For children with craniosynostosis presenting intracranial hypertension and facial hypoplasia, different techniques have been used. Commonly, an initial frontoorbital advancement to expand a tight cranium volume was followed by Le Fort III osteotomy to repair midface abnormalities several months later. The 2-stage surgeries were unified into a 1-step procedure to optimize treatment. We here report results of frontofacial monobloc advancement, applying gradual distraction by a rigid external distraction device. Three patients were treated with excellent functional and cosmetic outcome with high

The newer generation of CT scanners allows reconstruction of images in coronal, sagittal, and oblique planes from a single set of axial scans. These computer-generated images are described as reformatted. We have found an oblique image reformatted along the plane connecting the apex of the orbit and the center of the globe to be especially useful in assessing orbital disorders. We have named this image the longitudinal orbital projection. This projection allows direct visualization of the inferior rectus muscle and orbital floor in acute and old orbital trauma. With the image produced life siz

BACKGROUND: Frontoorbital advancement and remodeling (FOAR) is a common surgical approach used for craniosynostosis, conferring functional and aesthetic benefit. There are few reports examining predisposing factors for postoperative complications within the first year after surgery. This study aimed to establish complication rates and identify risk factors for inferior outcomes in a large population of patients undergoing FOAR.
METHODS: All patients who underwent FOAR from 2013 through 2022 at the authors' institution were retrospectively studied. Perioperative and postoperative data were col

INTRODUCTION: Graves' ophthalmopathy (GO) is the most common and difficult-to-treat extrathyroidal symptom of Graves' disease. Though retraction of the upper eyelid is the most common clinical feature of GO, it can have a much more severe clinical manifesta-tion with symptoms such as conjunctival chemosis, keratopathy, extraocular muscle dysfunction, proptosis of the bulb and dysthyroid optic neuropathy. Treatment methods include control of the thyroid function, corticosteroid and immunosuppressive therapy as well as radiotherapy. These approaches are ineffective in one-third of cases, with pa

The frontal sinus outflow pathway is complex and can be influenced by the configuration of the uncinate process (UP). The UP can attach superior to the lamina papyracea, skull base, and middle turbinate. The factors associated with superior attachment remain unclear. This study analyzed the relationships between different types of superior UP attachment and characteristics of the surrounding structures including the agger nasi cell, skull base, and middle turbinate. This retrospective study utilized computed tomography images of 836 sides with identifiable sinus structure from 434 Taiwanese pa

The aim of this study was to evaluate the morphologic structure of the frontal sinuses with postero-anterior cephalometric radiographs in Turkish adolescents and to compare the findings between genders. The study sample consisted of 148 subjects divided into two groups (74 male subjects; mean age: 14.55 ± 1.42 years-74 female subjects; mean age: 14.95 ± 1.80 years). The right and left maximum height and width of the frontal sinus, maxillary width, nasal width, cranial width, antegonial width parameters were measured in 148 postero-anterior cephalometric radiographs. The data were analyzed usin

OBJECTIVE: To investigate the new surgical pathway of endoscopic frontal sinus surgery for frontal sinus lesions through the upper-agger nasi approach.
METHOD: The computed tomography (CT) scans from 32 patients were collected and subjected to three-dimensional reconstruction by Mimics. The distance in sagittal planes from anterior ethmoid artery to midpoint of axilla and to skull base attachment at anterior middle turbinate was measured. The distance in coronal planes between the perpendicular plate of middle turbinate and the orbital lamina was also detected as well as the height of agger n

OBJECTIVE: The treatment of frontobasilar fractures is a demanding aspect of craniofacial fracture management. A sequel of inadequate or improper fracture management presents cosmetic and functional problems which are very difficult to correct. The aim of this manuscript was to examine a group of growing patients treated for frontobasilar fractures and provide clinicians a possible therapeutic option for the treatment of these challenging fractures.
METHODS: In this investigation, 12 patients under the age of 16 years treated for severe injuries to the frontobasilar region were included. Thei

The surgical treatment of frontal sinus fractures is foreshadowed by fears of late complications. Complications such as meningitis and mucoceles should be prevented by cranialization or obliteration of the frontal sinus. These procedures are still standard treatment despite of recent developments over the last two decades in endoscopic sinus surgery, in medical imaging and surgical instrumentation. Nowadays the role of cranialization and obliteration is challenged by refined endoscopic frontal sinus surgery techniques, the widely-used image-guided systems and the multiplanar high-resolution co

Even as a rare multiple congenital anomalies/mental retardation syndrome, the C-syndrome (CS, or Opitz C-trigonoecephaly syndrome) is, at long last, beginning to attract attention because of its developmental and causal complexity. Also, the possibility that the apparently balanced translocation recently described in an affected Japanese boy may soon provide a molecular/causal insight into this disorder. The manifestations recorded in the previously published patients, those autopsied within recent years, and the unpublished instances in our files suggest that the CS is a heterogeneous genetic

In the conventional fronto-orbital advancement method, there is a limit to advancement because of scalp skin tension and an absence of a supraorbital bar fixating point. In a case of insufficient advancement after the primary operation, a secondary re-advancement must be performed. In such a condition, additional fronto-orbital advancement by distraction osteogenesis has proved to be very useful. The authors used additional distraction osteogenesis in three infant cases: two of nonsyndromic craniosynostosis and one of Apert's syndrome. They were able to perform these operations safely using th

The standardized bilateral fronto-orbital advanced method of osteotomy established at the University of Wuerzburg is applied in all forms of craniosynostosis except scaphocephalus. The intention behind early operation is to halt progression of the disorder and to institute the physiological direction that growth should take. The preoperative severity of the disorder, the particular symptoms of the various malformations concerned, and the postoperative course of growth were analyzed and assessed both clinically and cephalometrically using the retrospective evaluations of the file data of 131 ch

Standard craniofacial techniques to expand the cranial vault were successful in treating elevations of intracranial pressure (ICP) in 7 older children. Of the 7 patients, a diagnosis of craniosynostosis was made in 6 and deformational head deformity with slit ventricle syndrome in 1. There were 5 boys and 2 girls, whose ages ranged from 3 1/2 to 8 years (mean, 5.2 yr). Three patients had previously undergone surgical treatment of craniosynostosis. One patient presented with visual changes and papilledema. Another with a deformational skull deformity was shunt-dependent and was diagnosed with s

The particularities and the authors' management of frontobasal fractures in children are discussed on the basis of cases treated at the authors' department during the last five years. Fifteen children aged between four and 16 years (mean eight years) made up 14.5% of all frontobasal fractures. The causes of the injury were equally divided between falls from a height of at least 2 m and traffic accidents. All children except one were treated immediately after the accident. The exception was a child with an accident two and a half years before operation and two episodes of late onset meningitis.
