Change of Cerebral Oxygenation during Surfactant Treatment in Preterm Infants: "LISA" versus "InSurE" Procedures.

Objectives The aim of the study was to compare the effects on cerebral oxygenation in preterm infants of two different procedures for surfactant administration: the LISA (low-invasive method of surfactant administration) and the InSurE (Intubation, SURfactant administration, Extubation). Study Design Twenty premature infants with respiratory distress syndrome were assigned to receive surfactant either by "LISA" (n = 10) or "InSurE" (n = 10) procedure. Patients were continuously studied by near-infrared spectroscopy (NIRS) for the measurement of cerebral regional oxygenation (rSO2C) and calcula

Recombination, Solvation and Reaction of CN Radicals Following Ultraviolet Photolysis of ICN in Organic Solvents.

The fates of CN radicals produced by ultraviolet (UV) photolysis of ICN in various organic solvents have been examined by transient electronic and vibrational absorption spectroscopy (TEAS and TVAS). Near-UV and visible bands in the TEAS measurement enable direct observation of the CN radicals and their complexes with the solvent molecules. Complementary TVAS measurements probe the products of CN-radical reactions. Geminate recombination to form ICN and INC is a minor pathway on the 150 fs -1300 ps time scales of our experiments in the chosen organic solvents; nonetheless, large infrared trans

Implantation of a left ventricular assist device as a destination therapy in Duchenne muscular dystrophy patients with end stage cardiac failure: management and lessons learned.

Duchenne muscular dystrophy (DMD) is an X-linked recessive disorder, characterized by progressive skeletal muscle weakness, loss of ambulation, and death secondary to cardiac or respiratory failure. End-stage dilated cardiomyopathy (DCM) is a frequent finding in DMD patients, they are rarely candidates for cardiac transplantation. Recently, the use of ventricular assist devices as a destination therapy (DT) as an alternative to cardiac transplantation in DMD patients has been described. Preoperative planning and patient selection play a significant role in the successful postoperative course o

Cerebral oxygenation during different treatment strategies for a patent ductus arteriosus.

BACKGROUND: Preterm infants with a hemodynamically significant patent ductus arteriosus (hsPDA) are at risk for fluctuations in cerebral blood flow, but it is unclear how different hsPDA treatment strategies may affect cerebral oxygenation. OBJECTIVE: To compare regional cerebral oxygen saturation (rSO(2)) as measured by near-infrared spectroscopy (NIRS) in very low birth weight (VLBW) infants with a hsPDA treated with conservative management, indomethacin, or surgical ligation. METHODS: This prospective observational study enrolled 33 VLBW infants with a hsPDA diagnosed by echocardiogram an

A broadband continuous-wave multichannel near-infrared system for measuring regional cerebral blood flow and oxygen consumption in newborn piglets.

Near-infrared spectroscopy (NIRS) is a promising technique for assessing brain function in newborns, particularly due to its portability and sensitivity to cerebral hemodynamics and oxygenation. Methods for measuring cerebral blood flow (CBF) and cerebral metabolic rate of oxygen (CMRO(2)) have been developed based on broadband continuous-wave NIRS. However, broadband NIRS apparatus typically have only one detection channel, which limits their applicability to measuring regional CBF and CMRO(2). In this study, a relatively simple multiplexing approach based on electronically controlled mechani

Microcirculatory changes in right lobe grafts in living-donor liver transplantation: a near-infrared spectrometry study.

BACKGROUND: A continuing shortage of cadaveric liver even for adult patients has motivated not a few centers to proceed to living-donor liver transplantation using right lobe grafts. One of controversies is potential congestion in the graft anterior segment by the deprivation of the middle hepatic vein. METHODS: Hepatic tissue oxygenation and hemoglobin concentration were investigated with a near-infrared spectroscopy in the course of harvesting and implantation in living-donor liver transplantation. Twenty adult recipients of right lobe graft were involved in the study. The aim of the analys

Hearing Aids Reshape Neural Processing of Emotional Speech Without Improving Emotion Perception.

Hearing aids have improved speech intelligibility but not speech emotion perception in older adults with hearing loss. Hearing loss has also been linked to altered brain responses to emotional non-speech sounds, potentially contributing to the speech emotion perception deficits even under aided listening. While fMRI requires hearing aid removal due to metal incompatibility, fNIRS is silent and hearing-aid compatible, creating a novel opportunity to identify neural processes during aided listening. We leveraged fNIRS to examine how hearing aids affect brain function in experienced hearing-aid u

A high-selectivity fluorescent probe for hypoxia imaging in cells and a tumor-bearing mouse model.

Nitroreductase (NTR) with a high expression level in tumors has been considered as a biomarker of highly aggressive hypoxia tumors. Thus, it is important to develop powerful tools for tumor hypoxia detection. Here, we developed a two-photon fluorescent probe hTP-NNO2 for NTR detection. The probe with one-step synthesis exhibited high yield. hTP-NNO2 showed high selectivity and sensitivity for NTR and the detection limit was as low as 43 ng mL-1. hTP-NNO2 also showed low cytotoxicity and high stability, indicating that hTP-NNO2 is suitable for NTR detection in real-time and in situ under physio

Comparing executive function, evoked hemodynamic response, and gait as predictors of variations in mobility for older adults.

OBJECTIVE: Falls represent a major concern for older adults and may serve as clinically salient index events for those presenting in the prodromal stages of mild cognitive impairment. Declines in executive function performance and in gait consistency have shown promise in predicting fall risk; however, associated neurophysiological underpinnings have received less attention. In this study, we used a multimodal approach to assess fall risk in a group of older adults with and without a previous fall history. METHOD: Processing speed, inductive reasoning, verbal fluency, crystallized ability, ep

Layer-by-layer assembled fluorescent probes in the second near-infrared window for systemic delivery and detection of ovarian cancer.

