Eagle Syndrome: 3D endoscope-assisted anterior tonsillar fossa approach to styloid process.

BACKGROUND: The aim of this study was to describe the potential advantages of the 3D endoscope-assisted anterior tonsillar fossa approach to elongated styloid process. METHODS: A 58 years-old woman was reported to our Department with one-year history of odynophagia, latero-cervical pain on the left side, and ipsilateral foreign body sensation. Pain was dull and intermittent in nature. The patient had visited different clinics, she had a physiatric and maxillo-facial evaluation with two dental extraction without any relief of the symptoms. On physical examination a hard-bony consistency area w

Repair and augmentation of the lateral collateral ligament complex using internal bracing in dislocations and fracture dislocations of the elbow restores stability and allows early rehabilitation.

PURPOSE: Most elbow dislocations can be treated conservatively, with surgery indicated in special circumstances. Surgical options, apart from fracture fixation, range from repair or reconstruction of the damaged ligaments to static external fixation, usually entailing either a long period of immobilization followed by carefully monitored initiation of movement or dynamic external fixation. In general, no consensus regarding surgical treatment has been reached. A new method of open ligament repair and augmentation of the lateral ulnar collateral ligament using a non-absorbable suture tape in ca

Calcium activated nucleotidase 1 (CANT1) is critical for glycosaminoglycan biosynthesis in cartilage and endochondral ossification.

Desbuquois dysplasia type 1 (DBQD1) is a chondrodysplasia caused by mutations in CANT1 gene encoding an ER/Golgi calcium activated nucleotidase 1 that hydrolyses UDP. Here, using Cant1 knock-in and knock-out mice recapitulating DBQD1 phenotype, we report that CANT1 plays a crucial role in cartilage proteoglycan synthesis and in endochondral ossification. Specifically, the glycosaminoglycan synthesis was decreased in chondrocytes from Cant1 knock-out mice and their hydrodynamic size was reduced, whilst the sulfation was increased and the overall proteoglycan secretion was delayed. Interestingly

The frequency and treatment of dural tears and cerebrospinal fluid leakage in 266 patients with thoracic myelopathy caused by ossification of the ligamentum flavum.

STUDY DESIGN: Retrospective review. OBJECTIVE: To perform a single-institution analysis of incidence, treatment, and clinical outcome in patients with thoracic ossification of the ligamentum flavum (OLF) who experienced dural tears and cerebrospinal fluid (CSF) leakage. SUMMARY OF BACKGROUND DATA: There is a paucity of clinical reports focusing on dural tears and CSF leakage after thoracic OLF surgery. Because dural adhesion and dural ossification are common features of thoracic OLF, the incidence of CSF leakage in OLF patients is high and represents a significant clinical challenge. METHOD

Juvenile osteochondritis dissecans.

Juvenile osteochondritis dissecans (JOCD) has been a recognized entity for more than 100 years. Despite our long recognition of OCD, the natural history and most effective therapies are poorly understood. Although conclusive evidence of an exact cause is lacking, there is widespread agreement that JOCD is related to repetitive trauma. Patients with JOCD present with vague pain and occasionally, mechanical symptoms. The diagnosis of JOCD can be confirmed on plain radiographs. Magnetic resonance imaging has emerged as the study of choice to evaluate the stability of the lesion and integrity of t

Sagittal synostosis and ossified scalp hematoma: cause or consequence?

In this paper, the author presents 2 cases of sagittal synostosis with scaphocephaly that featured ossified scalp hematomas straddling the sagittal suture in the midparietal region. These ossified lesions were originally cephalohematomas. Collection of blood under the pericranium across the midline was possible in these cases because sagittal synostosis had obliterated the sagittal suture and its dense attachment to overlying periosteum. Scaphocephaly very likely exacerbated the difficulty of the deliveries and contributed to the causation of the scalp hemorrhages. The alternative hypothesis,

Delayed bone age due to a dual effect of FGFR3 mutation in Achondroplasia.

Achondroplasia (ACH), the most common form of human dwarfism is caused by a mutation in the Fibroblast Growth Factor Receptor 3 (FGFR3) gene, resulting in constitutive activation of the receptor. Typical radiological features include shortening of the tubular bones and macrocephaly, due to disruption of endochondral ossification. Consequently, FGFR3 has been described as a negative regulator of bone growth. Studying a large cohort of ACH patients, a delay in bone age was observed shortly after birth (for boys p=2.6×10(-9) and for girls p=1.2×10(-8)). This delay was no longer apparent during ad

Heterotopic bone formation: presentation of an experimental rat model and a clinical case.

