Development of The COMBINED approach: Integrating a brief behaviour change intervention supported by a physiotherapist for people with rotator cuff disorders.

BACKGROUND: Modifiable lifestyle factors (smoking, inactivity, overweight) could contribute to the onset and persistence of rotator cuff disorders, yet physiotherapy practice rarely addresses them. Brief interventions (BIs) can support behaviour change but are not routinely embedded in musculoskeletal care. OBJECTIVES: To develop The COMBINED approach: a physiotherapist-supported intervention integrating BI's targeting modifiable health behaviours into routine consultations for people with rotator cuff disorders; and to explore how physiotherapists can be supported to deliver it. MATERIALS A

Family history and heritability of rotator cuff tears.

BACKGROUND: Prior studies suggest familial clustering of rotator cuff tears (RCT) but are limited by sample size or by approach, and no study has estimated narrow-sense heritability of RCT. We perform a comprehensive assessment of the familial heritability of RCT. METHODS: We used data from 3 studies: the cuffGen study, Vanderbilt Health Biorepository (BioVU), and UK Biobank (UKB). In cuffGen, imaging confirmed RCT cases and controls completed a baseline questionnaire inquiring about their family history of RCT. In BioVU and UKB, RCT status was identified using electronic health record data,

Development and validation of a risk prediction model for rotator cuff tears following acute anterior shoulder dislocation.

PURPOSE: Following shoulder dislocation, injuries such as rotator cuff tear (RCT), axillary nerve injury, greater tuberosity (GT) fracture, Hill-Sachs lesion, and Bankart lesion frequently occur. This study aimed to develop and validate a clinical prediction model for the occurrence of RCT after acute anterior shoulder dislocation. METHODS: This study included all patients with acute anterior shoulder dislocation who presented to the emergency orthopedics department of the same institution from July 2021 to March 2025. Lasso regression was used to select independent variables for the model. A

Is biceps augmentation effective for scope-assisted lower trapezius tendon transfer in posterosuperior irreparable rotator cuff tears? A retrospective short-term clinical comparison.

INTRODUCTION: Although favorable clinical outcomes of scope-assisted lower trapezius tendon transfer (SALTT) have been reported in posterosuperior irreparable rotator cuff tears (PSIRCTs) patients, the limited restoring of static stability can be supported by biceps augmentation as a static stabilizer. This study aimed to compare the outcomes of scope-assisted lower trapezius tendon transfer (SALTT) with and without biceps augmentation in patients with posterosuperior irreparable rotator cuff tears (PSIRCTs). MATERIAL AND METHODS: This retrospective clinical comparative study was performed wi

Low Incidence of Adverse Events of a Novel Self-Tensioning No. 2 Round Suture in Rotator Cuff Repair: An IDEAL Stage 2a Registry Cohort Analysis.

BACKGROUND: Despite technical and material improvements in rotator cuff repair, clinical and radiological failure remains common. Following suture fixation, tension and footprint compression decrease from time zero. A novel suture has been designed to shorten when submerged in liquid to maintain tension and increase repair construct security. The aim of this study was to assess the safety and clinical outcomes (IDEAL 2a assessment) in patients receiving rotator cuff repair with the self-tensioning suture with a minimum of 12 months follow up. Clinical registries allow early identification of o

Loosening the Lid on Shoulder Osteoarthritis: How the Transcriptome and Metabolic Syndrome Correlate with End-Stage Disease.

Metabolic syndrome (MetS) associated with Osteoarthritis (OA) is an increasingly recognised entity. Whilst the degenerative pattern in cuff-tear arthropathy (CTA) has been well documented, the biological processes behind primary shoulder OA and CTA remain less understood. This study investigates transcriptomic differences in these conditions, alongside the impact of MetS in patients undergoing total shoulder replacement. In a multi-centre study, 20 OA patients undergoing total shoulder replacement were included based on specific treatment indications for OA and cuff-tear arthropathy as well as

The effect of tranexamic acid and epinephrine on visual clarity during arthroscopic shoulder surgery: A meta-analysis of RCTs.

INTRODUCTION: Visual clarity in arthroscopic rotator cuff repair is essential to reduce the operative time and for efficiency of repair. Tranexamic acid (TXA) in open shoulder surgery has been shown to reduce blood loss but its use in shoulder arthroscopy for rotator cuff repair for improved clarity is not understood. The purpose of this SR is to determine the effect of TXA and epinephrine on visual clarity in shoulder arthroscopy for rotator cuff repair. HYPOTHESIS: We hypothesise that visual clarity should improve in those that have TXA compared to those who do not receive TXA. METHODS: A

Shearing Osteochondral Fracture of the Humeral Head: A Case Report.

CASE: A 30-year-old man was found to have a posteriorly displaced osteochondral shear fracture of his left humeral head, likely from dislocation and relocation. The fragment was fixed in its native position using headless screws, and the patient regained full strength and range of motion within 9 months. CONCLUSIONS: Osteochondral shear fractures of the humeral head appear to shear off the anteromedial articular joint surface after dislocation and relocation. One should be suspicious for associated labral tears. Arthroplasty is a valid treatment option, but select patients may be eligible for
