Between 1988 and 1992, 54 patients with complicated tibial non-union were treated by tibio-fibular fusion. In 6 cases of comminuted fracture the operation was performed within the first 2 months, especially in the case of distal fractures. Fifteen patients suffered from aseptic tibial pseudarthrosis, and further 6 from an additional defect. In 25 patients treatment for infected non-union was needed and in 3 cases tibio-fibular synostosis was induced after segmental transport and non-docking. Most of the patients had conspicuous soft tissue lesions, and 14 required transplantation of a regional

BACKGROUND: We report the long-term results of a cohort of patients after radial head replacement with a bipolar design and a smooth cementless stem at a mean follow-up of 10.4 years.
METHODS: Of 17 possible patients from a previous minimum 2-year follow-up study, 16 were available for review. Patients were assessed using clinical and radiographic examination and with standardized outcome measures. Range of motion, stability, and radiographic evaluation of implant loosening and joint degeneration were assessed. Comparisons were performed using the Wilcoxon signed rank test for unequal groups.

Pathology of the sesamoid bones have been rarely described in the literature especially those of the index. We present the case of a 45 years old amateur tennis player who presented with pain on the radial border of the index finger at the level of the metacarpophalangeal joint. The clinical examination and the operative findings allow us to conclude that the symptoms were due to tendinitis of the flexors of the index associated with fracture of the sesamoid and cartilaginous degenerative changes. We also present a review of the literature.

PURPOSE: Direct articular surface visualization may improve reduction accuracy in intra-articular distal radius fractures. A ligament-sparing volar radiocarpal arthrotomy has been described to complement the standard volar approach, but the articular surface area (SA) visualized has not been compared to the traditional dorsal radiocarpal arthrotomy. This study quantified and compared articular SA visualization through volar and dorsal ligament-sparing radiocarpal arthrotomies in a cadaveric model.
METHODS: Twenty-four fresh-frozen cadaveric specimens (12 matched pairs) underwent either a vola

OBJECTIVES: Intramedullary nailing (IMN) is the standard treatment for tibial shaft fractures; however, postoperative malalignment remains a relevant complication. Deviations in distal nail position can influence tibial alignment, and eccentric drilling during distal interlocking screw placement may alter fracture reduction and length. The purpose of this study was to quantify the effect of intentionally eccentric distal interlocking screw placement on tibial alignment in a controlled cadaveric model.
METHODS: Ten paired, formalin-fixed, human-cadaveric tibias underwent standardized IMN via s

PURPOSE: To determine whether early radiographic trajectory data can predict the timing of stability in conservatively managed distal radius fractures (DRFs) and to identify which clinical and radiographic features most influence stabilization timing.
METHODS: This retrospective cohort study analyzed data from 1,585 adult patients with conservatively managed DRFs collected from a single-institution fracture registry (2020-2024). Radiographic stability was classified according to predefined parameters of radial height, volar tilt, radial inclination, and ulnar variance at weekly follow-ups. An

BACKGROUND: Greater tuberosity (GT) fractures associated with shoulder dislocation carry a high risk of secondary displacement, even when initial postreduction alignment appears acceptable. However, the specific factors predicting such displacement remain unclear. The aim of the study was to investigate risk factors associated with displacement of GT fragments following anterior shoulder dislocation.
METHODS: This retrospective study included 52 patients (20 males, 32 females; mean age, 71.5 years) with anterior shoulder dislocation and concomitant GT fractures. Following closed reduction, pa

OBJECTIVES: Managing 33-C3 femur fractures with medial wall bone defects poses a significant challenge for orthopedic surgeons. The gold standard treatment for arbeitsgemeinschaftfür osteosynthesefragen (AO)/orthopedic trauma association (OTA) 33-C3 distal femur fractures with medial wall bone defects remains elusive. This study employs finite element analysis to compare the stability and mechanical behavior of 3 internal fixation patterns (single lateral distal locking plate, retrograde intramedullary nail, and dual plates) for 33-C3 femur fractures with medial wall bone defects. INTRODUCTION: Four-part fracture dislocations of the proximal humerus occurring in young age are extremely difficult fractures with a high incidence of complications. The risk of avascular necrosis is high; hence, prosthetic replacement is the treatment of choice in older patients with these complex fractures; on the other hand, the longevity of the prosthesis is the main concern in young age. Thus, every effort should be made to fix these fractures in the young. The purpose of this study is to evaluate the results of fixation in a series of young patients with four-part fracture dislocations;

BACKGROUND: Fracture care in obese patients is becoming an everyday problem because the prevalence of obesity in European countries has tripled since the last 20 years.
PATIENTS AND METHOD: With the use of a custom made Ilizarov ring fixator with a ring diameter of 300 to 340 mm, fracture stabilization in three morbidly obese patients was performed. The patients' body mass index (BMI) ranged from 59 to 89. There were one proximal tibia fracture and two pilon fractures. The tibia fracture was stabilized with a 340-mm frame and the pilon fractures were stabilized by primary ankle arthrodesis wi

INTRODUCTION: Treatment of lower extremity segmental bone loss is difficult. Masquelet et al. proposed a two-stage technique: first, debridement and filling of bone loss with an acrylic spacer; second, bone reconstruction by filling with cancellous bone in the space left free (following cement removal) inside the so-called self-induced periosteal membrane. In the originally described technique, the fracture site is stabilized by an external fixator, which remains in place throughout the bone healing process, i.e., often longer than 9 months with all the known disadvantages of this type of asse

BACKGROUND: Pediatric diaphyseal femoral fracture (PDFF) is one of the most common injuries requiring hospitalization. Elastic stable intramedullary nailing (ESIN) is commonly used for PDFFs in ages 5 to 11. The optimal treatment method for length unstable PDFF is a subject of ongoing debate. This study aimed to compare clinical, radiographic, and pedobarographic outcomes of ESIN between length stable and unstable PDFF.
METHODS: We retrospectively reviewed patients undergoing ESIN treatment for isolated PDFF between 2016 and 2021. Exclusion criteria were (1) history of ipsilateral or contrala
