Two-stage procedure protocol for minimally invasive plate osteosynthesis technique in the treatment of the complex pilon fracture.

PURPOSE: Here we introduce a two-stage procedure protocol for minimally invasive plate osteosynthesis (MIPO) to treat complex pilon fractures and evaluate surgical wound healing and infection rates after this method was applied in clinical practice. METHOD: This is a retrospective study design. The protocol consisted of immediate (within eight to 24 hours) open reduction and internal fixation of the fibula, using a fibular plate or one third tubular plate and application of an external fixator spanning the ankle joint. Patients were discharged after initial stabilisation and revaluated approx

Endosteal strut augment reduces complications associated with proximal humeral locking plates.

BACKGROUND: Locking-plate technology has renewed interest in plate fixation for treating proximal humerus fractures. Complications associated with these devices, including loss of reduction, screw cutout, and intra-articular penetration, are frequent. Establishing a second column of support may reduce complications and improve clinical outcome scores. QUESTIONS/PURPOSES: We asked whether addition of an endosteal cortical allograft strut, used as an augment to locking-plate fixation for displaced proximal humerus fractures, would reduce complications and improve clinical outcome scores. Broad ligament hematoma causing fetal death in a case of fracture pelvis.

Although motor ventricular accidents complicates 6-7% of all pregnancies, the experience of pelvic fractures in near term gravid patients is limited. We present a unique case of fetal death caused by bilateral broad ligament hematomas following maternal pelvic fracture which improved our understanding of management of such cases.

Complications encountered in proximal humerus fractures treated with locking plate fixation.

OBJECTIVES: We evaluated the complications encountered following locking plate fixation of proximal humerus fractures. METHODS: The study included 103 patients (70 females, 33 males; mean age 62 years; range 21 to 90 years) who were treated with open reduction and locking plate fixation for proximal humerus fractures between September 2005 and April 2009. Fixation was performed using the PHILOS locking plate in 93 patients, and S3 humerus plate in 10 patients. Postoperatively, a shoulder-arm sling was applied for six weeks and a standard rehabilitation program was used in all the patients. In

Chronic Essex-Lopresti injuries: an alternative treatment method.

BACKGROUND: Currently, no technique has met general acceptance for the restoration of forearm longitudinal stability in chronic Essex-Lopresti injuries. The purpose of this study is to present an alternative treatment method for chronic Essex-Lopresti lesions by radial head replacement and ulnar shortening osteotomy. METHODS: Seven patients with a mean age of 42.4 years were included in the study. Five patients had a staged approach, and 2 underwent both procedures simultaneously. The pain level was assessed with the use of a visual analog scale. Elbow, forearm, and wrist range of motion was

[Efficacy of ultrasound controlled direct reposition of fragments of the posterior vertebral body facet].

To calculate canal compromise and decrease of midsagittal diameter caused by retropulsion of fragments into the spinal canal we analyzed the pre- and postoperative computed tomographies of 32 patients with unstable thoracolumbar burst fractures treated by USS (universal spine system). Our intention was to examine the efficiency of ultrasound guided repositioning of the dispaced fragments which was performed in all 32 cases. We found a clear postoperative enlargement of canal area (ASP preoperatively 55%, postop. 80%) and midsagittal diameter (MSD preop. 58%, postop. 78%). 10 of 13 patients pre

Reverse End-to-Side Triceps to Axillary Nerve Transfer for Treatment of Axillary Nerve Palsy After Reverse Total Shoulder Arthroplasty.

Nerve injuries are uncommon, yet significant complications following reverse total shoulder arthroplasty (rTSA), with the axillary nerve being most frequently injured. We report a case of a 67-year-old female who developed persistent motor dysfunction and decreased sensation in the axillary distribution following rTSA for a comminuted proximal humerus fracture. Electrodiagnostic studies confirmed right axillary nerve injury with reduced motor unit recruitment. After no improvement in deltoid function at three months following rTSA, we discussed treatment options for the patient, who opted for

Cerebral Myiasis Associated with Artificial Cranioplasty Flap: A Case Report.

