Assessing the influence of maternal vitamin D deficiency in early pregnancy and subsequent improvement on perinatal outcomes and long-term child development: a retrospective cohort study.

This study examined perinatal and long-term outcomes where maternal vitamin D deficiency was present in early pregnancy but normalized in mid-pregnancy and deficiency during both early and mid-pregnancy. In this retrospective study, we reviewed the clinical records of 5,169 singleton pregnant women who received a test of serum 25-hydroxy-vitamin D [25(OH)D] two times in the first and second trimester in our hospital from 2016 to 2022. The level of 25(OH)D was categorized into deficiency (<10ng/mL), insufficiency (10-20ng/mL), and sufficiency (≥20ng/mL). Women were grouped based on 25(OH)D stat

Comparative Efficacy of Ventriculosubgaleal Shunt Versus Ventricular Reservoir in Achieving Shunt Independence for Infants With Posthemorrhagic Hydrocephalus by 6 Months.

BACKGROUND AND OBJECTIVES: Low-birth weight, premature infants often have severe intraventricular hemorrhage (IVH), which can result in posthemorrhagic hydrocephalus (PHH), sometimes requiring cerebrospinal fluid diversion. Initial temporizing management of PHH includes placement of a ventriculosubgaleal shunt (VSGS) or ventricular access device (VAD). Studies have found similar permanent shunt conversion rates between VSGS and VAD but were limited by sample scope and size. The rate of conversion to permanent shunt within 6 months post-IVH in premature infants, comparing the efficacy of VSGS a

Standard versus restricted fluid administration in preterm infants undergoing pharmacological treatment for haemodynamically significant patent ductus arteriosus.

BACKGROUND: Fluid management in patent ductus arteriosus (PDA) treatment is debated, with fluid restriction traditionally used to promote closure, though its impact on outcomes is uncertain. METHODS: This retro-prospective cohort single-center study compared two cohorts of preterm neonates receiving pharmacological treatment for hemodynamically significant PDA (hsPDA). The 2021-2023 cohort followed an updated, higher fluid intake protocol based on neonatal weight and day of life (80-100 ml/kg/day for extreme preterm <1000 g on day 1), while the 2018-2020 cohort adhered to a more restrictive a

Outbreak of Vancomycin-Resistant Enterococcus in a NICU: Insights into Molecular Detection and Infection Control.

Vancomycin-resistant enterococci (VRE) are a major cause of healthcare-associated infections (HAIs). However, the clinical significance of VRE colonization and the subsequent risk of VRE infection in hospitalized patients are not fully established. Prolonged hospital stays have been observed in neonates colonized by VRE. The mortality rate in pediatric patients with VRE infections ranges from 0% to 42% in both endemic and outbreak settings, often occurring in VRE-colonized neonates. Host and bacterial factors associated with a worse outcome are not fully understood yet. We describe an outbreak

Prophylactic antibiotics to prevent postcesarean infection: which antimicrobial, when, how, and why?

Before the introduction of routine antibiotic administration, the rate of postcesarean infection exceeded 30% to 50%. Since the 1970s, postpartum infection occurs in approximately 1% to 2% of patients after vaginal birth and in 5% to 20% following cesarean delivery. Evidence from randomized clinical trials and systematic reviews has demonstrated that the administration of a single dose of antibiotics within 60 minutes before skin incision significantly reduces maternal infection-related morbidity without adverse neonatal outcomes. These results have informed clinical guidelines of professional

Refining microbiome resolution: KoSCAPEbio identifies Klebsiella oxytoca complex species in16S rRNA amplicon data and uncovers associations with NEC.

Klebsiella are early colonizers and commensals of human hosts but are also opportunistic pathogens. Research has focused on Klebsiella pneumoniae due to its global contribution to nosocomial infection and antibiotic resistance. Yet Klebsiella oxytoca and several closely related species are increasingly recognized as clinically significant bacteria underlying enteric and respiratory disorders. Inadequate interspecies discrimination inhibits advancement of our understanding of these diseases including necrotizing enterocolitis (NEC) in infants. Here we leverage the taxonomic potential of the amp
