Fibrodysplasia ossificans progressiva (FOP) is an extremely rare and disabling genetic disorder characterized by congenital malformation of the great toes and by progressive heterotopic endochondral ossification in predictable anatomical patterns. Although elevated levels of bone morphogenetic protein 4 (BMP4) occur in lymphoblastoid cells and in lesional cells of patients with FOP, mutations have not been identified in the BMP4 gene, suggesting that the mutation in FOP may reside in a BMP4-interacting factor or in another component of the BMP4 pathway. A powerful antagonist of BMP4 is the sec

The results of operative resection of a post-traumatic proximal radioulnar synostosis performed by one surgeon in eighteen limbs of seventeen consecutive patients during an eight-year period were reviewed retrospectively. The resection was performed an average of nineteen months after the injury; eight limbs had the resection less than twelve months after the injury. A free fat graft was used in the first eight patients. No adjuvant non-steroidal anti-inflammatory medication or low-dose radiation was used postoperatively as prophylaxis against heterotopic ossification. We classified the proxim

BACKGROUND: Various salvage surgical procedures for painful hip dislocation in adolescent patients with cerebral palsy exist. To date, no significant differences among these surgical techniques have been described. In our institution the McHale procedure is the standard of care for painful chronically dislocated hips with or without deformity of the femoral head in patients with cerebral palsy. This study focuses on mid-term results after surgical treatment.
METHODS: Surgical reports and patient charts were analyzed retrospectively. All x-rays were evaluated and migration of the proximal femu

INTRODUCTION: The incidence of geriatric acetabular fractures continues to increase due to demographic changes. In the elderly, anterior column fractures are common, and standard approaches are associated with a considerable risk for surgery-associated complications. Therefore, a minimally invasive approach was developed in our department. The aim of this study was to examine early and mid-term results regarding the use of this novel two-incision minimally invasive (TIMI) approach in patients aged over 55 years with acetabular fractures.
METHODS: From July 2007 to April 2014, 47 patients aged

STUDY DESIGN: Focused review of the literature.
OBJECTIVE: Assist spine specialists in diagnosis and treatment of cervical spine anomalies found in selected genetic syndromes.
SUMMARY OF BACKGROUND DATA: Cervical spine instability and/or stenosis are potentially debilitating problems in many genetic syndromes. These problems can be overlooked among the other systemic issues more familiar to clinicians and radiologists evaluating these syndromes. It is imperative that spine specialists understand the relevant issues associated with these particular syndromes.
METHODS: The literature was revi

BACKGROUND: Patients with a traumatic brain injury (TBI) and bone fractures often show an enhanced fracture healing, as well as an increased incidence of heterotopic ossifications (HO). It has been suggested that unknown osteoinductive factors may be released by the injured brain into the systemic blood circulation and act peripherally on the affected tissues. The aim of this study was to investigate whether serum from TBI patients is osteoinductive.
METHODS: Sixty-one consecutive patients were classified into four groups: TBI and long-bone fracture (group I, n = 12), isolated severe TBI (gro

BACKGROUND: Periprosthetic supracondylar fractures of the femur after total knee arthroplasty are not common but are usually difficult to treat due to the advanced age of patients and frequently accompanying osteoporosis. Retrograde intramedullary nailing can be effective in promoting healing of these fractures by providing sufficient stability, but the number of beneficiaries is small due to its limited applicability and the postoperative function has rarely been assessed. This study evaluated the efficacy of retrograde intramedullary nailing for the treatment of periprosthetic supracondylar

Despite the technical and clinical limitations of surface EMG, it is essential in the physical medicine and rehabilitation field. Surface EMG has evolved from a secondary means of clinical assessment to a primary factor in determining and predicting clinical outcomes. Computer models of electrical muscular activity are currently implemented to assist in designing proper instrumentation and electrode with optimum dimensions. These models could be expanded to simulate pathological motor functions to help understand functional abnormalities even before clinical interventions. Currently, several g

AIM: Ossification of the ligamentum flavum (OLF) is a primary cause of thoracic myelopathy. A relatively safe surgical technique based on radiological type is described for OLF-induced thoracic myelopathy.
MATERIAL AND METHODS: Forty patients with thoracic myelopathy caused by OLF were studied retrospectively. The OLF was divided into fused and non-fused types according to the computed tomography and magnetic resonance imaging findings. All patients underwent posterior decompression. For the fused type, open-door laminectomy and for the non-fused type, French-door laminectomy surgical techniq

