What is the optimal timing for surgery in infective endocarditis with cerebrovascular complications?

Neurologic dysfunction complicates the course of 10-40% of left-side infective endocarditis (IE). In right-sided IE, instead, when systemic emboli occur, paradoxical embolism should be considered. The spectrum of neurologic events includes embolic cerebrovascular complication (CVC), intracranial haemorrhage, ruptured mycotic aneurysm, transient ischaemic attack (TIA), meningitis, encephalopathy and brain abscess. Cardiopulmonary bypass might exacerbate neurological deficits due to: heparinization and secondary cerebral haemorrhage; hypotension and cerebral oedema in areas of the disrupted bloo…

Central nervous system candidiasis beyond neonates: Lessons from a nationwide study.

UNLABELLED: Though candidiasis is the most frequent invasive fungal infection, Candida spp. central nervous system (CNS) infections are rare but severe. To further describe clinico-patho-radiological presentations of this entity, we report a retrospective study from January 2005 to December 2018 including patients aged ≥ 28 days with proven or probable CNS candidiasis in France. Twenty-four patients were included. Seventeen patients (70%) had CNS localization secondary to disseminated candidiasis (10 with hematologic malignancies [HM]; the seven other patients had infective endocarditis [IE]).

The Spectrum of CNS Infections in Children and Adolescents in Denmark 2017-2023: A Multicenter Population-based Cohort Study.

BACKGROUND: The epidemiology of pediatric central nervous system (CNS) infections has shifted over recent decades and remains incompletely described. We aimed to describe the incidence, etiology, age distribution and burden of pediatric CNS infections in a Danish population. METHODS: This population-based cohort study included all previously healthy Danish children 0-17 years of age in East Denmark from January 1, 2017, to December 31, 2023, with microbiologically verified CNS infections or cerebral abscess/empyema verified by imaging. RESULTS: Totally 465 patients were included. The total a

Multiple nocardial abscesses of cerebrum, cerebellum and spinal cord, causing quadriplegia.

In this paper we present a case of a diabetic patient with nocardial abscesses of cerebrum, cerebellum and the spinal cord. The present case is the first case in the literature of solitary intramedullary abscess in cervical spinal cord, causing tetraplegia. Nocardia asteroides grew in a culture of the abscess pus. After either surgical excision or drainage of lesions, a triple combination regimen of chemotherapy (amikacin, ceftriaxone and trimethoprim-sulfamethoxazole) was given, but the patient was lost in the postoperative period. This case gives suggestive evidence of an association between

[Cerebral abscess and cyanotic congenital heart disease].

Cerebral abscess is a classical complication of cyanotic congenital heart disease. The authors report 7 cases of cerebral abscess diagnosed since 1982. One asymptomatic patient died of a postoperative cerebral haemorrage. The child was repatriated from Africa for complete correction of his cardiac lesion. The presentation of the other 6 cases was quite typical : headaches, pyrexia and vomiting with a neurological deficit in 4 cases : two hemiparesias and two homonymous lateral hemianopsia. These 6 patients recovered without sequeilae. Four underwent surgical drainage of the abscess with antibi

Complications of chronic otitis media.

In summary, recent literature indicates that the complications of chronic otitis media have been decreasing. However, even with the advent of modern and more powerful antimicrobials and aggressive surgical eradication of disease, the morbidity and mortality are still high. Some complications may initially be quite obvious and some complications may be quite subtle. Therefore, the most important tools in making early diagnosis are careful history and physical examination, and a high index of suspicion for impending complications.

Intracranial infection in children.

This article discusses the neuroradiologic characteristics of central nervous system infections in children. These disorders are divided into two groups: those of congenital infections and those of infections in the infant and child. Congenital infections are referred to by the acronym TORCH. Inflammatory diseases in the infant and child encompass meningitis, cerebritis, and encephalitis. The heterogeneous etiologies often can be suggested by the neuroradiologic findings, as demonstrated with computed tomography and magnetic resonance imaging. This article also addresses the sequelae of destru

[Nocardiosis: clinical observations apropos of 9 cases].

We have retrospectively analyzed 9 cases of nocardiosis that were studied in our hospital over the past 10 years. 66% of the patients had an associated disease and half of them were on steroid treatment. The clinical manifestations of the infection were pulmonary and cutaneous and in 44% of cases, it disseminated to the central nervous system. The diagnosis was made in all cases isolating the microorganism from pulmonary secretions, puss, spinal fluid and blood. Nocardia asteroides was isolated in 8 cases, and nocardia caviae in one case. There was a 55% mortality rate. The different clinical
