Total aortic arch repair: risk factor analysis and follow-up in 199 patients.

OBJECTIVES: Aortic arch surgery is associated with substantial perioperative risks. New prostheses as well as novel perfusion techniques have been developed to reduce the risks of these procedures. The routine application of these new techniques warrants reassessment of risk factors of aortic arch repair. METHODS: Between April 2010 and December 2015, 199 patients [61% male, median age 63 years (interquartile range 52-70 years)] underwent total aortic arch repair in our institution. Forty-four per cent of the patients presented with acute aortic dissections (ADs, 32% with malperfusion), 22% w

Placement of endovascular stent-grafts for emergency repair of acute traumatic aortic rupture: a single-centre experience.

The aim of this retrospective study was to evaluate mid- and long-term results of endovascular stent-graft placement for emergency repair of acute traumatic thoracic aortic rupture. From 1996 through 2005, 22 consecutive patients (mean age: 38.7 years) underwent endovascular repair of acute traumatic thoracic aortic rupture located at the aortic isthmus in most cases. All patients were at high surgical risk due to severe associated injuries. The endografts were inserted via femoral or iliac artery access under fluoroscopic guidance. Follow-up was performed postinterventionally, at 6 and 12 mon

Targeting the NLRP3 Inflammasome With Inhibitor MCC950 Prevents Aortic Aneurysms and Dissections in Mice.

Background Aortic aneurysms and dissections are highly lethal diseases for which an effective treatment strategy is critically needed to prevent disease progression. The nucleotide-binding oligomerization domain-like receptor pyrin domain containing 3 (NLRP3)-caspase-1 inflammasome cascade was recently shown to play an important role in aortic destruction and disease development. In this study, we tested the effects of MCC950, a potent, selective NLRP3 inhibitor, on preventing aortic destruction and aortic aneurysm and dissection formation. Methods and Results In a model of sporadic aortic ane

CE-MRA-FLOWnet: Fast and Accurate Generative AI-Based Aortic Hemodynamic Mapping from Contrast-Enhanced MRA.

BACKGROUND: 4D flow MRI hemodynamic biomarkers (e.g. peak velocity and wall shear stress [WSS]) have shown promise for improved risk-stratification in patients with thoracic aortic disease (TAD). However, lengthy 4D flow scan times, complex data analysis and need for dedicated expertise limit clinical translation. In this study, we developed a fluid-physics informed deep generative adversarial neural network, CE-MRA-FLOWnet, to predict aortic hemodynamics directly from standard-of-care contrast-enhanced magnetic resonance angiography (CE-MRA) images. METHODS: We retrospectively identified 139

Emergency treatment of acute rupture of the descending thoracic aorta using endovascular stent-grafts.

Open surgical management of acute rupture of the descending thoracic aorta (DTA) is associated with high mortality and morbidity. Endovascular stent-grafts (ESGs) could provide a less invasive treatment alternative to conventional open surgery. The purpose of this report detailing our experience using ESG for treatment of acute rupture of the DTA is to determine the indications for endovascular repair. From June 2000 to April 2005, 17 patients presenting rupture of the DTA were treated using commercially available ESGs at our institution. There were two women and 15 men, with a mean age of 41.

Characteristics of thoracic aortic aneurysm rupture in vitro.

UNLABELLED: Ascending thoracic aortic aneurysms (ATAAs) are focal dilatations in the aorta that are prone to rupture or dissection. To accurately evaluate the rupture risk, one must know the local mechanical conditions at the rupture site and understand how rupture is triggered in a layered fibrous media. A challenge facing experimental studies of ATAA rupture is that the ATAA tissue is highly heterogeneous; experimental protocols that operate under the premise of tissue homogeneity will have difficulty delineating the location conditions. In this work, we employed a previously established poi

Role of bovine serum albumin-glutaraldehyde glue in the formation of anastomatic pseudoaneurysms.

BACKGROUND: Questions exist concerning the safety of bovine serum albumin-glutaraldehyde (BSAG) glue in thoracic aortic surgery, vis-à-vis embolization and pseudoaneurysm formation. We examined clinical experience with BSAG glue to determine if such problems were detected. METHODS: We studied 99 consecutive patients (25 female and 74 male, age range 27 to 86 years) in whom BSAG glue or similar product was used for reinforcement of thoracic aortic suture lines (n = 87 BSAG glue, 12 GRF [French] glue). BSAG glue was used selectively and sparingly for acute aortic dissection or tissue fragility.

Correcting mitochondrial loss mitigates NOTCH1-related aortopathy in mice.

Loss-of-function mutations in NOTCH1 were previously linked to thoracic aortopathy, a condition for which non-surgical treatment options are limited. Based on clinical proteome analysis, we hypothesized that mitochondrial fusion and biogenesis in aortic smooth muscle cells (SMCs) are crucial for regulating the progression of NOTCH1-related aortopathy. Here we demonstrate that SMC-specific Notch1 knockout mice develop aortic pathology, including stiffening, dilation and focal dissection. These changes are accompanied by decreased expression of MFN1/2 and TFAM, mirroring findings in human patien

Open Surgical Repair Remains the Gold Standard for Treating Aortic Arch Pathology.

BACKGROUND: Endovascular arch repair technology is driven in large part by the assumption that open arch operations are high-risk. We wanted to evaluate the clinical results of open arch reconstruction in the modern era in a large group practice. METHODS: From October 2003 to June 2014, 567 patients underwent aortic arch operations: hemiarch repair was performed in 429 patients (75.7%; group A), total arch repair in 129 (22.7%; group B), and patch repair in the remaining 9 (1.6%). The procedure was an emergency in 88 patients (20.5%) in group A and in 41 patients (31%) in group B. Redo sterno

Thoracic aortic aneurysms and pregnancy.

Half of acute aortic dissection in women under the age of 40 occurs during pregnancy or peripartum period. Marfan syndrome is the most common syndromic presentation of ascending aortic aneurysm, but other syndromes such as vascular Ehlers-Danlos syndrome, Loeys-Dietz syndrome and Turner syndrome also have ascending aortic aneurysms and the associated cardiovascular risk of aortic dissection and rupture. Management of aortic root aneurysm has been established in recent recommendations, even if levels of evidence are weak. Pregnancy and postpartum period should be followed very closely and deter