Heterotopic bone formation (HBF), an ill-defined phenomenon, refers to the formation of bone in tissue that normally does not ossify. Two existing theories to explain HBF are that two cellular entities, one from the bone and the other from the muscle or fascia (two cell types) are involved, and that stem cell responds to a factor induced by trauma (one cell + factor). In our report, the HBF in a patient's vertical abdominal wound was possibly due to IEL, a stem cell, which is stimulated by a factor from xiphoid when it is traumatized by surgical incision. After 28 days' culture rat tissue spec

Anterior cervical vertebrectomy and interbody fusion for multi-level spondylosis and ossification of the posterior longitudinal ligament.

Multi-level cervical spondylosis and ossification of the posterior longitudinal ligament (OPLL) are well-documented causes of myelopathy. The choice of surgical procedures remain controversial. Between January 1983 and December 1987, we have performed anterior cervical vertebrectomy in 45 patients with cervical myelopathy caused by multi-level spondylosis and OPLL. They consisted of 19 patients with cervical spondylosis, 12 with OPLL, and 14 with combined lesions of both cervical spondylosis and OPLL. There were 32 men and 13 women. The mean age was 55 years, ranging from 35 to 70 years. In al

Surgical treatment for posterior distal humeral shear fractures.

BACKGROUND: Posterior shear fractures of the distal humerus are rare and have a complex, controversial injury mechanism. This study aims to investigate the morphological characteristics, injury mechanisms, and treatment outcomes of these fractures. METHODS: From March 2021 to October 2024, 13 patients with posterior shear fractures of the distal humerus were treated with open reduction and internal fixation. Among them, 7 were male and 6 were female, with a mean age of 40 years (range: 25-66 years). Five cases involved the left side, and 8 involved the right side. All fractures were closed. M

Comparative outcomes of hip arthroscopy-assisted surgery and the direct anterior approach for Pipkin Type I and II femoral head fractures.

INTRODUCTION: Comparative evidence regarding the direct anterior approach and hip arthroscopy-assisted surgery for Pipkin type I and II fractures remains limited. This study compares these minimally invasive strategies and evaluates whether fragment excision or internal fixation yields superior functional outcomes. MATERIALS AND METHODS: Thirty-nine patients with displaced Pipkin type I or II femoral head fractures treated between April 2010 and June 2023 were retrospectively analyzed, with a minimum follow-up of 2 years. Twenty-five underwent hip arthroscopy-assisted surgery and 14 underwent

Loose Fit Monobloc Versus Cemented Bipolar Radial Head Prosthesis in the Treatment of Radial Head Fractures: A Comparative Study.

Background: Radial head arthroplasty is commonly used for unreconstructable radial head fractures, particularly in cases associated with elbow instability. Both loose-fit monobloc and cemented bipolar radial head prostheses are available, but comparative evidence regarding their clinical and radiographic outcomes remains limited. This study aimed to compare mid-term outcomes between these two implant designs. Methods: A retrospective observational study was conducted on 26 patients who underwent radial head arthroplasty for acute radial head fractures between November 2020 and October 2023. Fo

The coronoid as the key fragment of trans-ulnar fracture-dislocations of the elbow: Insights from a retrospective cohort comparison using the coronoid-centric Mayo classification system.

BACKGROUND: Trans-ulnar fracture-dislocations of the elbow are rare injuries with complex fracture patterns and variable outcomes. Traditional classification systems offer limited prognostic value. A recently introduced coronoid-centric Mayo classification distinguishes injury subtypes based on coronoid attachment and identifies trans-ulnar basal coronoid (TUBC) fractures as a particularly challenging entity. This study evaluated outcomes across Mayo fracture types and explored factors associated with inferior results in TUBC injuries. MATERIALS AND METHODS: In this retrospective cohort study

Outcomes of Multiple Drilling for Osteonecrosis of the Femoral Head: A Retrospective Cohort Study With a Minimum 10-Year Follow-Up.

BACKGROUND: Multiple drilling techniques, a minimally invasive variation of core decompression, have shown promise for joint preservation in early-stage osteonecrosis of the femoral head (ONFH). However, long-term outcomes remain insufficiently reported. This study extends the follow-up period to 30 years, aiming to deliver a thorough evaluation of the sustained effectiveness and potential limitations of multiple drilling in the management of ONFH. METHODS: This retrospective analysis reviewed the original cohort from previous research, comprising patients who underwent multiple drilling proc
