitated the classification of these injuries into five types. The recognition and diagnosis of each specific injury pattern will define the correct treatment choice in each instance. Special attention should be focused on injuries with comminution and bone loss in the medial wall and floor of the orbit, with loss of cartilaginous nasal support, and with orbital displacement and dystopia. An open, direct approach to these fractures with meticulous reduction, internal fixation, and repair of the medial canthal lig

OBJECTIVES: To assess the changing mid-face fracture patterns using a computed tomography scan.
METHODOLOGY: Fifty patients with mid-face trauma requiring open reduction and fixation were studied using 1.6 mm axial, sagittal, coronal and 3D images. Images were evaluated clinically, intra-operatively and finally were compared with standard Le Fort lines. Results. The male population dominated the female at a ratio of 11.5:1. The majority of the mid-face fractures were seen in the age group of 21-30 years. Road traffic accident (78%) was the major etiological factor followed by work-related acc

INTRODUCTION: Distal radius fractures are the most commonly reported fractures in adults. Treatment has changed in recent years to open reduction and palmar plate fixation. Penetration of the dorsal screw, however, is a well-known complication. Intraoperative anteroposterior and lateral radiographs lack the exact assessment of dorsal screw length and intraoperative measurement is therefore very likely to be inaccurate in a comminuted dorsal radial cortex. Secondary extensor tendon ruptures are reported in up to 6% following palmar plate fixation of distal radius fracture.

Introduction An intertrochanteric (IT) femur fracture is an extra-capsular fracture between greater and lesser trochanters. Unstable IT fractures are those where there is poor contact between fracture fragments (especially medial and posterior cortices), comminution, and fracture pattern, such that the weight-bearing forces tend to displace the fracture further or a reverse oblique type. Proximal femoral nailing (PFN) is one of the modalities for proximal femoral fractures. A newer modality for proximal femoral fracture is PFNA2, i.e., PFN anti-rotation, which makes use of a helical blade for

Using an experimental system that mimics stone fragmentation in the renal pelvis, we have investigated the role of stress waves and cavitation in stone comminution in shock-wave lithotripsy (SWL). Spherical plaster-of-Paris stone phantoms (D = 10 mm) were exposed to 25, 50, 100, 200, 300 and 500 shocks at the beam focus of a Dornier HM-3 lithotripter operated at 20 kV and a pulse repetition rate of 1 Hz. The stone phantoms were immersed either in degassed water or in castor oil to delineate the contribution of stress waves and cavitation to stone comminution. It was found that, while in degass

INTRODUCTION: Open reduction and internal fixation (ORIF) with articular reconstruction is the standard treatment for distal humerus fractures; however, ORIF is challenging in elderly patients with severe comminution and poor bone quality. Although favorable outcomes of total elbow arthroplasty (TEA) have been reported for complex distal humerus fractures deemed unsuitable for ORIF, clinical data in Japanese patients remain limited. This study reports the clinical outcomes of TEA in Japanese patients aged 65 years or older when ORIF was deemed unfeasible.
METHODS: We retrospectively reviewed

Background Displaced proximal humerus fractures in elderly patients are associated with high rates of fixation failure due to poor bone quality and medial column comminution. Augmentation of locking plate fixation with an intramedullary fibular strut autograft has been proposed to enhance medial support and improve outcomes. This study evaluated early radiological and functional outcomes following open reduction and internal fixation (ORIF) using a proximal humerus internal locking system (PHILOS) plate augmented with a fibular strut autograft. Objectives The objectives of this study are to ev

Background Acetabular fractures with quadrilateral plate involvement have been shown to have a high rate of complications. Anatomic suprapectineal plating systems have been developed to manage these injuries with good short-term outcomes. However, long-term maintenance of anatomical reduction and functional outcomes has yet to be established. The aim of this study is to maintain reduction and functional outcomes at a minimum of five years of follow-up. Materials and methods This is a retrospective cohort study from a prospective database examining patients aged over 16 years following fixation

OBJECTIVE: To determine whether fully threaded transiliac-transsacral (TI-TS) fixation is biomechanically superior to partially threaded TI-TS fixation of vertically unstable transforaminal sacral fractures.
METHODS: Vertically unstable zone 2 sacral fractures were created in 20 human cadaveric pelves with a unilateral osteotomy and resection of 1 cm of bone through the foramen of the sacrum to represent comminution. Ten specimens received either 2 7.3-mm fully threaded or 2 7.3-mm partially threaded TI-TS screw fixation at the S1 and S2 body, and every specimen received standard 3.5-mm 8-hol

Objective Epidural hematoma (EDH), most often caused by rupture of the middle meningeal artery secondary to head trauma with fracture of the temporal bone, is a potentially fatal condition that can lead to elevated intracranial pressure, herniation, and death within hours following the inciting traumatic incident, unless surgical evacuation is accomplished. Several markers have been found to be associated with hematoma expansion in intracerebral hemorrhage (ICH) patients, including: the CT Blend Sign, Swirl Sign, and Black Hole Sign. This study aims to examine these markers, along with intrad

PURPOSE: "Damage" is an engineering term defining a period between a state of material perfection and the onset of crack initiation. Clinically, it is a loss of fixation due to microstructural breakdown, indirectly measured as a reduction of stiffness of the bone-implant construct, normalized by the cross-sectional area and length of the bone. The purpose of this study was to characterize damage in a cadaver model of extra-articular distal radius fracture with dorsal comminution treated using 2-column volar distal radius plates.
METHODS: Ten matched distal radii were randomly divided into 2 g

AIMS: The crucial role of the radial head in the stability of the elbow in terrible triad injury is acknowledged. This retrospective study aims to compare the results of resection of a severely comminuted radial head with or without prosthetic arthroplasty as part of the reconstruction for this injury.
PATIENTS AND METHODS: The outcome of radial head resection was compared with prosthetic arthroplasty in 29 and 15 patients with terrible triad injuries, respectively. There were ten female patients (34.5%) in the resection group and six female patients (40%) in the prosthesis group. The mean ag
