Oncocytic papillary cystadenoma with prominent mucinous differentiation of parotid gland: A case report.

We describe the case of an oncocytic papillary cystadenoma with mucinous differentiation of the parotid gland in a 64-year-old male. Histologically, the tumor exhibited distinctive areas of intracystic papillary growth pattern with microcystic and macrocystic spaces containing mucinous secretions and small crystals. The cyst wall and papillary fronds were lined by oncocytic admixed with numerous mucocytes. Lymphoid tissue and invasive features were not identified. The tumor showed strong expression of CK7 and mammaglobin in oncocytes, and BRST-2 and MUC4 in mucocytes. p63, ER, PR, SOX10, DOG-1

Management of head and neck melanoma: results of a national survey.

We recently reported on the safety of minimally invasive parotid region sentinel node biopsy and level I-sparing radical neck dissection for head and neck melanoma. We therefore wished to assess the state of practice in the United States through a survey of specialists in head and neck surgery. We hypothesized that there would be significant variation in the management of these facets of head and neck melanoma. To test this hypothesis, a 10-question online survey on management of head and neck melanoma was distributed to the members of the American Head and Neck Society. Responses were matched

Applied anatomy of the latissimus dorsi free flap for refinement in one-stage facial reanimation.

BACKGROUND: The face can be reanimated after long-term paralysis by free microneurovascular tissue transfer. Flaps from gracilis and pectoralis minor usually require a two-stage procedure with a cross-face nerve graft. Latissimus dorsi has a much longer muscular nerve, the thoracodorsal nerve, which could avoid the need for a second cross-face nerve graft. Our hypothesis is that the neurovascular pedicles of small segments of latissimus dorsi would be long enough to reach the opposite side of the face and to provide a reliable blood and nerve supply to the flaps. METHOD: To test this hypothes

[A case with parotid carcinoma showing a pathologically complete response after treatment with S-1 alone].

We report a 57-year-old male case of adenocarcinoma of the parotid gland with metastases to the cervical lymph nodes (T3N2bM0, Stage IV A). S-1 was administered at the conventional dose of 120mg/day for 4 weeks followed by a 2-week rest. After 4 courses of S-1, PET findings showed a remarkable regression of the tumor resulting in a partial response (PR) for the primary lesion. After this treatment, partial parotidectomy and upper neck dissection were performed. The histological effect was Grade 3.

Perforator flaps and supermicrosurgery.

The introduction of supermicrosurgery, which allows the anastomosis of smaller caliber vessels and microvascular dissection of vessels ranging from 0.3 to 0.8mm in diameter, has led to the development of new reconstructive techniques. New applications of this technique are for crushed fingertip replantations with venule grafts, toe tip transfers for fingertip loss, partial auricular transfers for total tracheal and eyelid defects, and lymphaticovenular anastomoses under local anesthesia for lymphedema. Regarding free flaps, free perforator-to-perforator flaps, including deep inferior epigastri

Bilateral mucosa-associated lymphoid tissue lymphomas of parotid glands: a 13-year interval.

Benign lymphoepithelial lesions of salivary gland may have a population of monoclonal B cells. There is controversy regarding the clinical significance of monoclonality in these lesions. Morphologically and clinically, benign lymphoepithelial lesions of the salivary gland with monoclonal B cells falls within the spectrum of low-grade B-cell lymphomas of mucosa-associated lymphoid tissue. We report a case of bilateral parotid lymphomas of mucosa-associated lymphoid tissue, separated diagnostically by a 13-year interval. Polymerase chain reaction analysis detected similar clones in the bilateral

Unexpected Detection of Parotid Gland Malignancy during Primary Extracapsular Dissection.

OBJECTIVE: To evaluate the oncologic and functional outcome in cases in which a false preoperative working hypothesis "benign parotid tumor" led to a primary extracapsular dissection being performed rather than a traditional, more radical surgical modality. STUDY DESIGN: Case series with chart review. SETTING: Academic tertiary referral center. SUBJECTS AND METHODS: The records of all patients treated for malignant tumors of the parotid gland between 2006 and 2012 were retrospectively studied. Patients were excluded from our study sample for insufficient data or if their histories indicated

Facial nerve reconstruction using vascularized nerve grafts based on the lateral femoral circumflex vascular system.

