Rotator cuff re-tear or non-healing: histopathological aspects and predictive factors.

PURPOSE: The aim of the study was to evaluate the histopathological changes that occur in the tendon and subacromial bursal tissue in patients with rotator cuff tear trying to correlate these changes to their healing capability. METHODS: Eighty-four patients were clinically evaluated with the Constant Scale. Radiographs and MRI were performed preoperatively and ultrasound were performed postoperatively. For each patient, a biopsy of the supraspinatus tendon and subacromial bursa was performed, and the specimens were histopathologically analyzed. RESULTS: Tendons histopathological features co

Role of platelet-rich plasma in articular cartilage injury and disease.

Clinical and laboratory research aimed at biological approaches to cartilage repair are currently in high demand due to the poor regenerative capacity of articular cartilage in the setting of a diseased articular environment. Platelet-rich plasma (PRP) takes advantage of supraphysiological concentrations of platelets and their growth factors harbored in α-granules, which together attempt to return the diseased articular cartilage to a preinjury state. The local use of PRP directly at the site of cartilage injury is thought to stimulate a natural healing cascade and accelerate the formation of

Does latissimus dorsi tendon transfer provide a normal scapular rhythm and external rotation strength in posterosuperior massive irreparable rotator cuff tears? A kinematic analysis in a retrospective cohort.

BACKGROUND: Posterosuperior massive irreparable rotator cuff tears (PMIRT) are rare and disabling conditions. When conservative treatment fails, latissimus dorsi tendon transfer (LDTT) is a viable surgical option for symptom relief in carefully selected patients. However, its effectiveness in restoring glenohumeral function and its influence on scapulothoracic rhythm remain subjects of ongoing debate. PURPOSE: The purpose of this study was to evaluate the clinical outcomes of LDTT and assess its impact on scapulothoracic rhythm using kinematic and electromyographic analysis. MATERIAL AND MET

Does a brace influence clinical outcomes after arthroscopic rotator cuff repair?

PURPOSE: The goal of this study was to report the clinical effects of two different braces after rotator cuff repair. METHODS: Forty patients who underwent an arthroscopic rotator cuff repair were prospectively allocated in this study. Twenty patients were immobilized in 15° external rotation brace (ER Group), and twenty were immobilised in an internal rotation sling (IR Group). For all patients, four surveys were done: in the immediate pre-operative period (T0), at 1 month (T1), at 3 months (T2) and at 6 months after surgery (T3). Range of motion and pain were evaluated by an independent phy

Treatment of Osteochondral Fracture of the Lateral Femoral Condyle with TWINFIX Ti Suture Anchor "X"-Shaped Internal Fixation under Arthroscopy: A Surgical Technique and Three Cases Report.

Osteochondral fracture of the lateral femoral condyle is a rare intra-articular injury with or without patellar dislocation. This kind of disease is commonly seen in the knee joint sprain during strenuous activity. At present, open reduction is often used to treat osteochondral fractures. In recent years, with the development of arthroscopy, we have been able to complete the reduction and internal fixation of fractures under arthroscopy. In this paper, three cases of osteochondral fracture of lateral femoral condyle were treated with arthroscopic TWINFIX Ti suture anchor internal fixation, and

A novel surgical approach to lipomatous tumours of the deltoid region.

Resection of large lipomatous tumours in the subdeltoid region remains technically challenging due to the risk of injury to the axillary neurovascular bundle. We describe a novel deltoid release and reinsertion technique for resection of large lipomatous tumours of the sub-deltoid region and report the functional and oncologic outcomes of six patients who underwent this procedure. Three cases were diagnosed histologically as atypical lipoma and three cases were diagnosed as lipoma. There was one local recurrence in a case of an atypical lipoma. Rotator cuff function was comparable to that of t

Range of motion limitation after rotator cuff repair.

HYPOTHESIS: This study was conducted to identify preoperative factors correlating with limited motion after rotator cuff repair (RCR) and to evaluate the affect of loss of motion on outcome. We hypothesized that patients with preoperative ROM loss, diabetes, and workman's compensation claims would exhibit postoperative ROM loss at 3 months. MATERIAL AND METHODS: Preoperative and postoperative evaluations, including outcomes assessment and physical examination parameters, were reviewed for 345 patients who underwent RCR. Correlations between demographic, physical examination, and surgical vari

Evaluation of the effects of extremity elongation on brachial plexus nerves via intraoperative neuromonitoring in patients undergoing reverse total shoulder arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is one of the most common treatment method for irreparable rotator cuff tear. One of the significant complications of this surgery is nerve injury. This study aimed to identify the patterns of brachial plexus nerve involvement and to determine the surgical stages associated with increased nerve vulnerability during reverse total shoulder arthroplasty using intraoperative neuromonitoring (IONM). Additionally, the relationship between extremity lengthening, acromiohumeral distance (AHD) changes and intraoperative nerve events were evaluated.

A combination of subscapularis tendon transfer and small-head hemiarthroplasty for cuff tear arthropathy: a pilot study.

This pilot study reports the clinical outcomes of a combination of partial subscapularis tendon transfer and small-head hemiarthroplasty in patients with rotatator cuff arthropathy. A total of 30 patients (30 shoulders; eight men and 22 women) with a mean age of 74 years (55 to 84) were assessed at a mean follow-up of 31 months (24 to 60). The inclusion criteria were painful cuff tear arthropathy with normal deltoid function and a non-degenerative subscapularis muscle and tendon and a preserved teres minor. Outcome was assessed using the University of California Los Angeles score, the Japanese

Reverse Delta-III total shoulder replacement combined with latissimus dorsi transfer. A preliminary report.