Considerable progress has been made recently in understanding the complex pathogenesis and treatment of spondyloarthropathies (SpA). Currently, along with traditional disease modifying anti-rheumatic drugs (DMARDs), TNF-α, IL-12/23 and IL-17 are available for treatment of such diseases as ankylosing spondylitis (AS) and psoriatic arthritis (PsA). Although they adequately control inflammatory symptoms, they do not affect the abnormal bone formation processes associated with SpA. However, the traditional therapeutic approach does not cover the regenerative treatment of damaged tissues. In this r

This study investigated the pathological process of Notch signaling in the osteogenesis of ligamentum flavum tissues and cells, and the associated regulatory mechanisms. Notch receptors, ligands, and target genes were identified by quantitative polymerase chain reaction (qPCR) in ligamentum flavum cells and immunohistochemistry in ligamentum flavum sections from ossification of the ligamentum flavum (OLF) patients and controls. The temporospatial expression patterns of JAG1/Notch2/HES1 in human ligamentum flavum cells during osteogenic differentiation were determined by qPCR. Lentiviral vector

Styloid-carotid artery syndrome was first described by Eagle and is associated with cervical and facial pain caused by head movement resulting from mechanical compression of the carotid nerve plexus due to an elongated styloid process. The case of a 49-year-old man with persistent cervical pain, neurological symptoms and an elongated styloid process of 7.5 cm is reported here; this patient was successfully treated using Piezosurgery. In addition, a literature review is included.

BACKGROUND AND OBJECTIVE: The elongated styloid process may produce characteristic head and neck pain syndromes, commonly known as Eagle's syndrome. An awareness of this syndrome is important to all health practitioners involved in the diagnosis and treatment of neck and head pain. It has been estimated that 2 to 28% of the general adult population has radiographic appearance of elongated styloid process. The objective of the study was to assess the elongation of styloid process on digital panoramic radiographs and to evaluate the prevalence of elongation according to age, sex and types.

PURPOSE: The aim of this study was to investigate the proliferation and differentiation behaviour of a defined cell population gained from the human growth plate, namely, chondro-progenitorcells (CPCs), in the initial inflammatory phase of growth plate injury response in vitro.
METHODS: Growth plate cells were sorted via FACS and differentiated along adipogenic and osteogenic lineage to confirm their progenitor features. To mimic the inflammatory phase of injury response at the growth plate they were treated with IL-1β and exposed to cyclic mechanical loading. A BrdU assay was used to investi

Muenke syndrome caused by the FGFR3(P250R) mutation is an autosomal dominant disorder mostly identified with coronal suture synostosis, but it also presents with other craniofacial phenotypes that include mild to moderate midface hypoplasia. The Muenke syndrome mutation is thought to dysregulate intramembranous ossification at the cranial suture without disturbing endochondral bone formation in the skull. We show in this study that knock-in mice harboring the mutation responsible for the Muenke syndrome (FgfR3(P244R)) display postnatal shortening of the cranial base along with synchondrosis gr

Each year, 1.4 million people in the United States are seen in a hospital for a traumatic brain injury. Those with moderate-to-severe traumatic brain injury frequently go through a course of inpatient neurorehabilitation prior to discharge back into the community. A broad overview of neurorehabilitation is presented, including the standards for admission to inpatient rehabilitation and the members and roles of the neurorehabilitation team. Common medical complications that are managed after moderate-to-severe traumatic brain injury are reviewed. The spectrum of arousal issues is summarized. Th

OBJECTIVE: An intercostal muscle (ICM) flap is used to buttress the bronchial stump or bronchial anastomosis during thoracic surgery for airway reconstruction. Such flaps sometimes show ossification after surgery. Previous reports have suggested that such ossification requires a functional periosteum and good vascularization. We examined the background of ICM flap ossification and its relationship with complications and pain after surgery.
METHODS: We surveyed the clinical records of 47 patients who underwent bronchial stump reinforcement with an ICM flap during thoracic surgery at Kansai Med
