[Radiotherapy of non-malignant diseases: principles and recommendations].

The plenty options and high quality of radiation therapy for non-malignant disorders is not well known outside the field of radiology. It is necessary to transfer this information to cooperating general practitioners, surgeons, orthopedics and other specialists. To warrant quality assurance and quality control and to allow a uniform performance of radiotherapy of non-malignant conditions, general guidelines and recommendations according to the German Working Group of Scientific Medical Societies are useful. This paper summarizes the essential aspects of radiotherapy for non-malignant diseases:

[Indomethacin--short-term therapy vs. single low dosage radiation for prevention of periarticular ossifications after total hip endoprosthesis].

INTRODUCTION: With a general incidence of about 40% periarticular ossifications (PAO) constitute one of the more frequent complications after total hip arthroplasty. A prospective, randomized therapy--study investigates the prophylactic values of Indomethacin-Short-Term-Therapy and Single-Dose-Radiatio, respectively. MATERIAL AND METHODS: Consecutive patients for elective, cementless THA were randomized; the Indomethacin-Group (A, n = 31 at the end of the study) was admitted 100 mg Indomethacin/supp. at the day of operation and 3 x 25 mg orally from postoperative day 1 to 10. Other NSAID were

Complications of shoulder arthroplasty.

Early and mid-range followup studies of shoulder arthroplasty have been encouraging, showing good and excellent results in > 90% of shoulders. Despite this success, complications in shoulder replacement surgery are inevitable, with an incidence of approximately 14%. Numerous complications have been identified and include the following factors in order of decreasing frequency: instability, rotator cuff tear, ectopic ossification, glenoid component loosening, intraoperative fracture, nerve injury, infection, and humeral component loosening. Successful treatment of these difficulties requires car

A Case of Juvenile Stroke due to Carotid Artery Dissection from an Elongated Styloid Process-Revisiting Conservative Management.

Carotid artery dissection is a significant etiology of juvenile stroke. Blunt trauma from an elongated styloid process can rarely cause carotid artery dissection, which is one of well-known clinical presentations of Eagle's syndrome as known as stylocarotid syndrome. Growing number of publications contributed improved awareness and diagnostic modalities for this clinical entity, thus the carotid artery dissection from an elongated styloid process is often diagnosed appropriately. The management of carotid artery dissection in stylocarotid syndrome tends to be nonconservative (ie, removal of th

HLA-B27-mediated activation of TNAP phosphatase promotes pathogenic syndesmophyte formation in ankylosing spondylitis.

Ankylosing spondylitis (AS) is a type of axial inflammation. Over time, some patients develop spinal ankylosis and permanent disability; however, current treatment strategies cannot arrest syndesmophyte formation completely. Here, we used mesenchymal stem cells (MSCs) from AS patients (AS MSCs) within the enthesis involved in spinal ankylosis to delineate that the HLA-B27-mediated spliced X-box-binding protein 1 (sXBP1)/retinoic acid receptor-β (RARB)/tissue-nonspecific alkaline phosphatase (TNAP) axis accelerated the mineralization of AS MSCs, which was independent of Runt-related transcripti

Relationship of stylohyoid chain and cervical internal carotid artery detected by 3D angiography.

OBJECTIVE: The calcification of the stylohyoid chain (SHC), elongated styloid process (SP), larger SP' angle and its shortened distance of cervical internal carotid artery (CICA) are risk factors for bony compression and the stylocarotid syndrome. METHODS: 3D-CTAs of 125 patients were analyzed in terms of the SP length, its angulations, type of the SHC and relationships of its proximity to the CICA. RESULTS: Elongated SP was observed frequently both in females (47%) and males (55%). The mean distance of the CICA to the SP was calculated as 8.2 ± 3.3 mm. This short distance between the CICA a

Metaplastic breast carcinoma with extensive osseous differentiation: a report of two cases and review of the literature.

Invasive breast cancer with osseous metaplasia is rare. Here we report two cases of metaplastic breast carcinoma with extensive osseous differentiation. Case 1: The patient was a 60-year-old woman with a right breast tumor, about 4 cm in diameter. Mammogram and ultrasound presented an irregular-shaped mass suspected for malignancy. Core needle biopsy confirmed invasive carcinoma and the patient underwent a modified radical mastectomy. Case 2: The patient was a 48-year-old woman with a left breast tumor, about 3 cm in diameter. Mammogram demonstrated a well-circumscribed mass with extensive den

Fibrodysplasia ossificans progressiva (FOP) in South Africa: dental implications in 5 cases.

Fibrodysplasia ossificans progressiva (FOP) is a severe genetic disorder in which progressive ossification of subcutaneous tissues leads to immobility and profound physical handicap. Dental management of affected persons may be constrained by impaired mobility of the temporomandibular joints. Equally, the traumatic aspects of dental intervention can exacerbate the ossification process. In the later stages of the disorder, thoracic immobility impairs pulmonary ventilation and compromises dental anesthesia. We have undertaken dental appraisal and treatment of 5 South Africans with FOP, with ages

Acromioclavicular motion after surgical reconstruction.