BACKGROUND: Reverse total shoulder arthroplasty allows the restoration of active overhead elevation in patients with a massive rotator cuff tear and pseudoparesis of elevation. However, it does not restore active external rotation, the lack of which can also constitute a substantial functional handicap and compromise the outcome of this arthroplasty. Latissimus dorsi tendon transfer reliably restores control of active external rotation in rotator-cuff-deficient shoulders. In this preliminary study, we assessed the results of the combination of a latissimus dorsi transfer to the greater tuberos

[The surgical treatment of scapular fractures].

Scapular fractures are predominantly managed non-operatively. However, a subset of patients develops persistent pain, weakness, and functional impairment due to malunion, non-union, rotator cuff dysfunction, or scapulothoracic dyskinesia. Early identification of patients at risk for ongoing symptoms is essential, as timely surgical intervention may improve outcomes. Owing to the lack of randomized controlled trials comparing operative and non-operative management, surgical indications remain controversial. This article reviews the biomechanics of the scapula, mechanisms of injury, fracture typ

Subcutaneous Implantable Cardioverter-Defibrillator Lead Dislodgement Masquerading as Left Shoulder Pain.

Implantable cardioverter-defibrillators (ICDs) have demonstrated efficacy in the prevention of sudden cardiac death secondary to cardiac arrhythmias in eligible patients. Complications with the subcutaneous ICD (S-ICD) are rarer than with the transvenous ICD but do still exist. Our patient presented four weeks after the insertion of S-ICD with complaints of left shoulder pain radiating to the chest wall and swelling over the S-ICD site. He was initially treated for rotator cuff injury and subacromial impingement syndrome but upon obtaining chest radiography was found to have a lead displacemen

Management of complications after reverse shoulder arthroplasty.

Reverse total shoulder arthroplasty (RTSA) has become the treatment of choice for patients with rotator cuff arthropathy. Complication rate after RTSA has been reported to be three to five times that of conventional total shoulder arthroplasty. Intraoperative and postoperative complications include neurological injury, infection, dislocation or instability, acromial or scapular spine fracture, hematoma, and scapular notching. Knowledge of optimal component placement along with preoperative planning and recognition of risk factors are essential in optimizing patient outcome. The purpose of this

Impingement pain affects kinematics of breast cancer survivors in work-related functional tasks.

BACKGROUND: Breast cancer survivors may encounter upper limb morbidities post-surgery. It is currently unclear how these impairments affect arm kinematics, particularly during functional task performance. This investigation examined upper body kinematics during functional tasks for breast cancer survivors and an age-matched control group. METHODS: Fifty women (aged 35-65) participated: 25 breast cancer survivors who had undergone mastectomy and 25 age-range matched controls. Following basic clinical evaluation, including shoulder impingement tests, motion of the torso and upper limbs were tra

Dynamic analysis of intraarticular pressure in the glenohumeral joint.

Dynamic analysis of intraarticular pressure was performed in 180 glenohumeral joints. The intraarticular pressure demonstrated characteristic changes during shoulder movement. In the healthy group minimum pressure was measured at 40 degrees of elevation in the scapular plane. In patients with contractures the pressure increased in the early phase of elevation and persisted. Pressure changes of the group with incomplete tears of the rotator cuff resembled those of the healthy group, whereas those in the massive tear group showed only slight pressure changes. When patients with rotator cuff tear

Individualized rotation of left double lumen endobronchial tube to improve placement success rate: a randomized controlled trial.

BACKGROUND: In conventional practice, the left double lumen tube (DLT) is rotated 90° counterclockwise when the endobronchial cuff passes glottis. Success rate upon the first attempt is < 80%, likely owing to varying morphology of the bronchial bifurcation. METHODS: We conducted a randomized controlled trial to compare 90° counterclockwise rotation versus individualized degree of rotation in adult patients undergoing elective thoracic surgery using left DLT. The degree of rotation in the individualized group was based on the angle of the left main bronchi as measured on computed tomography (C

Evaluating Cost Disparities and Utilization of Shoulder Arthroscopy: A National Analysis of Racial, Ethnic, and Socioeconomic Determinants.

BACKGROUND: Shoulder arthroscopy is increasingly common for treating various shoulder pathologies, but racial, ethnic, and geographic disparities in its use persist, especially as more procedures move to ambulatory surgery centers. Identifying and addressing these disparities is crucial for ensuring equitable orthopaedic care in the United States. HYPOTHESIS/PURPOSE: It was hypothesized that racial and ethnic disparities exist in shoulder arthroscopy utilization. The purpose of this study was to assess these disparities and their contributing factors in greater detail. STUDY DESIGN: Descript

Effect of concentric exercise-induced fatigue on proprioception, motor control and performance of the upper limb in handball players.

BACKGROUND: The last phases of a competitive game are when shoulder injuries most commonly happen, and fatigue is thought to be a major contributing factor, perhaps because of reduced proprioception and motor control. The purpose of this study was to investigate the effect of concentric fatigue on proprioception, motor control, and performance of the upper limb in handball players. METHODS: Forty-six right-handed handball players (all males, age 26.1 ± 5.54 years) were included in this test-retest laboratory experiment. Proprioception was assessed using joint reposition sense (JRS), threshold