BACKGROUND: Cranioplasty is a commonly performed procedure for the repair of cranial defects. Various materials have been used for this procedure and have a good safety profile. Human cerebral myiasis is an exceedingly rare condition. It involves the invasion of live or dead human tissues by larvae of the insect species dipterous. CASE DESCRIPTION: We describe the first case of cerebral myiasis associated with an artificial cranioplasty bone flap. There was delayed cerebral cortex infestation of the species dipterous after cranioplasty with polymethyl methacrylate bone flap. The patient initi

Good performance of a titanium femoral component in cementless hip arthroplasty in younger patients: 97 arthroplasties followed for 5-11 years.

We performed 97 uncemented primary total hip arthroplasties in 80 patients having an average age of 50 years. The femoral implant was a titanium stem with a proximal circumferential plasma spray-coating. Three different acetabular components were used: a threaded and partly porous-coated design in 70% of the cases. The average follow-up period was 8 years. 1 stem was revised 9 years after insertion due to a comminuted fracture of the proximal femur, 1 stem was revised 9 years after insertion due to a deep infection. No stem revisions were due to aseptic loosening. 1 femora had areas of distal

Fractures of the distal radius. Intermediate and end results in relation to radiologic parameters.

The purpose of this study was to quantify the functional impairment following distal radial fractures and to identify the factors affecting prognosis. One hundred fifteen patients were assessed six months and two years following initial injury. On final assessment, subjectively, 56% had good, 39% had fair, and 5% had poor results. Median grip strength improved from 51% to 78%, range of movement from 87% to 94%, and wrist torque from 93% to 100%. Redisplacement occurred in 59%; only 33% clinically and 19% radiologically had perfect cosmetic results. Radial malunion was important functionally. O

Intramedullary Ulnar Fixation for the Treatment of Monteggia Fracture.

BACKGROUND: Although many pediatric Monteggia fractures can be treated nonoperatively, the presence of any residual radiocapitellar subluxation following ulnar reduction mandates a more aggressive approach to restore and maintain ulnar length. In younger children, restoration and maintenance of ulna length may be achieved through intramedullary fixation of the ulnar shaft. DESCRIPTION: A Steinmann pin or flexible intramedullary nail is introduced percutaneously through the olecranon apophysis and advanced within the medullary canal to the ulnar fracture site. If necessary, the ulnar length an

Three-dimensional quantitative study and functional outcome analysis of coronoid fracture in different elbow injury patterns.

BACKGROUND: Coronoid fractures are often part of a complex fracture-dislocation of the elbow. For an optimum prognosis, it is important to understand the characteristics of coronoid fractures in different elbow injury patterns. Significant differences in these characteristics have been observed in various studies, but these previous studies have suffered from limitations, such as insufficient quantification and unknown prognostic differences among different injury patterns. Therefore, we aimed to quantitatively analyze coronoid fracture characteristics and functional outcomes in different elbo

Timing of diaphyseal femur fracture nailing: is the difference night and day?

INTRODUCTION: Most hospitals are faced with reduced personnel, resources, and provider fatigue or shift changes when day turns to night. For these reasons, some have suggested that diaphyseal femur fractures should be fixed during the daytime. The purpose of this study is to determine whether the time of surgery affects the post-operative difference in femoral version (DFV) and femoral length (DFL) between the fixed and uninjured sides following intramedullary nailing (IMN). MATERIALS AND METHODS: Over a 10-year period, 340 patients underwent IMN of a diaphyseal femur fracture (AO types 32-A

Simultaneous obliteration and treatment of infected frontal sinus fractures: novel use of the fibula flap.

We present two cases of infected frontal sinus fractures requiring simultaneous obliteration of the frontal sinus and reconstruction of the horizontal buttress. In both cases, a free fibula flap was used for obliteration and bony reconstruction in a single stage. We believe that this is a novel technique for treatment of infected frontal sinus fractures obviating the need for secondary staged procedures.