PURPOSE: A retrospective long-term study was carried out to determine whether there was any correlation between the clinical motion of the acromioclavicular joint evaluated by a test we set up using 90° of abduction and 0° of external rotation against resistance [90°/0°RTest] and the cross arm test (compared to the healthy side) and full return to everyday activities after surgical repair. METHODS: A clinical and radiographic evaluation was carried out on 51/80 subjects at a 5.4-year mean follow-up, treated for acromioclavicular joint dislocation with an extra-articular artificial loop, betwe

[Influence of thyroid cartilage ossification in laryngeal ultrasound].

BACKGROUND: Topological features of the larynx and hypopharynx may be depicted during sonographic examination of the neck, although ultrasound is seen of minor diagnostic importance. The varying extend of thyroid cartilage ossification appears to be the major limiting factor. We intended to assess the sonographic accessability of laryngeal structures with high end resolution ultrasound devices. METHODS AND PATIENTS: We examined 101 patients endoscopically and by ultrasound. Normal laryngeal findings and pathologies were examined and analysed considering limitations by thyroid cartilage ossifi

Advancement in post-meningitic lateral semicircular canal labyrinthitis ossificans.

OBJECTIVE: To assess whether lateral semicircular canal (LSCC) ossification is more advanced than that in the cochlear basal turn, in order to judge the value of the former as a predictor. METHODS: Retrospective review of 33 paediatric patients from our cochlear implant programme, with profound sensorineural hearing loss after bacterial meningitis. Magnetic resonance imaging (MRI), computed tomography (CT) scans and operative findings were reviewed. RESULTS: On CT, LSCC ossification scores were more advanced than those for the cochlear basal turn in 69.9 per cent of implanted ears. Forty-fiv

The effect of Emdogain on ectopic bone formation in tubes of rat demineralized dentin matrix.

BACKGROUND: Emdogain (EMD) is made from enamel matrix proteins (EMPs) from the tooth germ of swine and propylene glycol alginate (PGA) as a matrix. The function of EMD is known to differentiate cells of the dental follicle into cementoblasts. However, little is known about the effect of EMD on mesenchymal cells in other tissue. OBJECTIVE: The purpose of this study was to investigate whether EMD has the ability to induce hard tissue when applied with or without demineralized dentin matrix. METHODS: Half of the dentin tubes prepared from rat incisors were demineralized by treatment with 0.6 N

Imaging of traumatic dislocation of the hip in childhood.

BACKGROUND: Traumatic hip dislocation in childhood is a rare consequence of violent trauma. After reduction, outcome is usually favourable although epiphyseal necrosis can occur. Reduction must be carried out as soon as possible and is achieved easily, although if the labrum is involved, surgery may be required to achieve complete reduction. OBJECTIVE: To analyze a retrospective series of traumatic hip dislocations in children, describing the therapeutic and imaging strategy. MATERIALS AND METHODS: A total of 42 patients were studied. Their mean age was 10 years 3 months. All relevant radiog

[Action of vitamin D on mineralization].

The effects of 1,25 (OH) (2)D on bone mineralization appear to be indirect by stimulating the calcium and phosphate supply, mainly by absorption from intestine. The disturbance in vitamin D action results in rickets in infancy and osteomalacia in adults. An association has been recently reported between mild vitamin D insufficiency and decreased bone mineral density in the elderly, which enhances the importance of appropriate vitamin D intake.

Premature vertebral endplate ossification and mild disc degeneration in mice after inactivation of one allele belonging to the Col2a1 gene for Type II collagen.

STUDY DESIGN: Skeletal tissues of mice with an inactivated allele of the Col2a1 gene for Type II collagen ("heterozygous knockout") were studied. OBJECTIVE: To determine whether a heterozygous inactivation of the Col2a1 gene has a role in the etiology of spine disorders such as disc degeneration. SUMMARY OF BACKGROUND DATA: Mutations in the COL2A1, COL11A1, COL11A2, and COL9A2 genes have been linked to spine disorders. However, the mechanism by which genetic factors lead to disc degeneration still are largely unknown. METHODS: Spine tissues were studied using radiograph analyses; convention

Omniflex modular femoral component. Two- to five-year results.

A prospective study was completed on 106 consecutive cementless arthroplasties (94 patients) to determine the clinical and radiographic outcome. Ninety-two hips (81 patients) had complete follow-up data. The average age was 46 years, and the average follow-up period was 41 months (24-63 months). Clinically 94% had no or slight pain. Two patients had activity-limiting thigh pain. Most patients had no or slight limp and walked without support. Patients had similar outcomes regardless of their bone type. Ninety-three percent of hips were radiographically stable. Six were loose; three of those had
