Why Do Patients With Aneurysmal Subarachnoid Hemorrhage Postpone Treatment?

OBJECTIVE: Early diagnosis of cerebral arterial aneurysm (CAA) rupture is critical, as recurrent ruptures significantly worsen the prognosis and increase mortality. The time to seek help varies: thunderclap headache and seizures prompt immediate treatment, while less obvious manifestations are often overlooked. The relationship between the nature of symptoms and the promptness of CAA rupture diagnosis remains poorly understood. To identify predictors of help-seeking behavior in patients with recurrent CAA ruptures. METHODS: A retrospective single-center analysis from the medical records of 44

Unilateral Duplication of the Foramen Spinosum With Unusual Bilateral Variation of Hypoglossal Canals in a Dry Human Skull: Their Clinical Implications.

The foramen spinosum is one of the foramina of the skull that lies bilaterally close to the spine of the sphenoid in the middle cranial foss … n the course of contents. Recognition of such variations is of considerable clinical importance for neurosurgeons, skull base surgeons, and radiologists, as these anomalies may influence surgical planning and increase the risk of inadvertent neurovascular injury during procedures involving the middle and posterior cranial fossae. We report a case with duplicati …

Fostering evidence building for brainstem cavernous malformations: a critical appraisal of available evidence's quality through a systematic review and BSCM Reporting Guideline proposal.

Brainstem cavernous malformations (BSCMs) are clusters of dilated, thin-walled, sinusoidal vessels prone to hemorrhage, often leading to life-threatening neurological deficits. Despite their rarity, prompt intervention is frequently required. Advances in diagnostics and treatment, including microsurgery and stereotactic radiosurgery, have improved management, yet no clear consensus exists due to considerable variability and lack of standardization across studies. Inadequate reporting further limits the generation of high-quality evidence. This study aims to evaluate publishing rigor and propos

[Aggressive pituitary tumors and carcinomas: modern classification, advances and prospects in treatment].

Despite slow growth of most pituitary tumors and high rates of total resection and/or effective therapy, pituitary neoplasms are characterized by aggressive behavior with high growth rate, frequent relapses and resistance to standard treatments in 10% of cases. In modern WHO classifications of tumors of the central nervous system, endocrine and neuroendocrine tumors, the authors propose the definition «pituitary neuroendocrine tumor» instead of previous «pituitary adenoma» and «metastasizing pituitary neuroendocrine tumor» instead of «pituitary carcinoma». Currently, there are no effective pro

Intraventricular Subependymoma With Obstructive Hydrocephalus: A Case Report and Literature Review.

Subependymomas are benign tumors of the ventricles that grow from the ventricular wall into the cerebrospinal fluid spaces within the brain, obstructing the flow of the cerebrospinal fluid and causing obstructive hydrocephalus. It is estimated that ependymomas represent between 0.2% and 0.7% of all intracranial tumors. They arise most frequently in the fourth ventricle (50-60%) and the lateral ventricles (30-40%). We present the case of a 50-year-old patient, previously diagnosed with an intraventricular process, admitted in our clinic. At neurological examination, the patient was cooperative,

An in vivo evaluation of the safety and efficacy of using decellularized bovine parietal peritoneum membranes as dural substitutes.

PURPOSE: The reconstruction of dura matter is a challenging problem for neurosurgeons. A number of materials for dural reconstruction have recently been developed, but some of them have poor biocompatibility, poor mechanical properties, and adverse effects. Bovine parietal peritoneum is a promising natural material for regenerative medicine and reconstructive surgery. In this study, we conducted an in vivo evaluation of the safety and efficacy of using decellularized bovine peritoneum membranes (BPMs) as natural dural substitutes in a rabbit model. METHODS: The dural defects in mature New Zea…

Aesthetic transaxillary subpectoral placement of vagus nerve stimulator in children and young adults: A technical note.

INTRODUCTION: Vagus nerve stimulation (VNS) is a neuromodulation therapy for drug-resistant epilepsy (DRE), refractory status epilepticus, and treatment-resistant depression. The lead is tunneled into the subcutaneous space and connected to the generator, which is usually implanted in a subcutaneous pocket below the clavicle. Surgical complications in the chest region include skin breakdown or infection. An alternative approach is to perform a subclavear subpectoral implantation. In our surgical series, we report a new aesthetic implantation method for VNS generators in children and young pati
