Effect of Permissive Mild Hypercapnia on Cerebral Vasoreactivity in Infants: A Randomized Controlled Crossover Trial.

BACKGROUND: Mechanical ventilation interferes with cerebral perfusion via changes in intrathoracic pressure and/or as a consequence of alterations in CO2. Cerebral vascular vasoreactivity is dependent on CO2, and hypocapnia can potentially lead to vasoconstriction and subsequent decrease in cerebral blood flow. Thus, we aimed at characterizing whether protective ventilation with mild permissive hypercapnia improves cerebral perfusion in infants. METHODS: Following ethical approval and parental consent, 19 infants were included in this crossover study and randomly assigned to 2 groups for whic

Low Cerebral Oxygenation Is Associated with Cognitive Impairment in Chronic Hemodialysis Patients.

BACKGROUND/AIMS: High rates of cognitive impairment (CI) are an alarming problem in patients undergoing chronic hemodialysis (HD). Its pathophysiology remains unclear and there are indications that brain ischemia might be one of the key causes. Cerebral tissue oxygenation, as measured by near-infrared spectroscopy, is known to be decreased in HD patients. However, it is unknown whether CI is associated or not associated with lower cerebral oxygenation in these patients. The primary aim of our study was to probe this possible association. Our secondary aim was to assess other factors possibly r

Determining the venous oxygen reservoir: A novel, hypothetical approach to titration of supplemental oxygen in preterm newborns.

While normal oxygen saturation is commonly thought to be a marker of normal oxygenation, cutaneous saturation does not account for the sufficiency of oxygen within each cell or that of the system overall. Rather, cutaneous oximetry simply defines the saturation of haemoglobin (Hb) with oxygen in a pulsatile vessel. Assessment of sufficiency is best determined by measurement of the amount of oxygen left over following aerobic respiration. This left over oxygen is 'stored' on Hb in the venous compartment and can be calculated as the venous oxygen content. We hypothesize that the development of a

Deoxygenation of inspiratory muscles during cycling, hyperpnoea and loaded breathing in health and disease: a systematic review.

Assessing inspiratory muscle deoxygenation and blood flow can provide insight into anaerobic stress, recruitment strategies and mechanisms of inspiratory muscle limitation. Therefore, this review aimed to synthesize measurements of inspiratory muscle oxyhaemoglobin (O2 Hb), deoxyhaemoglobin (HHb), blood volume and flow of the inspiratory muscles acquired via near-infrared spectroscopy (NIRS) during cycling, hyperpnoea and loaded breathing in healthy non-athletes, healthy athletes and patients with chronic obstructive pulmonary disease (COPD) or chronic heart failure (CHF). Changes in cerebral oxygen saturation and blood flow during hypoxic gas ventilation therapy in HLHS and CoA/IAA complex with markedly increased pulmonary blood flow.

BACKGROUND: Hypoxic gas ventilation therapy has recently been performed to prevent post-birth increased pulmonary blood flow in cases of congenital heart diseases with increased pulmonary blood flow. However, how the oxygen supply to the tissues changes during breathing a hypoxic gas mixture, remains unknown. The changes in cerebral oxygen saturation and blood supply during hypoxic gas ventilation therapy using a nitrogen gas mixture were studied. METHODS AND RESULTS: Cerebral regional oxygen saturation (cerebral rSO(2)) was measured by near-infrared spectroscopy, and changes in middle cerebr

Association of serum prealbumin and its changes over time with clinical outcomes and survival in patients receiving hemodialysis.

BACKGROUND: In patients receiving maintenance hemodialysis (MHD), a low serum prealbumin is an indicator of protein-energy wasting. OBJECTIVE: We hypothesized that baseline serum prealbumin correlates independently with health-related quality of life (QoL) and death and that its change over time is a robust mortality predictor. DESIGN: Associations and survival predictability of serum prealbumin at baseline and its changes over 6 mo were examined in a 5-y (2001-2006) cohort of 798 patients receiving MHD. RESULTS: Patients with serum prealbumin >or= 40 mg/dL had greater mid-arm muscle circum

Sensing intracellular oxygen using near-infrared phosphorescent probes and live-cell fluorescence imaging.

The development and application of a methodology for measurement of oxygen within single mammalian cells are presented, which employ novel macromolecular near infrared (NIR) oxygen probes based on new metalloporphyrin dyes. The probes, which display optimal spectral characteristics and sensitivity to oxygen, excellent photostability, and low cytotoxicity and phototoxicity, are loaded into cells by simple transfection procedures and subsequently analyzed by high-resolution fluorescence microscopy. The methodology is demonstrated by sensing intracellular oxygen in different mammalian cell lines,

Estimation of regional cerebral blood flow distribution in infants by near-infrared topography using indocyanine green.