AIM: When unilateral facial nerve resection is performed for tumor resection, immediate reconstruction is desirable to achieve more natural recovery of facial expressions. We performed facial nerve reconstruction using vascularized nerve grafts based on the lateral femoral circumflex vascular system. Herein, we describe the details, advantages, and challenges associated with this technique. PATIENTS AND METHODS: We evaluated 9 patients who underwent reconstruction using a vascularized nerve graft harvested from the lateral femoral circumflex vascular system. All patients had at least 1 year o

Expressions of VEGF, Vasohibin-1, and Vasohibin-2 in Benign Parotid Gland Tumor.

The vasohibin (VASH) family includes vasohibin-1 (VASH1), whose expression is induced in vascular endothelial cells by angiogenic factors such as vascular endothelial growth factor (VEGF) to inhibit angiogenesis via a negative feedback loop, and vasohibin-2 (VASH2), a homolog of VASH1, which is mainly expressed in cancer cells and promotes angiogenesis. VASH proteins have been associated with tumor angiogenesis and prognosis in various tumor types, drawing attention to their role in tumor biology. To date, no studies have reported on the expression of VEGF, VASH1, and VASH2 in benign parotid g

An end-to-end deep convolutional neural network-based dose engine for parotid gland cancer seed implant brachytherapy.

BACKGROUND: Seed implant brachytherapy (SIBT) is a promising treatment modality for parotid gland cancers (PGCs). However, the current clinical standard dose calculation method based on the American Association of Physicists in Medicine (AAPM) Task Group 43 (TG-43) Report oversimplifies patient anatomy as a homogeneous water phantom medium, leading to significant dose calculation errors due to heterogeneity surrounding the parotid gland. Monte Carlo Simulation (MCS) can yield accurate dose distributions but the long computation time hinders its wide application in clinical practice. PURPOSE:

The Chimeric Scapulodorsal Vascularized Latissimus Dorsi Nerve Flap for Immediate Reconstruction of Total Parotidectomy Defects With Facial Nerve Sacrifice: Building a New Program and Preliminary Results From 25 Cases.

BACKGROUND: Total parotidectomy with facial nerve sacrifice creates 2 challenging reconstructive problems: restoration of facial contour and facial nerve rehabilitation. Strong evidence suggesting that vascularized nerve grafts are superior to nonvascularized nerve grafts motivated our team to develop a chimeric scapulodorsal flap combining the usual harvestable local tissues with the vascularized latissimus dorsi motor nerve (SD-LDVxN). We present our experiences developing a new program at University of California, San Diego, highlighting our first case here, and present preliminary retrospe

Implementation of an optimization method for parotid gland sparing during inverse planning for head and neck cancer radiotherapy.

PURPOSE: To guide parotid gland (PG) sparing at the dose planning step, a specific model based on overlap between PTV and organ at risk (Moore et al.) was developed and evaluated for VMAT in head-and-neck (H&N) cancer radiotherapy. MATERIALS AND METHODS: One hundred and sixty patients treated for locally advanced H&N cancer were included. A model optimization was first performed (20 patients) before a model evaluation (110 patients). Thirty cases were planned with and without the model to quantify the PG dose sparing. The inter-operator variability was evaluated on one case, planned by 12 ope

Ultrasound-guided percutaneous microwave ablation of parotid gland adenolymphoma: A case report.

RATIONALE: Parotid gland adenolymphoma is one of the most common benign tumors in parotid gland, and mainly treated by surgery. Despite the widespread of ultrasound-guided percutaneous microwave ablation, there is no report concerning its application in parotid gland adenolymphoma. Herein, we reported a 2-year follow-up result of a male patient underwent ultrasound-guided percutaneous microwave ablation for parotid gland adenolymphoma. PATIENT CONCERNS: A 73-year-old man was admitted due to a hypoechoic nodule measuring 2.67 × 1.42 × 1.35 cm in posterior-inferior area of parotid gland with a
