Abdominal solid organ injury in trauma patients with pelvic bone fractures.

BACKGROUND: We analyzed the clinical progression of trauma patients with pelvic bone fractures so to determine the risk factors associated with sustaining concurrent abdominal solid organ injuries. METHODS: This study was a retrospective chart review. Subjects were categorized based on injury type: solid organ versus non-solid organ injury groups. These study groups were compared based on demographics, treatments, and clinical outcomes. Potential risk factors that may contribute to the occurrence of abdominal solid organ injury in trauma patients with pelvic bone fractures were evaluated. RE

Conservative management of an abdominal gunshot injury with a peritoneal breach: wisdom or absurdity?

Surgical exploration has been the standard of care for abdominal gunshot injuries. The authors report a case of a 28-year-old man who sustained a transabdominal gunshot injury, which entered the anterior abdominal wall and exited adjacent to the T12 vertebra posteriorly with a tangential trajectory. On presentation, the patient was haemodynamically stable with no peritoneal signs. Based on trajectory of the bullet, intra-abdominal injury was suspected. Therefore a CT scan abdomen with intravenous and rectal contrast was performed. The CT scan revealed no extravasation of the rectal contrast bu

Abdominal crush injury in the Sichuan earthquake evaluated by multidetector computed tomography.

AIM: To investigate the features of abdominal crush injuries resulting from an earthquake using multidetector computed tomography (MDCT). METHODS: Fifty-one survivors with abdominal crush injuries due to the 2008 Sichuan earthquake underwent emergency non-enhanced scans with 16-row MDCT. Data were reviewed focusing on anatomic regions including lumbar vertebrae, abdominal wall soft tissue, retroperitoneum and intraperitoneal space; and types of traumatic lesions. RESULTS: Fractures of lumbar vertebrae and abdominal wall soft tissue injuries were more common than retro- and intraperitoneal in

Traumatic abdominal wall hernia secondary to handlebar injury: a case report.

A traumatic abdominal wall hernia (TAWH) caused by a handlebar collision is very rare. It is difficult for the emergency physician to exclude the possibility of mesenteric or intestinal injuries when viewing handlebar trauma. Because TAWH may have no other clinical symptoms, early recognition and differentiation from haematoma is important. An unusual case of TAWH caused by a handlebar injury is reported, where a multi-layer muscular defect was found along with a segment of incarcerated jejunum and omentum. The latest literature is reviewed, and valuable ideas on the management of blunt abdomi

[An experience of intraoperative blood salvage in emergency laparotomy for severe abdominal injury].

A 34-year-old male patient, who had fallen from a balcony suffering liver injury, underwent emergency laparotomy for right liver lobectomy and portal vein repair. For the first two hours of operation, the blood loss exceeded 12,000 ml, and his hemoglobin level dropped to 2.6 g x dl(-1) despite administration of 30 units of packed red cells (MAP). At this point, no more MAP was available in our hospital. Then we decided to initiate intraoperative blood salvage in order to minimize the further loss of hemoglobin. For 26,200 ml of the total blood loss, 1160 ml of packed red cells were restored fr

Delayed presentation of porta hepatis injury following blunt abdominal trauma.

A 73 year old lady developed abdominal pain, anaemia and obstructive jaundice 18 days after a road traffic accident. The jaundice was due to compression of the biliary confluence by a haematoma which was caused by a laceration of the left portal vein. The portal vein was repaired (lateral venorrhaphy) and post-operative recovery was uncomplicated. Porta hepatis injuries are difficult to diagnose and delayed presentation is not uncommon. Significant morbidity and mortality may ensue if aggressive management is not adopted.

Relevance of intra-abdominal pressure monitoring in non-operative management of patients with blunt liver and splenic injuries.

PURPOSE: Non-operative management (NOM) has been validated for blunt liver and splenic injuries. Literature on continuous intra-abdominal pressure (IAP) monitoring as a part of NOM remains to be equivocal. The study aimed to find any correlation between clinical parameters and IAP, and their effect on the NOM of patients with blunt liver and splenic injury. METHOD: A prospective cross-sectional study conducted at a level I trauma center from October 2018 to January 2020 including 174 patients who underwent NOM following blunt liver and splenic injuries. Hemodynamically unstable patients or th
