Esketamine combined with hydromorphone versus hydromorphone for post-operative patient-controlled intravenous analgesia in children: a randomized controlled trial.

BACKGROUND: Postoperative pain and opioid-related adverse effects remain significant concerns in pediatric surgical patients. Esketamine, an NMDA receptor antagonist, may enhance analgesia and reduce opioid consumption at low doses. This study aimed to evaluate the effects of low-dose esketamine combined with hydromorphone for PCIA on postoperative pain control, bowel recovery, and safety in children undergoing abdominal surgery. METHODS: This randomized controlled clinical trial enrolled children aged from 3 to 12 years, who were scheduled to undergo abdominal surgery under general anesthesi

A Systematic Review of How Cardiopulmonary Bypass Parameters Influence Electroencephalogram Signals.

BACKGROUND: Cardiopulmonary bypass (CPB) is an essential technique for cardiac surgery but significantly increases the risk of perioperative neurological complications. Electroencephalography (EEG) enables real-time monitoring of brain function and provides sensitive biomarkers for early detection of cerebral injury. However, a systematic synthesis of how CPB-related physiological, pharmacological, and technical factors influence EEG signals, and how these insights can be integrated into clinical decision-making, is still lacking. OBJECTIVE: To systematically review the effects of temperature

Impact of Delayed Inguinal Hernia Repair in Infants.

INTRODUCTION: Inguinal hernia repair is one of the most common pediatric surgical procedures performed with an estimated incidence of 1-5% in children. Patients can present with or without symptoms, with symptomatic hernias (incarcerated or strangulated) that may require emergent or urgent operative intervention. During the COVID-19 pandemic, restrictions, and shutdowns of elective surgeries at our institution resulted in delaying elective inguinal hernia repair unless presenting with more emergent symptoms, with no apparent increase in complication. In our institution, this practice was conti

Intramuscular Fentanyl and Ketorolac Associated with Superior Pain Control After Pediatric Bilateral Myringotomy and Tube Placement Surgery: A Retrospective Cohort Study.

BACKGROUND: Bilateral myringotomy and pressure equalization tube insertion (BMT) is the most common surgery in children. Multiple anesthetic techniques for BMT have been proposed, but that which reliably promotes ideal recovery remains unclear. We sought to assess associations between anesthetic regimens that included single-agent (fentanyl or ketorolac) or dual-agent (fentanyl and ketorolac) analgesic therapy and the primary outcome of maximal postanesthesia care unit (PACU) pain score. Secondary outcomes included in-hospital rescue analgesic administration, recovery time, and emesis incidenc

[New surgical strategy in gastroschisis: treatment simplification according to its physiopathology].

INTRODUCTION: Gastroschisis (GS) is a congenital abdominal wall defect that permits bowel exposure to amniotic fluid (AF). Intestinal damage is related to the chemical action of AF and constriction. After birth at term, a thickened intestinal wall with inflammation and, in some cases, intestinal atresias were observed. Surgical repair and intestinal reubication may be difficult, and thus staged silo repair could be necessary. These patients require a long hospital stay owing to bowel damage causing severe intestinal hypoperistalsis and poor absorptive capacity. Total parenteral nutrition (TPN)

Comparative Study Between Intravenous Patient-Controlled Analgesia Morphine and Computerized Ambulatory Delivery Device Epidural Morphine for Post Operative Analgesia for Nuss Procedure in Pectus Excavatum: A Retrospective Analysis.

BACKGROUND: Pectus deformities are commonly seen in chest wall deformities among the pediatric age group. Pectus deformities occur due to defective growth of the sternum and its surrounding cartilage. The Nuss procedure is the technique of choice for correcting the deformity surgically which includes placing a convex bar under the sternum without resection or injury to costal cartilages. Adequate pain control is utmost to improve wound healing, patient satisfaction, short hospital stays, and decrease the financial burden on attendants. Therefore, it is necessary to investigate which analgesic

The use of drugs in emergency airway management in pediatric trauma.