Fluorescence imaging in the second near-infrared window (NIR-II, 1,000-1,700 nm) features deep tissue penetration, reduced tissue scattering, and diminishing tissue autofluorescence. Here, NIR-II fluorescent probes, including down-conversion nanoparticles, quantum dots, single-walled carbon nanotubes, and organic dyes, are constructed into biocompatible nanoparticles using the layer-by-layer (LbL) platform due to its modular and versatile nature. The LbL platform has previously been demonstrated to enable incorporation of diagnostic agents, drugs, and nucleic acids such as siRNA while providin

Comparing Near-Infrared Imaging with Indocyanine Green to Conventional Imaging During Laparoscopic Cholecystectomy: A Prospective Crossover Study.

BACKGROUND: The aim of this study was to test and validate a novel noninvasive method for intraoperative visualization of extrahepatic bile ducts during laparoscopic cholecystectomy. Injury to the common bile duct (CBD) is a rare but major complication of laparoscopic cholecystectomy. Most injuries occur when anatomy is unclear due to the presence of anatomic variations, acute inflammation, or adhesions. PATIENTS AND METHODS: Thirty patients were included, and each received an intravenous injection of 0.05 mg/kg of indocyanine green (ICG) (ICG-Pulsion(®); PULSION Medical Systems AG, Munich, G

Label-free intraoperative parathyroid localization with near-infrared autofluorescence imaging.

CONTEXT: The inability to accurately localize the parathyroid glands during parathyroidectomy and thyroidectomy procedures can prevent patients from achieving postoperative normocalcemia. There is a critical need for an improved intraoperative method for real-time parathyroid identification. OBJECTIVE: The objective of the study was to test the accuracy of a real-time, label-free technique that uses near-infrared (NIR) autofluorescence imaging to localize the parathyroid. SETTING: The study was conducted at the Vanderbilt University endocrine surgery center. SUBJECTS AND METHODS: Patients u

A multimodal approach to investigate biomarkers for psychosis in a clinical setting: the integrative neuroimaging studies in schizophrenia targeting for early intervention and prevention (IN-STEP) project.

Longitudinal clinical investigations and biological measurements have determined not only progressive brain volumetric and functional changes especially around the onset of psychosis but also the abnormality of developmental pathways based on gene-environment interaction model. However, these studies have contributed little to clinical decisions on their diagnosis and therapeutic choices because of subtle differences between patients and healthy controls. A multi-modal approach may resolve this limitation and is favorable to explore the pathophysiology of psychosis. The integrative neuroimagin

Can we improve the outcomes of pediatric congenital heart disease survivors?

Appreciating the complexity and evolving pathophysiology after palliation of CHD is critical to improving the outcome. Despite a growing patient population because of greater survival over the years, detailed data on the long-term outcomes of these patients is surprisingly sparse. The establishment in 2001 of the Pediatric Heart Network by the National Heart, Lung and Blood Institute has provided a collaborative multicenter platform to conduct large trials in CHD and to characterize outcomes. This chapter has emphasized how CHD care spans the continuum from fetal life to adulthood. Current res

Cerebral intravascular oxygenation correlates with mean arterial pressure in critically ill premature infants.

OBJECTIVES: Premature infants experience brain injury, ie, germinal matrix-intraventricular hemorrhage (GMH-IVH) and periventricular leukomalacia (PVL), in considerable part because of disturbances in cerebral blood flow (CBF). Because such infants are susceptible to major fluctuations in mean arterial blood pressure (MAP), impaired cerebrovascular autoregulation would increase the likelihood for the changes in CBF that could result in GMH-IVH and PVL. The objectives of this study were to determine whether a state of impaired cerebrovascular autoregulation could be identified reliably and conv

A novel infrared fluorescence method to identify regions of superficial microvenous reflux in patients with chronic venous disease.

OBJECTIVE: Reflux within the superficial microvenous network may play a critical role in the development of skin changes associated with chronic venous insufficiency. This study aimed to extend previous ex vivo observations to determine the in vivo utility of near infrared fluorescence (NIRF) imaging to assess superficial venous reflux in the leg. METHODS: A total of 28 limbs were examined in 17 participants. These included limbs with (CEAP C2, n = 6; C3, n = 1; and C4, n = 15) and without (CEAP C0, n = 6) venous disease. Non-invasive quantitative assessment of kidney injury using near-infrared autofluorescence imaging.

INTRODUCTION: Tubulointerstitial fibrosis represents the final common pathway to kidney failure, with tubular epithelial cell (TEC) injury playing a critical role. Current methods for assessing TEC injury rely on invasive biopsies or lack temporal resolution, limiting real-time monitoring of disease progression and therapeutic responses. METHODS: Near-infrared (NIR) autofluorescence was evaluated for non-invasive TEC injury assessment in three mouse models: unilateral ureteral obstruction (UUO; consistent tubulointerstitial injury), folic acid nephropathy (variable injury severity), and unila

Cerebral monitoring responses to bedside physiological challenges in comatose post-cardiac arrest patients.

To evaluate the effects of three simple bedside challenges on cerebral oxygenation and brain activity, measured non-invasively using near-infrared spectroscopy (NIRS) and frontal single-channel electroencephalography (EEG), in comatose post-cardiac arrest patients, and to examine whether these responses differ according to cerebral autoregulation status and intensive care unit (ICU) outcome and could aid early prognostication. Three bedside physiological challenges were conducted: (1) increasing the fraction of inspired oxygen (FiO₂) to 100%, (2) lowering the head-of-bed (HOB) to 0°, and (3) e