Near-infrared topography with indocyanine green was used to measure regional cerebral blood flow (rCBF) in the temporal lobes of infants. The mean rCBF in infants without neural abnormality was 14.5 +/- 3.1 ml/100 g/min, and the rCBFs in the fronto-temporal, temporal, and occipito-temporal regions were 15.1 +/- 3.9, 15.4 +/- 3.3, and 14.6 +/- 3.3 ml/100 g/min, respectively. Moreover, in one asphyxiated infant with infarction and one infant with subdural and intracerebellar hemorrhage, it was demonstrated that the area of defective blood flow could be detected as well as it can by SPECT. The effect of inhaled nitric oxide on splanchnic perfusion in a neonatal piglet model.

BACKGROUND: Inhaled nitric oxide (iNO) is a commonly used pulmonary vasodilator in neonates, but its effects on intestinal perfusion remain unclear. This study assessed the impact of iNO on splanchnic tissue oxygenation as a surrogate for intestinal perfusion using near infrared spectroscopy (NIRS), with NG-nitro-L-arginine methyl ester (L-NAME) used to inhibit endogenous nitric oxide synthase (NOS) and isolate the effect of exogenous NO. METHODS: Seventeen piglets (7-10 days old) were anesthetized and ventilated with NIRS probes placed on the forehead and abdomen. Piglets were allocated to g

Electronic and steric control of meso-tetraaryl porphycene tautomerism via regioselective β-functionalization.

Herein, we establish the first synthetic strategy for the direct β-C-H nitration of a free base meso-tetraaryl porphycene using HNO3 in CHCl3 in a selective and controlled manner. The reaction affords two regioisomers of the β-nitro porphycene (regioselectivity upto β : β' = 3 : 7) without the formation of any over-nitrated products or compromising the integrity of the macrocycle. Beyond synthetic access, β-functionalization enables precise electronic and steric modulation of the porphycene macrocycle, resulting in distinct optical properties, including enhanced Q-band absorption and red-shift

MGAM-FusionNet: A masked reconstruction and multi-scale attention fusion network for small-sample near-infrared spectral quantification of echinacoside in Cistanche deserticola.

Echinacoside is a major bioactive constituent and an important quality marker of Cistanche deserticola Y. C. Ma. Conventional high-performance liquid chromatography (HPLC) provides accurate reference measurements but is time-consuming and destructive, limiting its use for rapid quality assessment. Near-infrared (NIR) spectroscopy offers a rapid, non-destructive alternative; however, severe band overlap and matrix-related variation can compromise the generalization of chemometric and deep learning models when only small datasets are available. To address these challenges, we developed MGAM-Fusi

Magnetization Dynamics-Modulated Near-Infrared Mie Scattering Enables Homogeneous Femtomolar Immunosensing for Whole Blood.

Low-abundance biomarkers in whole blood hold immense clinical value for disease diagnosis. However, their detection remains constrained by the difficulty of conventional assays in simultaneously achieving high sensitivity, rapid turnaround, and operational simplicity. Here we introduce optomagnetic particle spectroscopy (OptoMPS), a homogeneous biosensing technology for magnetic label-based immuno-agglutination assays, which overcomes these limitations via the integration of nonlinear magnetization dynamics and orientation-dependent Mie scattering of magnetic particles. Under an optimally bias

MEP-Guided Scalp Acupuncture Combined with Repetitive Transcranial Magnetic Stimulation for Post-Stroke Upper Extremity Motor Impairment: A Randomized Controlled Study Protocol.

BACKGROUND: Post-stroke upper extremity motor impairment poses a significant challenge to rehabilitation. While repetitive transcranial magnetic stimulation (rTMS) and scalp acupuncture are effective individually, the precise mechanisms and synergistic efficacy of their combination remain to be fully elucidated. PURPOSE: This study aims to evaluate the therapeutic efficacy of MEP-guided scalp acupuncture combined with rTMS versus rTMS alone. Additionally, it intends to investigate the underlying neuromodulation mechanisms using functional near-infrared spectroscopy (fNIRS). METHODS: This pro

Cross-paradigm fNIRS brain activity in 1-month-old infants across The Gambia and the United Kingdom.

SIGNIFICANCE: Neonates undergo rapid development, yet the examination of emerging brain markers across paradigms, cognitive domains, and diverse global populations remains limited. AIM: We investigated whether brain responses at 1 month of age could be interrogated across paradigms to offer deeper context-specific insights into neurodevelopment. APPROACH: Functional near-infrared spectroscopy was used to assess frontal and temporal brain responses during natural sleep in 181 infants from a low-income setting (rural Gambia) and 58 infants from a higher-income setting (Cambridge, United Kingdo

Prefrontal oxygenation during verbal fluency in severe obesity.

Obesity is a highly prevalent chronic disease with major medical, psychosocial, and economic consequences worldwide. Beyond its association with metabolic and cardiovascular comorbidities, obesity is increasingly linked to impairments in executive functioning and alterations in prefrontal control networks. Neuropsychological and neuroimaging studies consistently indicate deficits in inhibition, cognitive flexibility, and executive control in overweight and obese individuals, accompanied by context-dependent changes in prefrontal activation under cognitive load. These findings underscore the re