Most patients who require emergency airway control receive drugs to induce rapidly sufficient anesthesia for direct laryngoscopy and endotracheal intubation, but there are no protocols that outline the use of specific drugs in general use. Drugs should safely and rapidly produce (1) unconsciousness; (2) paralysis; and (3) blunt intracranial pressure (ICP) responses to airway procedures. Consequences to be considered include increased ICP, hemorrhagic shock, and a full stomach. To refine the use of drugs used for airway procedures in pediatric trauma patients, the authors reviewed all cases of

Upper body central venous catheters in pediatric cardiac surgery.

BACKGROUND: A central venous catheter located in the jugular or subclavian vein provides rapid, reliable vascular access for pediatric heart surgery. However, intravascular catheters are associated with vessel injury. Stenosis or thrombosis of central veins in the upper body can lead to 'superior vena cava syndrome' with markedly elevated venous pressures in the head and neck, causing facial swelling and headaches. This complication may be especially serious for patients with superior cavopulmonary (Glenn) or total cavopulmonary (Fontan) circulation. The authors hypothesized that upper body ce

Mullerian adenosarcoma of the cervix in a 10-year-old girl: case report and review of the literature.

UNLABELLED: Müllerian adenosarcoma is a rare neoplasm usually found in postmenopausal women. It usually presents as a polypoid mass within the endometrium. It is a biphasic tumor, composed of a benign epithelial component and a malignant stromal component. To date, this neoplasm has been reported in only 16 adolescent girls. We present a case of a 10-year-old girl who was diagnosed with müllerian adenosarcoma arising from the endocervix, the youngest female ever reported. CASE REPORT: A 10-year-old previously healthy girl presented to the Emergency Department at the Children's Hospital of Eas

Perioperative Pain Management Strategies and Postoperative Outcomes in Tonsillectomy Patients Aged Seven to 14 Years: A Single-Center Study.

Background Tonsillectomy is a common pediatric procedure requiring effective perioperative pain management to minimize complications and promote recovery. In resource-limited settings, standardized protocols are often challenged by infrastructural constraints and variable adherence to multimodal analgesia guidelines, risking suboptimal pain control and delayed discharge. Objective To evaluate a standardized acetaminophen-based pain management protocol and its impact on postoperative outcomes in children aged seven to 14 years undergoing tonsillectomy at the Lady Reading Hospital, a tertiary ca

Prediction of Placenta Accreta Spectrum by Prenatal Ultrasound Staging System in Women with Placenta Previa with Scarred Uterus.

INTRODUCTION: Sternotomy has been the most common approach for the correction of congenital cardiac defects, and questions arise surrounding the safety, feasibility, and cost-effectiveness of implementing minimally invasive techniques in congenital cardiac surgery, despite their global adoption over the past three decades. Nevertheless, the availability of published data from India on this subject remains limited. We aim to describe the techniques and outcomes of minimally invasive cardiac surgery in selected pediatric cases, a subject not previously detailed within the Indian population, and

Posterior urethral valves and the risk of neurodevelopmental disorders in two FINNISH cohorts.

BACKGROUND: The Posterior Urethral Valve (PUV) is a persistent membrane of the urethra, which causes obstruction in the urogenital tract in boys. To our knowledge, no comprehensive reports have been published on whether PUV is associated to neurodevelopmental disorders. Here, we analyzed a cohort of PUV patients for neurodevelopmental disorders and verified findings in an older cohort. METHODS: In a register based study, we reviewed the hospital registries for patients treated for PUV during 1992-2013 to identify those with neurodevelopmental disorders. Primary outcome measure was any neurode

Does the Utilization of Allograft Tissue in Medial Patellofemoral Ligament Reconstruction in Pediatric and Adolescent Patients Restore Patellar Stability?

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction is one of several surgical procedures used to treat patellofemoral instability. Use of allograft tissue can preserve autogenous tissue and may be preferable in patients with connective tissue disorders or ligamentous laxity. Although there are successful reports in adults, it is unclear if the use of allograft tissue in MPFL reconstruction can restore patellofemoral stability in children and adolescents. QUESTIONS/PURPOSES: (1) Does allograft tissue in MPFL reconstruction in pediatric and adolescent patients restore patellar sta
