Liver resection combined with excision of vena cava.

BACKGROUND: This study was designed to review our experience with combined partial hepatectomy and vena caval replacement for primary and metastatic liver tumors. STUDY DESIGN: The medical records of all the patients who underwent liver resection and excision of the vena cava over a period of 13 years and 4 months at a single institution were analyzed. The types of tumors fell into four categories: 1) metastatic, 2) primary leiomyosarcoma of the inferior vena cava, 3) tumors with direct extension to the liver, and 4) cholangiocarcinoma. RESULTS: The perioperative mortality was 11% related to

Surgical treatment of infective endocarditis complicated by annular infection and cerebral infarction.

The surgical treatment of nine patients with infective endocarditis (IE) complicated by annular infection and five with IE complicated by cerebral infarction is described herein. In those with annular infection, after thorough débridement of the infected tissues, valve replacement was performed at the original position in five, at the supraannular position in three, and one underwent a translocation procedure. Aortic valve replacement was able to be performed at the original position in two patients by closing the defect at the aortic annulus with a patch after through débridement. The five pa

Use of immunoscintigraphy in the diagnosis of fever of unknown origin.

Fever of unknown origin (FUO) has been defined as an elevation in temperature (38 degrees C) for at least 2-3 weeks despite intensive invest … 52%. False-negative scans were especially noted in patients with endocarditis, pneumonia and small brain abscesses, where the lesions did not exceed a diameter of 0.5 cm. If patients with endocarditis were excluded, the imaging sensitivity and specificity were increased to 57% and 95%. This study demonstrates that 99mTc-anti-NCA-95 scanning is able to localize …

Characteristics of experimental Candida albicans infection of the central nervous system in rabbits.

Different concentrations (10(7), 10(5), 10(3) cfu/ml) of Candida albicans were injected intracisternally in rabbits. The highest inoculum was fatal within 14 h in all animals. In recipients of 10(5) and 10(3) cfu/ml inocula, the mean +/- SD peak cerebrospinal fluid (CSF) tumor necrosis factor-alpha (TNF alpha) concentrations were 1.6 +/- 2.42 and 0.3 +/- 0.59 ng/ml, respectively, at 6 h; the mean +/- SD CSF leukocyte and protein concentrations were 6291 +/- 6515 and 453 +/- 674 cells/mm3 (at 24 h) and 118 +/- 90 and 109 +/- 122 mg/dl (at 12 and 24 h), respectively. At 6-10 days after inoculati

[Analysis of the autopsy data on 48 cases of infectious endocarditis in patients over 14].

Forty eight cases with infections endocarditis (IE) are analyzed--27 males and 21 females (75% at the age of 31-60). Only in 12.5% IE has developed on intact valvular apparatus. In 83.4% the preceding valvular pathology is rheumatic heart defect (RHD), and congenital cardiac malformations and valvular dysplasia--8.3% each. The mitral-aortic valvular defects predominate in RHD, followed by aortic, tricuspid and mitral ones. In 83.3% X IE is with valvular localization, in 4.2%--parietal and in 12.5%--on valvular prostheses. Vegetations are highly specific for valvular IE (100%) as well as ulcera

The ultrasonographic diagnosis of bacterial meningitis and ventriculitis in infancy: six case reports.

Five infants with meningitis and ventriculitis, and a sixth patient with meningitis only are reported. In one hydrocephalic infant, infection of the central nervous system (CNS) was not suspected until cerebral ultrasonography revealed features of ventriculitis. It appears that in non-communicating hydrocephalus managed with a ventriculo-peritoneal (VP) shunt, infection may involve predominantly the "sequestered ventricles" rather than the spinal meninges. In four infants, bacterial meningitis had been proven but ventriculitis was not diagnosed until cerebral ultrasonography was performed. In

Correlative Magnetic Resonance Imaging and Histopathology in Small Ruminant Listeria Rhombencephalitis.

Background: Listeria rhombencephalitis, infection of the brainstem with Listeria monocytogenes, occurs mainly in humans and farmed ruminants and is associated with high fatality rates. Small ruminants (goats and sheep) are a large animal model due to neuropathological similarities. The purpose of this study was to define magnetic resonance imaging (MRI) features of listeria rhombencephalitis in naturally infected small ruminants and correlate them with histopathology. Secondly, the purpose of this study was to compare the results with MRI findings reported in humans. Methods: Twenty small rumi

Hypophysitis in surgical and autoptical specimens.

We present the clinical and histological findings of 11 cases of inflammatory anterior pituitary lesions, 8 of which were obtained during surgery and 3 of which were obtained from autopsies. Additionally, we extended the conventional classification of pituitary inflammatory disease by the new entity " secondary hypophysitis". Of the surgically obtained specimens 5 consisted of inflammatory extension into the pituitary gland out of the surrounding tissue. In all of these patients the inflammation originated from an additional tumor in the sellar region (4 craniopharyngiomas, 1 prolactinoma). Th

[Medicosurgical approach of intracranial pyogenic bacterial suppurations in Abidjan. Analysis of 26 clinical cases].

We report 19 cases of cerebral abscesses and 7 cases of subdural empyemas in 5 months to 63 years old patients (1983-1991). Bacteriological agents in 13 cases are. 6 Staphylococcus aureus, 5 negative Gram bacilli, 1 Streptococcus, 1 pneumococcus. Predisposing factors are sepsis of adjacent organs (42%), septicemia (19%), post-traumatic and post-surgical infections (19%). Lethality is 11.5%. The authors stress the interest of CT scan and bacteriological studies now available in Abidjan with the creation of the Department of Neurosurgery.

[Potential nosocomial disseminated infection due to Nocardia asteroides after a prosthesis insertion in an immunocompetent patient].

Nocardia infections are rare and usually occurred in immunocompromised patients with systemic dissemination from a lung infection. We report a case of an immunocompetent patient in whom Nocardia asteroides had cause psoas and cerebral abcess without pulmonary infection, a short period after a hip prosthesis insertion. The clinical history is highly suggestive of a hospital-acquired infection.

Septic metastatic endophthalmitis from Klebsiella pneumoniae liver abscess: CT and MR imaging characteristics--report of three cases.

Computed tomography (CT) and magnetic resonance (MR) imaging were performed in three diabetic patients with endophthalmitis and liver abscess secondary to Klebsiella pneumoniae infection. Ocular abnormalities included early uveoscleral thickening, fulminant exudative vitreous humor, and late phthisis bulbi. Characteristic imaging findings of endophthalmitis in diabetic patients with liver abscess should raise a high index of suspicion for K pneumoniae infection.

Interventional Management of Pulmonary Arteriovenous Malformations: Imaging Primer.

Pulmonary arteriovenous malformations (PAVMs) are abnormal connections between the pulmonary arteries and veins, bypassing the capillary network and exposing patients to serious complications such as stroke, brain abscess, and hemothorax. The estimated prevalence of PAVMs is approximately one in 2630 individuals, with hereditary hemorrhagic telangiectasia accounting for over 80% of cases. PAVMs are classified as simple, complex, or diffuse based on their angioarchitecture. Diagnostic evaluation involves transthoracic contrast-enhanced echocardiography for screening, chest CT for confirmation, …

Percutaneous Transcatheter Embolization for Recurrent Cerebral Embolism Due to Pulmonary Arteriovenous Malformation.

BACKGROUND Pulmonary arteriovenous malformations (PAVMs) are rare structural vascular abnormalities that create communications between pulmonary arteries and veins, resulting in right-to-left shunts. PAVMs are often asymptomatic; however, patients may exhibit a range of clinical symptoms, including dyspnea, hemoptysis, chest pain, and cyanosis. Common serious complications include cerebrovascular events (eg, paradoxical embolism resulting in stroke or transient ischemic attack) and infectious complications (eg, brain abscess). PAVMs do not always require treatment; management should be guided …

The differential diagnoses included cystic meningioma, hemangioblastoma, metastatic tumor, brain abscess, and chronic subdural hematoma. Both patients underwent frontotemporal craniotomy, and the first case revealed a cyst containing xanthochromic fluid and distinct mural nodules attached to the dura. In the second case, the cystic component contained …

Re-examining the Diagnostic Criteria for Wilson's Disease: A Case Report and Literature Review.

Wilson's disease (WD) is a heterogeneous genetic disorder for which diagnosis is challenging. We present the case of a 54-year-old woman with a complicated medical history, including hepatitis C, cirrhosis, hepatic encephalopathy, and extensive psychiatric disease, who was transferred to our hospital for management of a spinal epidural abscess. Further findings suggested undiagnosed WD, including bilateral rings around Descemet's membrane and modestly low ceruloplasmin but normal urinary copper excretion. Many algorithms have been proposed for Wilson disease diagnosis, including clinical, labo

Endovascular Embolisation of Pulmonary Arteriovenous Malformation Using Amplatzer Vascular Plugs.

Pulmonary arteriovenous malformation (PAVMs) are abnormal communications between pulmonary arteries and veins. The rarity of their occurrence, coupled with the risks they pose, including brain abscess, embolic stroke, and myocardial infarction, mandates that they should not be overlooked in the differential diagnosis of patients presenting with haemoptysis, dyspnea, clubbing, cyanosis, hypoxemia, or epistaxis. We present the case of a 41-year-old local female who presented to our hospital as an outpatient with decreased oxygen saturation (SpO2) of 70%-80% for the past two years with a final di…
